          Abstract

          Resumen Introducción: Los adultos jóvenes se han convertido en un colectivo de vulnerabilidad, aunado a la emergencia de salud pública de la COVID-19, ocasionado impactos psicológicos, que requieren cuidados de enfermería. Objetivo: Desarrollar un plan de cuidados enfermero a adulto joven con diagnóstico de Trastorno de Ansiedad Severo durante pandemia de COVID-19. Método: Caso clínico descriptivo, en adulto joven de 25 años, bajo la metodología del Proceso Atención de Enfermería, la valoración fue guiada con el enfoque de la Teoría General del Déficit de Autocuidado, se utilizó NANDA, se seleccionaron resultados e intervenciones a través de las Taxonomías NOC, NIC y Guías de Prácticas Clínicas. Resultados: Se identificaron 7 etiquetas de enfermería, 6 reales, 1 de riesgo, automutilación, estrés por sobrecarga, deterioro de la regulación del estado de ánimo, riesgo de suicidio, estreñimiento, deterioro de la resiliencia. Se desarrollaron 7 planes de cuidados. Conclusión: La aplicación del proceso permitió diagnosticar y jerarquizar de manera precisa, para lograr intervenir, favoreciendo el equilibrio emocional y bienestar personal.

          Abstract: Introduction: Young adults have become a vulnerable group, coupled with the public health emergency of COVID-19, caused psychological impacts, requiring nursing care. Objective: To develop a nursing care plan for young adults diagnosed with Severe Anxiety Disorder during a COVID-19 pandemic. Method: Clinical case study, in a 25-year-old young adult, under the methodology of the Nursing Care Process, the assessment was guided with the approach of the General Theory of Self-Care Deficit, NANDA was used, they were selected results and interventions through the NOC, NIC Taxonomies and Clinical Practice Guidelines. Results: 7 nursing labels were identified, 6 real, 1 risk, self-mutilation, stress due to overload, deterioration of mood regulation, risk of suicide, constipation, deterioration of resilience. 7 care plans were developed. Conclusion: The application of the process allowed to diagnose and rank accurately, to intervene, favoring emotional balance and personal well-being.

          Mental Health and Psychosocial Problems of Medical Health Workers during the COVID-19 Epidemic in China

          Objective We explored whether medical health workers had more psychosocial problems than nonmedical health workers during the COVID-19 outbreak. Methods An online survey was run from February 19 to March 6, 2020; a total of 2,182 Chinese subjects participated. Mental health variables were assessed via the Insomnia Severity Index (ISI), the Symptom Check List-revised (SCL-90-R), and the Patient Health Questionnaire-4 (PHQ-4), which included a 2-item anxiety scale and a 2-item depression scale (PHQ-2). Results Compared with nonmedical health workers (n = 1,255), medical health workers (n = 927) had a higher prevalence of insomnia (38.4 vs. 30.5%, p < 0.01), anxiety (13.0 vs. 8.5%, p < 0.01), depression (12.2 vs. 9.5%; p< 0.04), somatization (1.6 vs. 0.4%; p < 0.01), and obsessive-compulsive symptoms (5.3 vs. 2.2%; p < 0.01). They also had higher total scores of ISI, GAD-2, PHQ-2, and SCL-90-R obsessive-compulsive symptoms (p ≤ 0.01). Among medical health workers, having organic disease was an independent factor for insomnia, anxiety, depression, somatization, and obsessive-compulsive symptoms (p < 0.05 or 0.01). Living in rural areas, being female, and being at risk of contact with COVID-19 patients were the most common risk factors for insomnia, anxiety, obsessive-compulsive symptoms, and depression (p < 0.01 or 0.05). Among nonmedical health workers, having organic disease was a risk factor for insomnia, depression, and obsessive-compulsive symptoms (p < 0.01 or 0.05). Conclusions During the COVID-19 outbreak, medical health workers had psychosocial problems and risk factors for developing them. They were in need of attention and recovery programs.

            Motivations for adolescent self-harm and the implications for mental health nurses.

            WHAT IS KNOWN ON THE SUBJECT?: Self-harm is a relatively common occurrence in adolescents; however, there remains a lack of understanding about the motivations behind adolescent self-harm, and this poor understanding can have a negative impact on how mental health professionals respond to young people who self-harm. WHAT DOES THIS PAPER ADD TO EXISTING KNOWLEDGE?: This paper identifies the reasons for self-harm in a community sample of young people and finds that the functions of self-harm differ for different people and that there may be multiple reasons for self-harm. Findings provide support for the affect-regulation model of self-harm which states that young people self-harm to regulate how they are feeling, but provides little support for the interpersonal influence model which proposes that self-harm is an attempt to influence how other people respond to them. WHAT ARE THE IMPLICATIONS FOR PRACTICE?: Self-harm for most young people serves at least one specific function and is an indication of distress. There is a requirement for a non-pathologizing response towards young people who self-harm; self-harm should be understood as a meaningful behaviour rather than a symptom of an illness. Mental health nurses need to understand the multiple functions of self-harm. A better understanding of the individualized meaning behind self-harm can positively impact on attitudes towards young people who self-harm and provide for improved mental health service provision.

              Modelos metaparadigmáticos de Dorothea Elizabeth Orem

              RESUMEN Fundamento: Dorothea Elizabeth Orem ha descrito lo que llamó teoría general de enfermería, la cual consta de tres teorías relacionadas entre sí: la del autocuidado, la del déficit de autocuidado y la teoría de los sistemas de enfermería. Objetivo: fundamentar los modelos metaparadigmáticos de Dorothea Elizabeth Orem. Métodos: se realizó una revisión bibliográfica sistemática para desarrollar un análisis crítico reflexivo del contenido de artículos originales y de revisión, publicados en español e inglés entre 2001 y 2019. Las palabras clave utilizadas fueron enfermería de práctica avanzada, definición de enfermería de práctica avanzada, historia de enfermería de práctica avanzada la búsqueda fue realizada en las bases de datos SciELO, Pubmed/Medline, Ebsco, ClinicalKey, Springer, Web of Science e Infomed. Resultados: dentro del tema como son: Metaparadigma de enfermería según Dorothea Elizabeth Orem, La teoría del autocuidado, la teoría del déficit de autocuidado, la teoría de los sistemas de enfermería. Conclusiones: el modelo de Dorothea Elizabeth Orem proporciona un marco conceptual y establece una serie de conceptos metaparadigmáticos, definiciones, objetivos, para resaltar lo relacionado con los cuidados de la persona, por lo que se considera debe aceptarse para el desarrollo y fortalecimiento de la enfermería cubana.
