          Abstract

          Resumen Introducción y objetivo. Para el marcaje preoperatorio en dermolipectomía abdominal, frecuentemente se toma como referencia la horquilla vulvar y la base del pene para localizar la posición de la incisión. Sin embargo, estos puntos varían en cada individuo y no los consideramos adecuados para localizar la altura de la futura cicatriz. Es por ello que proponemos tomar como referencia un nuevo protocolo de medidas que permita tener como resultado un pubis más geométrico y armónico, determinando la altura de la incisión en relación al nivel del pubis. Material y método. Describimos una técnica de marcaje preoperatorio para la localización de la incisión horizontal y de las porciones laterales en dermolipectomía abdominal. Resultados. Basándonos en la geometría del pubis, marcamos un triángulo de base superior tomando como referencia inicial el origen del pliegue inguinal en la superficie cutánea, donde se hace profundo, y trazamos una línea horizontal que sirve de referencia para marcar las porciones laterales de la cicatriz horizontal. Conclusiones. Nuestra propuesta toma en cuenta puntos anatómicos constantes que permiten obtener una futura cicatriz abdominal de altura más adecuada y mantener la armonía y geometría del pubis.

          Translated abstract

          Abstract Background and objective. For preoperative marking in abdominal dermolipectomy, the vulvar notch and the base of the penis are frequently used as reference to locate the marking of the incision. However, these points may vary in each individual and we consider them not suitable for locating the height of the incision. Therefore, we propose a new measurement protocol that allows to get a more geometric and harmonic pubis by determining the height of the incision in relation to the level of the pubis. Methods. Our method for performing a preoperative marking for the location of the horizontal scar in dermolipectomy is described, so as the lateral portions. Results. Marking is based on the inverted triangular shape of the pubic area, taking as an initial reference the origin of the inguinal fold on the skin surface, where it becomes deep. In addition, the tracing of a horizontal line that serves as a refe- rence for marking the lateral portions of the horizontal scar is described. Conclusions. Our proposal considering some constant ana- tomical points, allows obtaining a future abdominal scar with a more adequate height in dermolipectomy maintaining the harmony and geometry of the pubis.

            Abdominoplasty: classic principles and technique.

            Examination of abdominal contour surgery candidates permits categorization of patients (the abdominolipoplasty system of classification and treatment) according to their treatable soft tissue layers of skin, fat, and muscle into the appropriate treatment level. Typically, postpartum abdominal wall changes are most thoroughly addressed by abdominoplasty. The indications and goals for abdominoplasty with liposuction (lipoadminoplasty) or without liposuction are presented. Surgical markings, technique, pain management, and postoperative care are described.

              Redefining abdominal anatomy: 10 key elements for restoring form in abdominoplasty.

              While traditional abdominoplasty methods can successfully achieve the objective of restoring a flat appearance, the results can be artificially board-like, lacking the subtle anatomical features of a three-dimensional abdomen, thus creating the potential for patient dissatisfaction. While often difficult to articulate, patient criticism is almost always distilled to the ubiquitous concern that the surgical abdomen lacks the natural features of an authentic, youthful abdomen. In an effort to provide a more anatomically accurate outcome and improve patient satisfaction, I have made a series of technical modifications to the abdominoplasty that I now perform. Ten key technical refinements, including a modified "Anatomy Defining" Progressive Tension Suture technique, were successively incorporated in 177 patients during the first 5 years of 2000-2014. All have been applied consistently in 961 abdominoplasty procedures during the subsequent 10 years, often accompanied by liposuction of adjacent lateral (non-abdominal) areas to ensure harmonious proportion. This series of refinements adds precision and detail by redefining the native anatomical nuances of the abdomen, an aesthetic objective that has been consistently achieved in BMI ranges between 20 and 35. Overall satisfaction with results was high (94%). The 10 elements described are safe, effective, and lasting.
