          Abstract

          RESUMO Relatamos estudo clínico-patológico de um caso de invasão do nervo óptico por melanoma maligno da úvea. Comentamos as formas mais comuns de invasão extra-ocular por melanomas da úvea ressaltando a raridade da invasão extra-ocular destas neoplasias através do nervo óptico. Enfatizados a importância de seccionar o mais longo possível o nervo óptico ao realizarmos enucleações.

          Translated abstract

          SUMMARY A case of optic nerve invasion by uveal malignant melanoma is related. The usual forms of extra-ocular invasion by uveal malignant melanomas are commented. The need of the largest possible optic nerve section while performing enucleations is emphasized.

            Uveal melanoma: case report of extension through the optic nerve to the surgical margin in the orbital apex.

            An unsuspected uveal melanoma arising in a phthisical eye extended through the optic nerve to the surgical margin in the orbital apex without involvement of other orbital structures. Step sections showed the continuous extension through the nerve. Factors likely to predispose to optic nerve extension include epithelioid cell type, large tumour size, peripapillary involvement, and glaucoma. Optic nerve extension carries the risks noted with orbital extension of intraocular tumour, but may in addition lead more directly to intracranial invasion, including a chiasmal syndrome. The role of exenteration and radiotherapy in these cases remains uncertain.

              Orbital extension of choroidal melanoma: within a long posterior ciliary nerve.

              Following enucleation, an epithelioid type of malignant choroidal melanoma involving the posterior pole was grossly and histologically found to exhibit direct extraocular extension along the emissary for the long posterior ciliary artery and nerve on the temporal side. In addition to a small tumor nodule on the outside of the sclera, melanoma extension was found up to the cut end in the otherwise well preserved ciliary nerve. After tenonectomy, additional extraocular melanoma extension in the core of this long posterior ciliary nerve was demonstrated for a total distance of 5 mm. As a result of the present findings, intraneural melanoma extension into the orbit by the way of a ciliary nerve has to be added to the other possible ways of direct extraocular melanoma extension.
