      Nonsurgical correction of a severe anterior deep overbite accompanied by a gummy smile and posterior scissor bite using a miniscrew-assisted straight-wire technique in an adult high-angle case

          Abstract

          In the present report, we describe the successful use of miniscrews to achieve vertical control in combination with the conventional sliding MBT™ straight-wire technique for the treatment of a 26-year-old Chinese woman with a very high mandibular plane angle, deep overbite, retrognathic mandible with backward rotation, prognathic maxilla, and gummy smile. The patient exhibited skeletal Class II malocclusion. Orthodontic miniscrews were placed in the maxillary anterior and posterior segments to provide rigid anchorage and vertical control through intrusion of the incisors and molars. Intrusion and torque control of the maxillary incisors relieved the deep overbite and corrected the gummy smile, while intrusion of the maxillary molars aided in counterclockwise rotation of the mandibular plane, which consequently resulted in an improved facial profile. After 3.5 years of retention, we observed a stable, well-aligned dentition with ideal intercuspation and more harmonious facial contours. Thus, we were able to achieve a satisfactory occlusion, a significantly improved facial profile, and an attractive smile for this patient. The findings from this case suggest that nonsurgical correction using miniscrew anchorage is an effective approach for camouflage treatment of high-angle cases with skeletal Class II malocclusion.

          The dilemma of extraction vs nonextraction treatment, along with the uncertain potential of orthodontic treatment to control vertical dimensions, still remains among the most controversial issues in orthodontics.             Correction of deep overbite and gummy smile by using a mini-implant with a segmented wire in a growing Class II Division 2 patient.

            A boy, aged 10.5 years, with a Class II molar relationship and a very deep overbite, complaining of a gummy smile and anterior crowding, was treated nonextraction with a mini-implant and Twin-block and edgewise fixed appliances. Severely extruded and retroclined maxillary incisors were intruded and proclined with a nickel-titanium closed-coil spring anchored to a mini-implant and segmented wires; this resolved the gummy smile and deep overbite efficiently without extruding the maxillary molars or opening the mandible. The mandibular incisors were proclined without direct orthodontic force during intrusion of the maxillary incisors; this helped the nonextraction treatment of mandibular incisor crowding. The Twin-block appliance with high-pull headgear promoted mandibular growth, restrained maxillary growth, and changed the canine and molar relationship from Class II to Class I. The patient's overbite and overjet were overtreated, and, 1 year postretention, the patient maintained a good overbite and overjet.

              Gummy smile and facial profile correction using miniscrew anchorage.

              This case report describes the treatment of a case involving a skeletal Class II facial profile with a gummy smile. While treating a facial profile and a gummy smile, the outcome may not always be successful with orthodontic therapy alone. For this reason, surgical therapy is often chosen to gain an esthetic facial profile and a good smile. However, sometimes the patients reject surgical treatment and an alternative method must be considered. Skeletal anchorage systems such as miniscrews are now frequently used for correcting severe malocclusion that should be treated by surgical therapy. In this case report, we treated a skeletal Class II malocclusion with a convex profile and a gummy smile using miniscrews, which were placed in the upper posterior and anterior areas. The active treatment period was 3.5 years, and the patient's teeth continued to be stable after a retention period of 36 months.
