      Photoaging Mobile Apps: A Novel Opportunity for Smoking Cessation?

          Abstract

          Most smokers start smoking during their early adolescence with the idea that smoking is glamorous; the problems related to lung cancer, vascular disease, and chronic pulmonary disease are too far in the future to fathom. In contrast, most adolescents view their image in a mirror as an important component of their personal life. A recent randomized controlled trial by Burford et al published in the Journal of Medical Internet Research demonstrated an increased quit rate of 21% in 18-30-year-old young adults by the help of photoaging desktop programs, in which an image is altered to predict future appearance [1]. Furthermore, the photoaging software has been shown to increase the motivation of 14-18-year-old females to quit [2]. However, the investigated programs only reach a small audience and are not freely available. We took advantage of the widespread availability of mobile phones and adolescents´ interest in appearance to develop a free mobile phone app which requires the user to take a self portrait (ie, a selfie), which is then displayed by the photoaging software as four images: consequences of (non-)smoking one pack a day for a year (Figure 1) or 15 years (Figure 2). Afterwards, the app explains the visual results and offers many sharing options with family and friends. By this means, the social network of the user may also be informed about the various beauty reducing effects of smoking, potential health consequences, and learn about the app. The underlying aging algorithms take into account the user’s current age and are based on publications showing an increased risk for acne and pale skin due to declined capillary perfusion (after one pack-year), as well as connective tissue changes and wrinkles in the longer term (after 15 pack-years) [3,4]. The app has been installed on over 50,000 Android and 27,000 iOS mobile phones within seven months after its release in Germany (10/27/2014 to 4/26/2015). As mobile phone use in Germany declines with age, the largest fraction of the app’s users are assumed to be 30 years or younger. Based on the publication from Burford et al, it is reasonable to speculate that the app could motivate smokers to quit. Taking into account that the smoking prevalence in the general German population is approximately 25% (approximately 19,250 of the 77,000 app users were smokers), about 4000 users (21%) would have quit after using the app. Further research is needed to investigate the effectiveness of app-based photoaging interventions to increase quit rates and to prevent smoking initiation.           Epidemiology of acne in the general population: the risk of smoking.

          Acne is a common skin disorder, but epidemiological data from the general population obtained by examination are scarce. Clinical experience suggests an association between smoking and acne, although confirmatory evidence from appropriate studies is lacking. To determine the prevalence and demographic factors of acne in a general population sample and to investigate the association of smoking and acne on a qualitative and quantitative level. In a cross-sectional study, 896 citizens (aged 1--87 years, median 42) of the City of Hamburg were dermatologically examined. The prevalence and severity of acne were recorded and further information on demographic variables, medical history, and alcohol and cigarette consumption were obtained by a standardized interview. According to the clinical examination, acne was present in 26.8% overall, and was more prevalent in men (29.9%) than women (23.7%) (odds ratio, OR 1.37, 95% confidence interval, CI 1.01--1.87). Prevalence followed a significant linear trend over age with peak prevalence between 14 and 29 years (P < 0.001). The reported age at onset was significantly lower in women than men (P = 0.015). According to multiple logistic regression analyses acne prevalence was significantly higher in active smokers (40.8%, OR 2.04, 95% CI 1.40--2.99) as compared with non-smokers (25.2%). A significant linear relationship between acne prevalence and number of cigarettes smoked daily was obtained (trend test: P < 0.0001). In addition, a significant dose-dependent relationship between acne severity and daily cigarette consumption was shown by linear regression analysis (P = 0.001). Smoking is a clinically important contributory factor to acne prevalence and severity.

            Internet-Based Photoaging Within Australian Pharmacies to Promote Smoking Cessation: Randomized Controlled Trial

            Background Tobacco smoking leads to death or disability and a drain on national resources. The literature suggests that cigarette smoking continues to be a major modifiable risk factor for a variety of diseases and that smokers aged 18-30 years are relatively resistant to antismoking messages due to their widely held belief that they will not be lifelong smokers. Objective To conduct a randomized controlled trial (RCT) of a computer-generated photoaging intervention to promote smoking cessation among young adult smokers within a community pharmacy setting. Methods A trial was designed with 80% power based on the effect size observed in a published pilot study; 160 subjects were recruited (80 allocated to the control group and 80 to the intervention group) from 8 metropolitan community pharmacies located around Perth city center in Western Australia. All participants received standardized smoking cessation advice. The intervention group participants were also digitally photoaged by using the Internet-based APRIL Face Aging software so they could preview images of themselves as a lifelong smoker and as a nonsmoker. Due to the nature of the intervention, the participants and researcher could not be blinded to the study. The main outcome measure was quit attempts at 6-month follow-up, both self-reported and biochemically validated through testing for carbon monoxide (CO), and nicotine dependence assessed via the Fagerström scale. Results At 6-month follow-up, 5 of 80 control group participants (6.3%) suggested they had quit smoking, but only 1 of 80 control group participants (1.3%) consented to, and was confirmed by, CO validation. In the intervention group, 22 of 80 participants (27.5%) reported quitting, with 11 of 80 participants (13.8%) confirmed by CO testing. This difference in biochemically confirmed quit attempts was statistically significant (χ2 1=9.0, P=.003). A repeated measures analysis suggested the average intervention group smoking dependence score had also significantly dropped compared to control participants (P<.001). These differences remained statistically significant after adjustment for small differences in gender distribution and nicotine dependence between the groups. The mean cost of implementing the intervention was estimated at AU $5.79 per participant. The incremental cost-effectiveness ratio was AU $46 per additional quitter. The mean cost that participants indicated they were willing to pay for the digital aging service was AU $20.25 (SD 15.32). Conclusions Demonstrating the detrimental effects on facial physical appearance by using a computer-generated simulation may be both effective and cost-effective at persuading young adult smokers to quit.               Aging Images as a Motivational Trigger for Smoking Cessation in Young Women

              Recruiting adolescents into smoking cessation programs has been challenging, and there is a lack of effective smoking cessation interventions for this age group. We aimed to assess whether the approach of using aging images can be used to recruit young, female smokers for a smoking cessation course. In this study, 853 14- to 18-year-old subjects were photographed (2006–2007). After software-aided aging, the images evoked strong emotions, especially in subjects with an advanced motivational stage to quit. Twenty-four percent of current smokers reported that the aging images increased their motivation to quit smoking (pre-contemplation: 8%; contemplation: 32%; and preparation: 71%). In multivariate analyses, the aged images had a high motivational impact to quit smoking that was associated with an increased readiness to stop smoking and the individual’s assessment of the aging images as shocking, but not with the number of previous attempts to quit and the assessment of the pictures as realistic. However, it was not possible to recruit the study population for a smoking cessation course. We concluded that aging images are a promising intervention for reaching young women and increasing their motivation to stop smoking. However, smoking cessation courses may not be appropriate for this age group: none of the recruits agreed to take a cessation course.
