          Abstract

          Resumen Introducción: La actividad de la enfermedad en los pacientes con artritis reumatoide (AR) es frecuentemente determinada utilizando el índice compuesto DAS28. Al utilizar variables subjetivas, la actividad inflamatoria determinada por este índice puede afectarse por factores no inflamatorios y ser sobrevalorada. El DAS28 y sus componentes subjetivos han sido poco estudiados en sudamericanos. Objetivos: Analizar el componente subjetivo del DAS28 y las variables asociadas a él en dos poblaciones sudamericanas. Metodología: Estudio retrospectivo de pacientes con AR del registro BIOBADAGUAY que incluye pacientes de Paraguay y Uruguay bajo tratamiento biológico. Se determinó la fórmula para el componente subjetivo del DAS28 (sDAS28) y se analizaron sus valores al inicio del tratamiento y a la semana 52, así como la asociación de los mismos con variables clínicas y serológicas. Resultados: La población paraguaya registró valores menores de edad, número decomorbilidades asociadas, tabaquistas, DAS28, EVAP, NAD que la población uruguaya. La procedencia, el IMC, el número de comorbilidades, el tabaquismo y uso de metotrexato se asociaron con el sDAS28 al inicio del tratamiento. El sDAS28 al inicio del tratamiento, fue el factor con mayor asociación a la variación del sDAS28 la semana 52 de seguimiento. No se obtuvo asociación con variables objetivas (i.e. PCR, VSG, y número de tratamientos asociados). Conclusiones: Variables no asociadas a la actividad inflamatoria de la enfermedad pueden influir sobre el sDAS28 y el DAS28 de pacientes sudamericanos. El manejo de los mismos debe ser individualizado y no basado únicamente en el valor del DAS28.

          Translated abstract

          Abstract Introduction: In patients with rheumatoid arthritis (RA), disease activity is often determined through the DAS28 index. Subjective components included in the index can be influenced by non-inflammatory factors, and overestimate disease activity. The DAS28 index and its subjective components have not been extensively studied in South Americans. Objectives: Analyze the subjective components of DAS28 as well as clinical and serological variables related to it in South American patients with rheumatoid arthritis treated with biologics. Methods: Retrospective longitudinal analysis of patients from BIOBADAGUAY registry. The formula for the subjective component of DAS28 was determined. DAS28 and its components were analyzed for both populations at treatment initiation, and 52 week follow up, and associations with clinical and serological variables were evaluated. Results: The Paraguayan patients had lower mean age, number of comorbidities and less smokers. They were also found to have lower DAS28, sDAS28, PGH and TJC. Country of origin, number of comorbidities, BMI, smoking history and methotrexate use were associated to subjective components of DAS28 at treatment initiation. Changes in sDAS28 at week 52 were highly determined by its initial value. No association was found with objective variables (CRP, ESR, number of treatments.) Conclusions: Variables other than inflammatory disease activity can influence sDAS28 and DAS28 of South American patients. Their management must be individualized and not be based solely on DAS28 value.

          Smoking and Rheumatoid Arthritis

          Rheumatoid arthritis (RA) is a chronic inflammatory disease caused by both genetic and environmental factors. Smoking has been implicated as one of the most important extrinsic risk factors for its development and severity. Recent developments have shed light on the pathophysiology of RA in smokers, including oxidative stress, inflammation, autoantibody formation and epigenetic changes. The association of smoking and the development of RA have been demonstrated through epidemiologic studies, as well as through in vivo and animal models of RA. With increased use of biological agents in addition to standard disease-modifying antirheumatic drugs (DMARDs), there has been interest in how smoking affects drug response in RA treatment. Recent evidence suggests the response and drug survival in people treated with anti-tumour necrosis factor (anti-TNF) therapy is poorer in heavy smokers, and possible immunological mechanisms for this effect are presented in the current paper.

              Racial and ethnic disparities in disease activity and function among persons with rheumatoid arthritis from university-affiliated clinics.

              Health outcomes in rheumatoid arthritis (RA) have improved significantly over the past 2 decades. However, research suggests that disparities exist by race/ethnicity and socioeconomic status, with certain vulnerable populations remaining understudied. Our objective was to assess disparities in disease activity and function by race/ethnicity and explore the impact of language and immigrant status at clinics serving diverse populations.
