      Effects of mechanical-bed massage on exercise-induced back fatigue in athletes

          Abstract

          [Purpose] The study aimed to preliminarily investigate the effects of mechanical-bed massage on exercise-induced back fatigue in athletes. [Subjects and Methods] Twelve male college athletes, randomly allocated to experimental or control groups, were instructed to perform reverse sit-up for 8 sessions until they became fatigued. The experimental group received a 20-min mechanical-bed massage session, while the control group rested on a bed for the same period of time. Visual Analogue Scale (VAS) on perceived back muscle fatigue, back muscle endurance, and Heart Rate Variability (HRV) parameters including stress index (SI), HRV index, SDNN, RMSSD, pNN50, LF, HF, and LF/HF were analyzed. [Results] Immediately and 24 hours after the intervention, the VAS significantly differed between the groups. Experimental group’s HF was significantly higher immediately after the intervention than control group. Experimental group’s LF and LF/HF were significantly lower immediately after the intervention than the control group. [Conclusion] Mechanical bed massage may help athletes overcome the subjective feelings of exercise-induced fatigue, modulate the automatic nervous system activity, especially for balancing sympathetic and parasympathetic activities. Therefore, mechanical bed massage may facilitate recovery from muscle and central fatigue after athlete training or competition.

          Measurement of human muscle fatigue.

          Human muscle fatigue has been studied using a wide variety of exercise models, protocols and assessment methods. Based on the definition of fatigue as 'any reduction in the maximal capacity to generate force or power output', the different methods to measure fatigue are discussed. It is argued that reliable and valid measures must include either assessment of maximal voluntary contraction force or power, or the force generated by electrical stimulation. By comparing tetanic stimulation and maximal voluntary contraction force one may reveal whether fatigue is of central origin, or whether peripheral mechanisms are involved. Adequate use of twitch interpolation provides an even more sensitive measure for central fatigue. Indirect methods as endurance times and electromyography show variable responses during exercise and no close relationship to fatigue. Hence these methods are of limited value in measurement of human muscle fatigue.

            The mechanisms of massage and effects on performance, muscle recovery and injury prevention.

            Many coaches, athletes and sports medicine personnel hold the belief, based on observations and experiences, that massage can provide several benefits to the body such as increased blood flow, reduced muscle tension and neurological excitability, and an increased sense of well-being. Massage can produce mechanical pressure, which is expected to increase muscle compliance resulting in increased range of joint motion, decreased passive stiffness and decreased active stiffness (biomechanical mechanisms). Mechanical pressure might help to increase blood flow by increasing the arteriolar pressure, as well as increasing muscle temperature from rubbing. Depending on the massage technique, mechanical pressure on the muscle is expected to increase or decrease neural excitability as measured by the Hoffman reflex (neurological mechanisms). Changes in parasympathetic activity (as measured by heart rate, blood pressure and heart rate variability) and hormonal levels (as measured by cortisol levels) following massage result in a relaxation response (physiological mechanisms). A reduction in anxiety and an improvement in mood state also cause relaxation (psychological mechanisms) after massage. Therefore, these benefits of massage are expected to help athletes by enhancing performance and reducing injury risk. However, limited research has investigated the effects of pre-exercise massage on performance and injury prevention. Massage between events is widely investigated because it is believed that massage might help to enhance recovery and prepare athletes for the next event. Unfortunately, very little scientific data has supported this claim. The majority of research on psychological effects of massage has concluded that massage produces positive effects on recovery (psychological mechanisms). Post-exercise massage has been shown to reduce the severity of muscle soreness but massage has no effects on muscle functional loss. Notwithstanding the belief that massage has benefits for athletes, the effects of different types of massage (e.g. petrissage, effleurage, friction) or the appropriate timing of massage (pre-exercise vs post-exercise) on performance, recovery from injury, or as an injury prevention method are not clear. Explanations are lacking, as the mechanisms of each massage technique have not been widely investigated. Therefore, this article discusses the possible mechanisms of massage and provides a discussion of the limited evidence of massage on performance, recovery and muscle injury prevention. The limitations of previous research are described and further research is recommended.

              Massage therapy research review.

              Moderate pressure massage has contributed to many positive effects including increased weight gain in preterm infants, reduced pain in different syndromes including fibromyalgia and rheumatoid arthritis, enhanced attentiveness, reduced depression and enhanced immune function (increased natural killer cells and natural killer cell activity).Surprisingly, these recent studies have not been reviewed, highlighting the need for the current review. When moderate and light pressure massage have been compared in laboratory studies, moderate pressure massage reduced depression, anxiety and heart rate, and it altered EEG patterns, as in a relaxation response. Moderate pressure massage has also led to increased vagal activity and decreased cortisol levels. Functional magnetic resonance imaging data have suggested that moderate pressure massage was represented in several brain regions including the amygdala, the hypothalamus and the anterior cingulate cortex, all areas involved in stress and emotion regulation. Further research is needed to identify underlying neurophysiological and biochemical mechanisms associated with moderate pressure massage. Copyright © 2014 Elsevier Ltd. All rights reserved.
