      Perspectives on endoscopic submucosal dissection training in the United States: a survey analysis

          Abstract

          Background and study aims 

          Endoscopic submucosal dissection (ESD) is a widely accepted method for en-bloc resection of dysplastic lesions and early cancer in Asia and Europe. A limiting factor in adoption of ESD in the United States is perceived lack of training opportunities. The aims of this study were to: (1) evaluate ESD experience of attendees at a University-sponsored ESD training course; (2) characterize effectiveness of the current ESD training regimen and its impact on ESD adoption in the United States; and (3) gauge trainees’ attitude towards ESD.

          Patients and methods 

          An electronic anonymous survey was distributed to the 86 physicians who participated in the University of Florida’s annual ESD course from 2014 to 2016. Main outcomes included participants’ practice setting, prior training, current ESD techniques, and planned training.

          Results 

          A total of 34 participants (40 %) completed the survey. Most of the respondents routinely use one or more endoscopic mucosal resection (EMR) techniques (97.1 %) in their practice. Most respondents (79 %) had no experience with ESD on humans prior to the ESD course. Following completion of course training, more participants reported ongoing hands-on ESD exposure, with 15/34 (44 %) having performed ESD in humans. Most participants identified potential hurdles for adoption of ESD.

          Conclusion 

          A dedicated ESD training course with hands-on experience, under the guidance of experts, notably increased use of ESD among participants. Limited availability of structured training opportunities, concerns over procedural length, lack of adequate number of lesions, and potential for serious adverse events (AEs) were identified as the main factors slowing adoption of ESD in the United States.

            Clinical outcomes of EMR for gastric tumors: historical pilot evaluation between endoscopic submucosal dissection and conventional mucosal resection.

            EMR is currently a standard treatment for mucosal gastric tumors. Endoscopic submucosal dissection (ESD) has been developed for en bloc resection. We evaluated the clinical outcomes of ESD compared with conventional EMR. Not applicable. A historical control study was performed between EMR and ESD. EMR of 245 gastric tumors was performed in 229 patients. Lesions were divided into two groups. Conventional EMR was performed in group A from February 1999 to June 2001, and ESD was performed in group B from July 2001 to March 2004. Group B was divided into subgroups: subgroup B-1 underwent ESD from July 2001 to March 2003 and subgroup B-2 from April 2003 to March 2004. All lesions were resected with conventional EMR or with ESD. En bloc resection rate, rate in completeness of resection, required time, remnant ratio, and complications were evaluated. With regard to lesions >10 mm in size, the en bloc resection rate and the rate in completeness of resection of group B was significantly higher than that of group A (p < 0.01). Although the required time was longer in group B than A (p < 0.01), it was shorter in subgroup B-2 compared with B-1 (p < 0.05) with lesions < or =10 mm in size. The remnant ratio and perforation rate were not different between groups. Not applicable. The en bloc resection rate was better with ESD than with conventional EMR. The required time was longer in ESD, but this disadvantage might be improved with experience.

              Current practice with endoscopic submucosal dissection in Europe: position statement from a panel of experts.

              Endoscopic submucosal dissection (ESD) is the gold standard technique for performing en bloc resection of large superficial tumors in the upper and lower gastrointestinal tract. Experience in Europe, however, is still limited and ESD is only performed in a few selected centers, with low volumes of cases, no description of training programs, and few published reports. In 2008, a panel of experts gathered in Rotterdam to discuss indications, training, and the wider use of ESD. The panel of experts and participants reached a consensus on five general statements: 1) ESD aims at treating mucosal cancer; 2) treatment aims for R0 resection; 3) ESD should meet quality standards; 4) ESD should be performed following national or European Society of Gastrointestinal Endoscopy (ESGE) guidelines or under institutional review board approval; and 5) ESD cases should be registered. Due to the high level of expertise needed to perform the technique safely, ESD should be performed in a step-up approach, starting with lesions presenting in the rectum or in the distal stomach, then colon, proximal stomach, and finally in the esophagus. Registration is advised either at the local site or at a national or ESGE level, and should include information on indication (Paris classification of lesion, location, and histological results prior to treatment), technique used (e. g. type of knife), results (en bloc and R0 resection), complications, and follow-up. The panel also agreed on minimal institutional requirements: good quality imaging, experienced histopathologist following the Japanese criteria (2-mm sections, micrometric invasion, vessel and lymphatic infiltration, etc), and dedicated endoscopic follow-up. Moreover, minimum training requirements were also defined: knowledge in indications and instruments, exposure to experts (currently all in Japan), hands-on experience in a model of isolated pig stomach and in live pigs, and management of complications. The experts did not reach a consensus on a minimum case load, or whether the technique should be restricted to expert centers. © Georg Thieme Verlag KG Stuttgart · New York.
