      Evaluation of Magnesium as an adjuvant in Ropivacaine-induced supraclavicular brachial plexus block: A prospective, double-blinded randomized controlled study

          Abstract

          Objective:

          Different additives have been used to prolong brachial plexus block. We evaluated the effect of adding magnesium sulfate to ropivacaine for supraclavicular brachial plexus blockade. The primary endpoints were the onset and duration of sensory and motor block and duration of analgesia.

          Methods:

          One hundred patients (25–55 years) posted for elective forearm and hand surgeries under supraclavicular brachial plexus block were divided into two equal groups (Groups RM and RN) in a randomized, double-blind fashion. In group RM ( n = 50), 30 ml 0.5% ropivacaine plus 150 mg (in 1 ml 0.9% saline) magnesium sulfate and in group RN ( n = 50), 30 ml 0.5% ropivacaine plus 1 ml normal saline were administered in supraclavicular block. Sensory and motor block onset times and block durations, time to first analgesic use, total analgesic need, postoperative visual analog scale (VAS), hemodynamic variables, and side effects were recorded for each patient.

          Findings:

          Though with similar demographic profile and block (sensory and motor) onset time, the sensory and motor block duration and time to first analgesic use were significantly longer and the total need for rescue analgesics was lower in group RM ( P = 0.026) than group RN. Postoperative VAS values at 24 h were significantly lower in group RM ( P = 0.045). Intraoperative hemodynamics was comparable among two groups and no appreciable side effect was noted throughout the study period.

          Conclusion:

          It can be concluded from this study that adding magnesium sulfate to supraclavicular brachial plexus block may increase the sensory and motor block duration and time to first analgesic use, and decrease total analgesic needs, with no side effects.

          Magnesium sulfate reduces intra- and postoperative analgesic requirements.

          In a randomized, double-blind study with two parallel groups, we assessed the analgesic effect of perioperative magnesium sulfate administration in 46 ASA physical status I or II patients undergoing arthroscopic knee surgery with total i.v. anesthesia. The patients received either magnesium sulfate 50 mg/kg preoperatively and 8 mg.kg-1.h-1 intraoperatively or the same volume of isotonic sodium chloride solution i.v. Anesthesia was performed with propofol (2 mg/kg for induction, 6-8 mg.kg-1.h-1 for maintenance), fentanyl (3 micrograms/kg for induction), and vecuronium (0.1 mg/kg for intubation). Intraoperative pain was defined as an increase of mean arterial blood pressure and heart rate of more than 20% from baseline values after the induction of anesthesia and was treated with bolus fentanyl (1-2 micrograms/kg). Postoperative analgesia was achieved with fentanyl (0.5 microgram/kg) and evaluated using the pain visual analog scale for 4 h. During the intraoperative and postoperative periods, patients in the magnesium group required significantly less fentanyl than those in the control group (control group 0.089 +/- 0.02 microgram.kg-1.min-1 versus magnesium group 0.058 +/- 0.01 microgram.kg-1.min-1; P < 0.05 and control group 0.021 +/- 0.013 microgram.kg-1.min-1 and magnesium group 0.0031 +/- 0.0018 microgram.kg-1.min-1; P < 0.01 for intraoperative and postoperative periods, respectively). We conclude that, in a clinical setting with almost identical levels of surgical stimulation, i.v. magnesium sulfate administration reduces intraoperative and postoperative analgesic requirements compared with isotonic sodium chloride solution administration. The perioperative administration of i.v. magnesium sulfate reduces intra- and postoperative analgesic requirements in patients with almost identical levels of surgical stimulus. Our results demonstrate that magnesium can be an adjuvant to perioperative analgesic management.

            Magnesium: a versatile drug for anesthesiologists

            Magnesium sulfate has been used in preeclampsia patients in order to prevent seizure. It is also used for the treatment of arrhythmia and asthma and as an anesthetic adjunct in patients undergoing surgery for pheochromocytoma. However, its potentiating effects on perioperative analgesia and muscle relaxation have drawn attention recently. These characteristics of magnesium (anesthetic- and analgesic-sparing effect) enable anesthesiologists to reduce the use of anesthetics during surgery and the use of analgesics after surgery. Magnesium sulfate has a high therapeutic index and cost-effectiveness. Considering these diverse characteristics useful for anesthesia, appropriate use of magnesium sulfate would improve surgical outcome and patients' satisfaction.

              Magnesium added to bupivacaine prolongs the duration of analgesia after interscalene nerve block.

              Local anesthetic adjuvants have been studied previously in an attempt to prolong the duration of analgesia after peripheral nerve blockade. Magnesium has been shown to have an antinociceptive effect in animal and human pain models. We evaluated the effects of adding magnesium sulphate to long-acting local anesthetics for interscalene nerve block to prolong the duration of analgesia and improve the analgesic quality. We enrolled 66 patients undergoing arthroscopic rotator cuff repair. The interscalene nerve block was performed with 0.5% bupivacaine 20 mL with epinephrine (1:200,000) plus either 10% magnesium sulphate 2 mL (Magnesium Group) or normal saline 2 mL (Saline Group). The following data were recorded for 24 hr after surgery: onset times and durations of sensory and motor blocks, analgesic duration, the pain numeric rating scale (NRS), postoperative fentanyl consumption, and complications. The duration of analgesia was longer in the Magnesium Group than in the Saline Group [mean and (standard deviation) 664 (188) min vs 553 (155) min, respectively; P = 0.017]. Patients in the Magnesium Group had significantly reduced pain NRS scores at 12 hr (P = 0.012), but the cumulative fentanyl consumption was similar in both groups. The onset times and durations of sensory and motor blocks were not significantly different between the two groups. The addition of magnesium sulphate to a bupivacaine-epinephrine mixture for interscalene nerve block prolongs the duration of analgesia and reduces postoperative pain.
