      Aneurisma “de novo” Reporte de un caso y revisión de la literatura Translated title: Aneurysm “de novo” Case report and literature review

          Abstract

          RESUMEN Introducción: Un aneurisma se define “de novo” cuando aparece en una angiografía repetida y no se veía en un estudio previo, siendo un requisito fundamental que los estudios vasculares realizados al pacientes sean de buena calidad. La mayor parte de los trabajos publicados, corresponden a pacientes que sufrieron una hemorragia subaracnoidea a consecuencia de la rotura de un nuevo aneurisma. Si bien su frecuencia es muy baja, en determinados pacientes (jóvenes, hipertensos, fumadores, aneurismas múltiples), se sugiere un seguimiento angiográfico por un largo periodo de tiempo. Hacer el diagnóstico de estos aneurismas antes de que se rompan, mejoraría sustancialmente el pronóstico. Materiales y Método: Se presenta un caso clínico, de un paciente que presentó una hemorragia subaracnoidea complicada con hematoma subdural agudo por rotura de un aneurisma, que por definición resulto ser “de novo”. Se realiza una puesta al día sobre esta patología. Discusión: La incidencia de aparición de aneurismas “de novo” oscila alrededor del 1 %, y el intervalo transcurrido entre el primer estudio angiográfico y la manifestación del mismo es variable, en promedio 10 años. Su presencia aumenta el riesgo de hemorragia subaracnoidea hasta en 6 veces. Hay distintos factores de riesgo, que aumentan la probabilidad de presentar estos aneurismas, que de tenerlos en cuenta, nos permitirían realizar un seguimiento adecuado y llegar en muchos casos a un diagnóstico oportuno. Conclusiones: Si bien los aneurismas “de novo” tienen una incidencia baja, pacientes que tienen hemorragia subaracnoidea, especialmente mujeres jóvenes, deberían tener un seguimiento angiográfico de no menos de 10 años. La detección de estos aneurismas antes de que causen un sangrado, mejora sustancialmente el pronóstico, dado que la morbimortalidad es mucho más baja con el tratamiento de los aneurismas no rotos.

          ABSTRACT Introduction: An aneurysm is defined as “de novo”, when it appears in a repeated angiography and not seen in a previous study, being a fundamental requirement that vascular studies the patients are of good quality. Most of the published works are for patients who suffered a subarachnoid hemorrhage due to rupture of a new aneurysm. Although their frequency is very low, in certain (young, hypertension, smoking, multiple aneurysms) patients, angiographic follow-up for a long period of time is suggested. Making the diagnosis of these aneurysms before they rupture, substantially improve the prognosis. Materials and Methods: A case who presented a complicated subarachnoid hemorrhage with acute subdural hematoma due to rupture of an aneurysm, which by definition turned out to be “de novo” is presented. An update on this pathology was performed. Discussion: The incidence of occurrence of “de novo” aneurysms hovers around 1%, and the interval between the first angiographic study and the manifestation of it is variable, on average 10 years. Its presence increases the risk of subarachnoid hemorrhage up to 6 times. There are several risk factors that increase the probability of these aneurysms, which take them into account, they allow us to properly track and in many cases get a timely diagnosis. Conclusions: Although “de novo” aneurysms have a low incidence, patients with subarachnoid hemorrhage, especially young women, should have angiographic follow-up of no less than 10 years. The detection of these aneurysms before they cause an subarachnoid hemorrhage, substantially improves prognosis, since morbidity and mortality is much lower with the treatment of unruptured aneurysms.

          De novo aneurysm formation 6 and 22 months after initial presentation in two patients.

          Two cases of angiographically and surgically documented de novo intracranial aneurysm formation are reported. The first patient, a 32-year-old woman, developed two new aneurysms within a 6-month period, and the second patient, a 27-year-old woman, developed two new aneurysms within a 22-month period. In both patients, the new aneurysms were symptomatic, causing stroke and subarachnoid hemorrhage, respectively. The development of aneurysms de novo over such a short period of time has important implications for periodic imaging in patients considered to be at high risk for intracranial aneurysm formation.
