Abstract

Resumen Actualmente la cronicidad es un término que preocupa con especial respeto a la sociedad, y en concreto a la enfermería de atención primaria. Bien es cierto, que el paciente ya no es un ser pasivo si no que ha pasado a ser el principal protagonista de sus cuidados. La cronicidad de nuestros pacientes, está dando lugar a la creación de nuevas estrategias para su abordaje, y desde este caso clínico queremos dar a conocer otra visión de la valoración de personas con enfermedades crónicas. Objetivo: Aplicar el proceso enfermero a una paciente captada en atención primaria, a través de la valoración mediante la teoría de rango medio de autocuidado de enfermedades crónicas Metodología: En la elaboración del plan de cuidados se utilizó el formato NANDA, los indicadores se plantearon según los criterios de resultados NOC y las intervenciones mediante el formato NIC. Conclusiones: El paciente con una o diversas enfermedades crónicas requiere del esfuerzo de enfermería de atención primaria para que esta eduque, guíe y apoye. Tratando de favorecer el empoderamiento de los pacientes y que ellos mismos formen parte de las decisiones en su salud. Todo ello, conlleva a reducir costes en el sistema sanitario, reducir la carga asistencial y mejorar la atención de procesos complejos de enfermedad.

Abstract Currently, chronicity is a term that concerns society and specifically primary care nursing. It is true that the patient is no longer a passive actor, but rather that he has become the main protagonist of his care. The chronicity of our patients is rising to the creation of new strategies for their approach, and from this clinical case we want to present another vision of the assessment of people with chronic diseases. Objective: To carry out the nursing care plan we used a patient recruited from primary care, through the assessment using the middle-range theory of self-care of chronic illness. Methodology: The NANDA format was used in the making of the care plan, the indicators were established according to the NOC results criteria and the interventions were established using the NIC format. Conclusion: The patient with one or several chronic diseases requires the effort of primary care nursing to educate, guide and support. Trying to increase the empowerment of the patients and try to be part of the decisions in their health. All of this is going to reduce costs in the health system, reducing the care burden and improving care for complex disease processes.
