          Abstract

          RESUMEN Objetivos: El cuidado institucionalizado de personas mayores es una realidad en España. Muchos hijos acompañan a sus padres en esa experiencia de vida y conforman su propia opinión. El objeto del estudio es conocer esa opinión, proponiendo una mirada hacia el futuro cuidado que ellos podrán recibir, partiendo de la opinión que tienen del cuidado actual que reciben sus padres. Metodología: La metodología utilizada es cualitativa, mediante entrevistas en profundidad y análisis de contenido, en una muestra integrada por hijos/as de residentes de dos centros residenciales de la ciudad de Jaén. Resultados: El resultado es que aceptan el modelo de cuidados actual para sus padres, salvo por algunas cuestiones que podrían mejorar, pero no lo admiten para sí mismos. Conclusiones: Se concluye que es necesaria la evolución de la asistencia, a través de la domiciliación y la especialización del cuidado.

          ABSTRACT Objectives: The nursing home care of the elderly is a reality in Spain. Many of them are accompanied by their children in this life experience, whom create their own opinion. Knowing the children opinion including a future perspective is the main purpose of the research, on the basis of their current opinion of caring. Methods: A qualitative survey is used, with in-depth interviews and content analysis. The sample was selected from two nursing homes in the city of Jaen. Results: The main outcome was than they are satisfied with this kind of caring but not for themselves. Conclusions: We conclude that elderly care needs moving to home and through care specialization.

            A sense of home in residential care.

            Moving into a residential care facility requires a great deal of adjustment to an environment and lifestyle entirely different from that of one's previous life. Attachment to place is believed to help create a sense of home and maintain self-identity, supporting successful adjustment to contingencies of ageing. The purpose of this study was to deepen our understanding of processes and strategies by which older people create a sense of home in residential care. Our findings show that a sense of home in residential care involves strategies related to three dimensions of the environment - attachment to place, to space and attachment beyond the institution - and that the circumstances under which older people manage or fail in creating attachment, consist of psychosocial processes involving both individual and shared attitudes and beliefs. Assuming that attachment is important to human existence regardless of age, attention must be paid to optimize the circumstances under which attachment is created in residential care, and how nursing interventions can help speed up this process due to the frail and vulnerable state of most older residents.

              Advanced nurse practitioners in municipal healthcare as a way to meet the growing healthcare needs of the frail elderly: a qualitative interview study with managers, doctors and specialist nurses

              Background The number of frail elderly people with complex nursing and medical care needs is increasing, and consequently, the healthcare burden. The implementation of Advanced Nurse Practitioners globally has been shown to make healthcare more effective and increase patient safety, continuity of care and access to care. In Sweden, research about Advanced Nurse Practitioners is limited. Thus, this study aimed to investigate the opinions of managers, doctors and nurses in primary care and municipal healthcare about the role of ANPs in municipal healthcare as a way to meet the increasing healthcare needs of the frail elderly. Methods Managers, doctors and specialist nurses in primary care and municipal healthcare adopted a qualitative, descriptive design through 12 semi-structured interviews. The data were analysed using content analysis. Results The participants expressed both opportunities as well as challenges with Advanced Nurse Practitioners in municipal healthcare. This role considered to satisfy frail elderly people’s healthcare needs and making the healthcare more effective as the doctors would have more time for other patients. The challenges mainly consist of doubts from the managers whether the nurses would be motivated to pursue further education to become an Advanced Nurse Practitioner if the role becomes a reality. The doctors were unsure if the nurses would consider taking the responsibility the role would imply. Conclusions Advanced Nurse Practitioner is considered to be a valuable resource not only for the frail elderly but also for the nurses in the municipal healthcare and for the doctors in primary care as they probably would make healthcare more effective. They might be a way to meet the increasing healthcare needs of frail elderly, however there are also challenges to overcome before they can become a reality in a Swedish healthcare context. Consequently, this role deserves further investigation.
