          Abstract

          RESUMEN Las lesiones por humedad representan un importante y grave problema en personas encamadas durante periodos de tiempo prolongados. En las unidades de cuidados intensivos, dichas lesiones suponen un importante reto para los profesionales de enfermería. Su prevención y tratamiento temprano con apósitos específicos es una tarea importante para disminuir la morbilidad del enfermo en cuidados intensivos.

          ABSTRACT Moisture injuries represent an important and serious problem in people bedridden for prolonged periods of time. In intensive care units, these injuries represent an important challenge for nursing professionals. Its prevention and early treatment with specific dressings is an important task to reduce the morbidity of the patient in intensive care.

          Effectiveness of an alternating pressure air mattress for the prevention of pressure ulcers.

          studies of the effectiveness of alternating pressure air mattresses (APAMs) for the prevention of pressure ulcers are scarce and in conflict. evaluating whether an APAM is more or equally effective as the standard prevention. randomised controlled trial. patients admitted to 19 surgical, internal, or geriatric wards in seven Belgian hospitals were included if they were in need of prevention of pressure ulcers. To define this need, two methods were used randomly: the Braden Scale or the presence of non-blanchable erythema (NBE). 447 patients were randomised into either an experimental or a control group. In the experimental group, 222 patients were lying on an APAM (Alpha-X-Cell, Huntleigh Healthcare, UK). In the control group, 225 patients were lying on a visco-elastic foam mattress (Tempur, Tempur-World Inc., USA) in combination with turning every 4 hours. Both groups had identical sitting protocols. there was no significant difference in incidence of pressure ulcers (grade 2-4) between the experimental (15.6%) and control group (15.3%) (P = 1). There were significantly more heel pressure ulcers in the control group (P = 0.006). There was an interaction effect between the risk assessment method and preventive measures for the development of all pressure ulcers and sacral pressure ulcers. fewer patients developed heel pressure ulcers on an APAM. Patients identified as being in need of prevention based on the presence of NBE had a tendency to develop fewer pressure ulcers on an APAM. Patients identified as being in need of prevention, based on the Braden Scale, appeared to develop more sacral pressure ulcers on an APAM.

              Redefinición del concepto y del abordaje de las lesiones por humedad: Una propuesta conceptual y metodológica para mejorar el cuidado de las lesiones cutáneas asociadas a la humedad (LESCAH)

              Actualmente se percibe una indefinición manifiesta acerca de la conceptualización y abordaje de las llamadas lesiones por humedad en los entornos clínicos, docentes e investigadores de nuestro país, que muy frecuentemente relacionan este término solo con las lesiones producidas por la orina y las heces. Dadas las diversas repercusiones que se intuye pueden tener este tipo de lesiones, se hace necesario consensuar y unificar los distintos aspectos relacionados con el cuidado de estas heridas. En el presente artículo, y siguiendo la tendencia internacional, se propone la adopción del término lesiones cutáneas asociadas a la humedad (LESCAH) para referirnos a la inflamación y erosión de la piel causada por la exposición prolongada a diversas fuentes de humedad, incluyendo la orina o las heces, el sudor, el exudado de la herida, el moco o la saliva. Desde esta premisa, se presentan y describen los diferentes tipos de LESCAH identificados en la clínica. Y finalmente, para facilitar la integración de la valoración, prevención y tratamiento de los pacientes con algún tipo de LESCAH, se propone un plan básico de actuación -denominado con el acrónimo CASPROT- con los distintos objetivos e intervenciones a realizar en cada fase.
