          Abstract

          As dislipidemias são um importante problema de saúde pública, e as terapias medicamentosas para seu controle são consideradas de alto custo, em razão de seu uso contínuo. Este artigo descreve os gastos e o perfil demográfico e epidemiológico dos pacientes atendidos pelo Componente Especializado da Assistência Farmacêutica do Sistema Único de Saúde, no período de 2003 a 2006, bem como identifica as características relacionadas ao gasto individual médio mensal com medicamentos. Foram incluídos 67.142 indivíduos na coorte histórica, e os resultados das análises descritivas e multivariada indicaram alta frequência de hipercolesterolemia pura em mulheres adultas e idosas residentes na região Sudeste, predominância de utilização de Sinvastatina e Atorvastatina e gasto médio mensal individual de aproximadamente R$88,00. Observou-se que o aumento da idade levou a um menor gasto, pacientes do sexo masculino tenderam a gastar mais e a Atorvastatina foi a que mais impactou no gasto. O perfil epidemiológico mostra-se coerente com a literatura, e o gasto individual foi elevado.

          Translated abstract

          Lipid disorders are a major public health problem and drug therapies to control them have high cost, due to the continued use. This paper describes the costs and demographic and epidemiological profiles of the patients treated by the Pharmaceutical Specialized Care of the Brazilian Unified Health Care System, from 2003 to 2006, and identifies the characteristics related to the individual average monthly expenses on drugs. We included in the historical cohort 67,142 individuals, and the results of descriptive and multivariate analyses indicated a high frequency of pure hypercholesterolemia in adult women and elderly residents in the Southeast, of higher use of simvastatin and atorvastatin and individual average monthly expenditure of approximately R$ 88,00. It was observed that increasing age led to lower spending; male patients were more likely to have higher expenditure; and atorvastatin was the drug that most increased expenditures. The epidemiological profile was consistent with the literature and individual spending was high.

              Risk factors for myocardial infarction in Brazil.

              Approximately three-quarters of cardiovascular disease deaths in the world come from developing countries, and acute myocardial infarction (AMI) is an important cause of death. Brazil is one of the largest countries in Latin America and the contemporary evaluation of risk factors for AMI is crucial for a more efficacious disease management. The Acute Myocardial Infarction Risk Factor Assessment in Brazil (AFIRMAR) study is a case-control, hospital-based study involving 104 hospitals in 51 cities in Brazil, designed to evaluate risk factors for a first ST-segment elevation AMI. A total of 1279 pairs, matched by age (+/- 5 years) and sex, were enrolled. The conditional multivariable analysis of 33 variables showed the following independent risk factors for AMI: > or =5 cigarettes per day (odds ratio [OR] 4.90, P or =126 mg/dL (OR 2.82, P or =0.94 (OR 2.45, P 120 mg/dL (OR 1.75, P R$1200 and college education (OR 0.68, P = 0.0239) The independent risk factors for AMI in Brazil showed a conventional distribution pattern (smoking, diabetes mellitus and central obesity among others) with different strengths of association; most of them being preventable by implementation of adequate policies.
