      Case Report: Multidisciplinary discussion and management of a case involving a non-seminomatous germ cell tumor complicated by the presence of cancerous thrombus in the inferior vena cava

          Abstract

          Testicular cancer predominantly manifests in younger males. It demonstrates a favorable response to cisplatin-based chemotherapy and exhibits a promising overall survival. Nevertheless, the emergence of certain rare complications, such as the development of an embolus in the vena cava due to cancer, suggests that the tumor exhibits a high degree of aggressiveness and is associated with a poorer prognosis. We present a case of stage IIIC mixed malignant germinoma with metastases to both lungs, left kidney, right paravascular iliac lymph nodes, periaortic lymph nodes, and an inferior vena cava tumor thrombus. Following Multi-disciplinary Team (MDT) discussion, treatment modalities including surgery and adjuvant chemotherapy were devised, resulting in optimal therapeutic outcomes without complications or significant adverse reactions.

          Testicular germ-cell tumours in a broader perspective.

          The germ-cell tumours are a fascinating group of neoplasms because of their unusual biology and the spectacular therapeutic results that have been obtained in these tumours. Traditionally, this group of neoplasms is presented in an organ-oriented approach. However, recent clinical and experimental data convincingly demonstrate that these neoplasms are one disease with separate entities that can manifest themselves in different anatomical sites. We propose five entities, in which the developmental potential is determined by the maturation stage and imprinting status of the originating germ cell. Recent progress begins to explain the apparent unpredictable development of germ-cell tumours and offers a basis for understanding their exquisite sensitivity to therapy.

            Recent global trends in testicular cancer incidence and mortality

            Abstract Testicular cancer (TCa) has a relatively rare incidence and mortality, but has not been thoroughly evaluated. We analyzed global variations and recent trends in TCa incidence and mortality. Age-standardized rates (ASRs) of TCa incidence and mortality were retrieved from the GLOBOCAN 2012 database. Temporal patterns were assessed using data obtained from the Cancer Incidence in Five Continents (volumes I–X) and World Health Organization Mortality databases. The incidence and mortality trends over the last 10 years were analyzed using join point analysis. Western and Northern Europe had the highest incidence of TCa (ASR = 8.7 and 7.2, respectively), with most countries showing an increase in incidence rates except for China, which had a stable incidence. Incidence rates were markedly increased in Southern European countries (average annual percent change of 6.8% in Croatia and 6.1% in Spain) but were attenuated in western Europe. The highest mortality rates were observed in western Asia (ASR = 0.7), with most countries showing a decrease in mortality. While the incidence of TCa has increased, mortality from TCa has decreased in most countries. More socioeconomically developed countries had a higher incidence of TCa with lower mortality.
