      Vergleich der Wirksamkeit von Gefitinib bei Patienten mit nichtkleinzelligem Lungenkarzinom nach Art der Mutation des epidermalen Wachstumsfaktor-Rezeptors

          Abstract

          Hintergrund: Exon-19-Deletionen und L858R-Punktmutationen sind die häufigsten Mutationen des epidermalen Wachstumsfaktor-Rezeptors (EGFR) beim nichtkleinzelligen Lungenkarzinom (NSCLC); sie sind prädiktiv für bessere klinische Verläufe nach einer Behandlung mit Gefitinib. Das Ziel dieser Studie war es, den Unterschied im Behandlungserfolg mit Gefitinib bei Patienten mit NSCLC je nach Art der aktiven EGFR-Mutation (Exon-19-Deletion oder L858R-Punktmutation) zu ermitteln. Methoden: Wir identifizierten Patienten mit fortgeschrittenem NSCLC, die Träger einer Exon-19-Deletion oder einer L858R-Punktmutation des EGFR waren und mit Gefitinib behandelt wurden. Die klinischen Verläufe wurden ausgewertet. Ergebnisse: Bei den 124 Patienten mit NSCLC und aktiver EGFR-Mutation betrugen das Gesamtansprechen 60,5%, das progressionsfreie Überleben 11,3 Monate und das Gesamtüberleben 27,3 Monate; zwischen den Patienten mit Exon-19-Deletion (61,8%, 11,3 bzw. 32,2 Monate) und denen mit L858R-Punktmutation (58,9%, 9,0 bzw. 27,7 Monate) bestand hierbei kein signifikanter Unterschied. Schlussfolgerung: Dies lässt die Annahme zu, dass zwischen NSCLC-Patienten, die eine Exon-19-Deletion tragen, und solchen mit einer L858R-Punktmutation kein Unterschied im Hinblick auf die klinische Wirksamkeit von Gefitinib besteht.Übersetzung aus Oncology 2014;87:215-223 (DOI: 10.1159/000362603)
