          Resumen

          La sialoadenitis aguda es una reacción adversa muy poco frecuente a la administración de contraste yodado, que causa una inflamación autolimitada de las glándulas salivales. Su patogenia no está bien establecida, aunque la insuficiencia renal puede ser un factor de riesgo. El diagnóstico es inicialmente clínico, y debe realizarse diagnóstico diferencial con angioedema, infecciones y litiasis. Ningún tratamiento o profilaxis ha demostrado beneficio hasta el momento. Aunque tiene buen pronóstico, en algunos casos se han descrito complicaciones.

          Presentamos el caso de un varón de 68 años que presentó inflamación de las glándulas salivales submandibulares tras la realización de una tomografía computarizada abdominal con administración de contraste yodado. Dado el uso creciente de contrastes yodados en pruebas de imagen y técnicas intervencionistas, es importante conocer posibles reacciones adversas como esta entidad.

          Abstract

          Acute iodide sialadenitis is a rare adverse reaction to iodinated contrast that causes self-limited salivary gland swelling. Its pathogenesis is still unclear, although kidney failure may be a risk factor. The diagnosis is initially clinical but angioedema, infections and lithiasis should be included in the differential diagnosis. No treatment or prophylaxis was proven to be beneficial. Although its prognosis is benign, associated complications have been reported.

          We report a case of 68-year-old man with swelling of the submandibular salivary glands after the administration of iodine-based contrast media during an abdominal computed tomography examination. Because of the widespread use of iodinated contrast enhanced imaging and interventional techniques, clinicians should be aware of this issue.

            Iodide mumps after contrast media imaging: a rare adverse effect to iodine.

            Exposure to iodinated contrast media may elicit a variety of adverse reactions. Anaphylactoid and delayed cell-mediated unwanted effects are common; rare adverse reactions include iodine-related sialadenopathy, iododerma, and acneiform eruptions. To describe the occurrence of iodide mumps in a patient examined using contrast-enhanced computed tomography and to outline differential diagnoses. A detailed diagnostic approach, including histologic analysis, skin tests, controlled reexposure, efficacy of premedication, and imaging studies, is presented. The findings unique to this reaction and differential diagnoses are highlighted. While undergoing repeated contrast-enhanced computed tomography a patient developed recurrent swellings of the sublingual glands, identified as iodide mumps. These swellings resolved within a few days. Imaging studies and histologic analysis showed distinct patterns without inflammation. The important role of iodine in this adverse reaction is demonstrated. Iodine from iodinated contrast media may rarely elicit noninflammatory edema of the salivary glands. The reaction should be differentiated from other swellings in the head and neck area.
