          Abstract

          Resumen Las tumoraciones congénitas de la cara son motivo frecuente de consulta al cirujano plástico pediátrico. La gran mayoría son benignas pero llamativas por su localización. Dentro de éstas, las tumoraciones de la línea media nasal cobran especial importancia por la posibilidad de presentar comunicaciones intracraneanas, porque pueden generar obstrucción al flujo de aire del neonato y porque la demora en su diagnóstico y tratamiento adecuados puede determinar secuelas a largo plazo. Nos centraremos en el diagnóstico y tratamiento de la heterotipia glial nasal a propósito de un caso en paciente de 2 años de edad.

          Translated abstract

          Abstract Congenital face tumors are frequently referred to the pediatric plastic surgeon. Most of them are benign lesions, but they are striking because of their localization. The nasal midline masses are especially important because they may have intracranial connections, they can cause airway obstruction and because the delay in correct diagnosis and treatment may cause long term sequelae. We will focus on the diagnosis and treatment of nasal glial heterotipia regarding a two-year-old patient.

          Midline Craniofacial Masses in Children

          Nasal dermoids, encephaloceles, and gliomas are rare congenital lesions that result from improper embryologic development. The differentiation between them and a firm understanding of their pathology is necessary to avoid unnecessary complications. In view of their potential intracranial connection, prompt diagnosis and treatment are paramount. The authors review the embryology, diagnoses, radiologic work-up, surgical management, and complications of these midline craniofacial masses in children.

            Nasal midline masses in infants and children. Dermoids, encephaloceles, and gliomas.

            Nasal dermoids, gliomas, and encephaloceles are uncommon congenital lesions that result from aberrant embryologic development. We have treated 46 children with these nasal lesions. In view of the potential intracranial connection, patients are at risk for intracranial infection, and early surgical correction is thus imperative. Neuroimaging studies may help to predict intracranial involvement.

              Nasal glioma presenting as capillary haemangioma.

              We report the case of a 5-month-old female infant with a congenital nasal tumour originally attributed to a capillary haemangioma. Doppler-flow ultrasound imaging revealed a solid mass surrounded by mildly enlarged vessels which had a flow pattern atypical of haemangioma. Histology showed non-malignant gliomatous cells with low proliferative activity. A diagnosis of nasal glioma was thus established and the patient underwent cranial MRT which excluded intracranial communication of the nasal glioma. Nasal gliomas arise from a skull defect, originating from the defective closure of the anterior neuroporus. They represent encephaloceles which have lost their intracranial connection. Nasal gliomas usually present shortly after birth as an intranasal obstruction or, as in our case, as a mostly extranasal tumour.
