          Abstract

          A presente pesquisa avaliou a efetividade de intervenções interdisciplinares, envolvendo médicos, farmacêuticos e nutricionistas, destinadas à promoção do Uso Racional dos Medicamentos. Trata-se de um estudo de caráter prospectivo e analítico, com uma população de mulheres idosas, onde a efetividade da intervenção foi avaliada de acordo com indicadores de Uso Racional dos Medicamentos preconizados pela Organização Mundial de Saúde. As análises estatísticas foram realizadas utilizando-se do teste t ou one-way ANOVA para variáveis discretas, e o teste de qui-quadrado para avaliação categórica das variáveis. Após a intervenção, houve uma redução média no consumo de medicamentos em relação às consultas da pré-intervenção (p=0,001). Os medicamentos utilizados na terapêutica cardiovascular foram os mais consumidos, o que se encontra em consonância com as doenças autoreferidas pelas idosas. Foi possível verificar que a intervenção interdisciplinar em idosas pôde contribuir para melhoria dos indicadores de Uso Racional dos Medicamentos, em especial os de prescrição.

          Translated abstract

          This research assessed the effectiveness of interdisciplinary interventions involving physicians, pharmacists and nutritionists, aimed at the promotion of the Rational Use of Drugs. It is a study of a prospective and analytical nature conducted with a population of elderly females, where the effectiveness of the intervention was assessed according to indicators of the Rational Use of Drugs established by the World Health Organization. Statistical analysis was performed using the t test or the one-way ANOVA for discrete variables, as well as the chi-square test for categorical analyses. After the intervention, an average reduction in drug consumption was observed in comparison with the research performed prior to the intervention (p=0.001). The drugs used in cardiovascular therapy were those consumed most, which tallies with the diseases mentioned by the elderly. It was seen that interdisciplinary intervention among the elderly can contribute to improve rational drug use indicators, especially prescription drugs.

          Explicit criteria for determining potentially inappropriate medication use by the elderly. An update.

          This study updates and expands explicit criteria defining potentially inappropriate medication use by the elderly. Additional goals were to address whether adverse outcomes were likely to be clinically severe and to incorporate clinical information on diagnoses when available. These criteria are meant to serve epidemiological studies, drug utilization review systems, health care providers, and educational efforts. Consensus from a panel of 6 nationally recognized experts on the appropriate use of medication in the elderly was sought. The expert panel agreed on the validity of 28 criteria describing the potentially inappropriate use of medication by general populations of the elderly as well as 35 criteria defining potentially inappropriate medication use in older persons known to have any of 15 common medical conditions. Updated, expanded, and more generally applicable criteria are now available to help identify inappropriate use of medications in elderly populations. These criteria define medications that should generally be avoided in the ambulatory elderly, doses or frequencies of administrations that should generally not be exceeded, and medications that should be avoided in older persons known to have any of several common conditions.

            Em busca de uma assistência adequada à saúde do idoso: revisão da literatura e aplicação de um instrumento de detecção precoce e de previsibilidade de agravos

            O aumento da população idosa exige dos gestores da área da saúde mudanças nos modelos assistenciais, em decorrência da significativa ampliação dos respectivos custos. Este artigo discute uma proposta de fluxo de acesso e definição de prioridades em uma rede ambulatorial. Utilizou-se um questionário breve, com oito perguntas objetivas, visando à captação e ordenação de idosos em risco de admissão hospitalar repetida, tomada como proxy de fragilização. Em uma amostra de 360 indivíduos, evidenciou-se que 75,8% dos idosos apresentavam baixo risco de adoecer. Cerca de 11% da amostra, contudo, apresentou risco de médio a alto de adoecer, utilizando de forma intensiva os serviços de saúde. Os achados corroboram a utilidade do procedimento proposto a fim de classificar idosos em diferentes grupos de risco de fragilização, possibilitando a hierarquização dos riscos nessa população e contribuindo para um sistema de saúde mais eficaz e resolutivo. Sugere-se testar a validade do método em um estudo prospectivo.

              Health outcomes and polypharmacy in elderly individuals: an integrated literature review.

              The purpose of this integrated literature review was to determine the extent of research available related to polypharmacy and its effect on the health outcomes of the elderly population. A search of the Cumulative Index of Nursing and Allied Health Literature and Medline was conducted for studies published between 1995 and 2003 that linked polypharmacy and outcomes in the elderly population. The 16 studies in this integrative literature review were conducted in the United States, Canada, Australia, and Europe. Polypharmacy was shown to be a statistically significant predictor of hospitalization, nursing home placement, death, hypoglycemia, fractures, impaired mobility, pneumonia, and malnutrition. The effect of polypharmacy on elderly individuals is significant as demonstrated by this literature review. Nurses are in a unique position to monitor and potentially eliminate adverse effects of a complex medication regimen. Nursing research on polypharmacy and its effects on nursing-sensitive outcomes will help define guidelines for prevention and intervention.
