          Abstract

          This review provides an overview of triaging critically ill or injured patients during mass casualty incidents due to events such as disasters, pandemics, or terrorist incidents. Questions clinicians commonly have, including “what is triage?,” “when to triage?,” “what are the types of disaster triage?,” “how to triage?,” “what are the ethics of triage?,” “how to govern triage?,” and “what research is required on triage?,” are addressed.

          Development of a triage protocol for critical care during an influenza pandemic.

          The recent outbreaks of avian influenza (H5N1) have placed a renewed emphasis on preparing for an influenza pandemic in humans. Of particular concern in this planning is the allocation of resources, such as ventilators and antiviral medications, which will likely become scarce during a pandemic. We applied a collaborative process using best evidence, expert panels, stakeholder consultations and ethical principles to develop a triage protocol for prioritizing access to critical care resources, including mechanical ventilation, during a pandemic. The triage protocol uses the Sequential Organ Failure Assessment score and has 4 main components: inclusion criteria, exclusion criteria, minimum qualifications for survival and a prioritization tool. This protocol is intended to provide guidance for making triage decisions during the initial days to weeks of an influenza pandemic if the critical care system becomes overwhelmed. Although we designed this protocol for use during an influenza pandemic, the triage protocol would apply to patients both with and without influenza, since all patients must share a single pool of critical care resources.

            Severe Acute Respiratory Syndrome

            Abstract The first cases of severe acute respiratory syndrome (SARS) occurred in China in November 2002. The agent causing this illness has been identified as a novel coronavirus, SARS-coronavirus. Since its introduction <1 year ago, this virus has infected 8098 people in 26 countries, killing 774 of them. We present an overview of the epidemiology, clinical presentation, diagnosis, and treatment of SARS based on the current state of knowledge derived from published studies and our own personal experience.

              Triage in medicine, part I: Concept, history, and types.

              This 2-article series offers a conceptual, historical, and moral analysis of the practice of triage. Part I distinguishes triage from related concepts, reviews the evolution of triage principles and practices, and describes the settings in which triage is commonly practiced. Part II identifies and examines the moral values and principles underlying the practice of triage.
