      Skin cancer screening participation and impact on melanoma incidence in Germany – an observational study on incidence trends in regions with and without population-based screening

          Abstract

          Background:

          The SCREEN (Skin Cancer Research to provide Evidence for Effectiveness of Screening in Northern Germany) project involved population-wide skin cancer screening with whole-body examination by general physicians and dermatologists. It was conducted in the German state of Schleswig-Holstein (July 2003–June 2004), but not in the German state of Saarland.

          Methods:

          The population-based registries of Schleswig-Holstein and Saarland provided data on melanoma incidence before, during, and after SCREEN to assess the association of skin cancer screening with incidence.

          Results:

          Approximately 19% of the Schleswig-Holstein population participated in SCREEN (women: 27%, men: 10%). A total of 52% of all melanomas diagnosed during SCREEN in Schleswig-Holstein were detected as part of the project. Melanoma incidence increased during SCREEN (invasive melanoma in women: +8.9 per 100 000 (95% confidence intervals (CI): 6.1; 11.7); men: +4.0 per 100 000 (95% CI: 1.6; 6.4)) and decreased afterwards (women: −10.6 per 100 000 (95% CI: −13.3; −7.9); men: −4.1 per 100 000 (95% CI: −6.5; −1.7)). Similar changes were not observed in Saarland that had no such project. The differences between the two states were greatest among women, the group with the greater SCREEN participation.

          Conclusion:

          The SCREEN project had a substantial impact on melanoma incidence. This is consistent with the impact of effective screening for other cancers.

          Socio-economic inequalities in breast and cervical cancer screening practices in Europe: influence of the type of screening program.

          The aim of this study was to describe inequalities in the use of breast and cervical cancer screening services according to educational level in European countries in 2002, and to determine the influence of the type of screening program on the extent of inequality. A cross-sectional study was performed using individual-level data from the WHO World Health Survey (2002) and data regarding the implementation of cancer screening programmes. The study population consisted of women from 22 European countries, aged 25-69 years for cervical cancer screening (n =11 770) and 50-69 years for breast cancer screening (n = 4784). Dependent variables were having had a PAP smear and having had a mammography during the previous 3 years. The main independent variables were socio-economic position (SEP) and the type of screening program in the country. For each country the prevalence of screening was calculated, overall and for each level of education, and indices of relative (RII) and absolute (SII) inequality were computed by educational level. Multilevel logistic regression models were fitted. SEP inequalities in screening were found in countries with opportunistic screening [comparing highest with lowest educational level: RII = 1.28, 95% confidence interval (CI) 1.12-1.48 for cervical cancer; and RII = 3.11, 95% CI 1.78-5.42 for breast cancer] but not in countries with nationwide population-based programmes. Inequalities were also observed in countries with regional screening programs (RII = 1.35, 95% CI 1.10-1.65 for cervical cancer; and RII = 1.58, 95% CI 1.26-1.98 for breast cancer). Inequalities in the use of cancer screening according to SEP are higher in countries without population-based cancer screening programmes. These results highlight the potential benefits of population-based screening programmes.

            Recent trends in breast cancer incidence rates by age and tumor characteristics among U.S. women

            Introduction A recent abstract presented in a breast cancer symposium attributed the sharp decrease in female breast cancer incidence rates from 2002 to 2003 in the Surveillance, Epidemiology, and End Results (SEER) cancer registries of the United States to the reduced use of hormone replacement therapy since July 2002. However, this hypothesis does not explain the decrease that began in 1999 in the age-standardized incidence rate of invasive breast cancer in the nine oldest SEER cancer registry areas, although the trend through 2003 was not statistically significant. In this paper, we examine temporal trends in invasive and in situ female breast cancer by age, stage, tumor size, and estrogen receptor/progestin receptor (ER/PR) status in the nine oldest SEER cancer registry areas and consider the implication of these trends in relation to risk factors and screening. Methods We performed a joinpoint regression analysis to fit a series of joined straight lines to the trends in age-adjusted rates and described the resultant trends (slope) by annual percentage change (two-sided, P < 0.05). Results A plot of the age-specific rates of invasive breast cancer shows a decrease in all 5-year age groups from 45 years and above between 1999 and 2003 and sharp decreases largely confined to ER+ tumors in age groups from 50 to 69 years between 2002 and 2003. In joinpoint analyses by tumor size and stage, incidence rates decreased for small tumors (less than or equal to 2 cm) by 4.1% (95% confidence interval [CI], 0.2% to 7.8%) per year from 2000 through 2003 and for localized disease by 3.1% (95% CI, 1.2% to 5.0%) per year from 1999 through 2003. No decrease in incidence was observed for larger tumors or advanced-stage disease during the corresponding periods. Rates for in situ disease were stable from 2000 through 2003 after increasing rapidly since 1981. Conclusion Two distinct patterns are observed in breast cancer trends. The downturn in incidence rates in all age groups above 45 years suggests a period effect that is consistent with saturation in screening mammography. The sharp decrease in incidence from 2002 to 2003 that occurred in women 50 to 69 years old who predominantly, but not exclusively, had ER+ tumors may reflect the early benefit of the reduced use of hormone replacement therapy.

              Inequalities in colorectal cancer screening participation in the first round of the national screening programme in England

              Background: Introduction of organised, population-based, colorectal cancer screening in the United Kingdom using the faecal occult blood test (FOBT) has the potential to reduce overall colorectal cancer mortality. However, socio-economic variation in screening participation could exacerbate existing inequalities in mortality. Methods: This study examined FOBT uptake rates in London, England in relation to area-level socio-economic deprivation over the first 30 months of the programme during which 401 197 individuals were sent an FOBT kit. Uptake was defined as return of a completed test kit within 3 months. Area-level deprivation in each postcode sector was indexed with the Townsend Material Deprivation Index. Analyses controlled for area-level household mobility, ethnic diversity and poor health, each of which was associated with lower return rates. Results: The results showed a strong socio-economic gradient in FOBT uptake, which declined from 49% in the least deprived quintile of postcodes to 38% in the middle quintile and 32% in the most deprived quintile. Variation in socio-economic deprivation between sectors accounted for 62% of the variance in return rates, with little attenuation as a result of controlling for ethnic diversity, household mobility or health status. Conclusion: These results highlight the need to understand the causes of socio-economic gradients in screening participation and address barriers that could otherwise increase disparities in colorectal cancer survival.
