      Prevalence of overweight and obesity among Iranian preschoolers: Interrelationship with physical fitness

          Abstract

          Background:

          The preschool years are a crucial time to study the determinants of childhood obesity, as it is when eating and physical activity habits are becoming established. The purpose of this study was to compare the prevalence of overweight and obesity among preschoolers living in the capital of Iran and to determine relationships between overweight and obesity and selected motor- and health-related fitness parameters.

          Materials and Methods:

          This exploratory cross-sectional study was conducted with 190 boys and 191 girls aged 5−6 years. Study children were selected from the kindergartens in Tehran, the capital of Iran. All children underwent anthropometric, motor- and health-related fitness tests. Height, body mass, body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR) and percentage of body fat (PBF) were measured for anthropometric assessments. Sit-and-reach, modified sit-ups, modified pull-ups, the 4 m × 9 m shuttle run, the 20 m sprint test and the 20 m multistage shuttle run test were measured for motor- and health-related fitness tests. Overweight and obesity prevalence was determined by the International Obesity Task Force, Centers for Disease Control and Prevention and World Health Organization standard criteria.

          Results:

          International Obesity Task Force criteria indicate almost 12% (23/190) of boys and 22.5% (43/191) of girls were overweight or obese with 4.73% (9/190) of boys and 10.99% (21/191) of girls in the obese category. Significant correlations were found between modified pull-ups test and body mass, BMI, WC, WHR, WHtR, PBF in boys and modified pull-ups and modified sit-ups tests were significantly correlated with body mass, BMI, WC, WHR, WHtR, and PBF in girls. Compared to their counterparts, overweight and obese boys demonstrated inferior performance in modified pull-ups and predicted VO 2max and overweight and obese girls demonstrated inferior performance in modified pull-ups, modified sit-ups, 4 m × 9 m agility shuttle run and predicted VO 2max

          Conclusion:

          This study highlighted the relatively high prevalence of overweight and obesity in both genders of preschoolers and found that overweight and obesity were associated with poor fitness performances. The findings provided evidence to support the establishment of tailored physical fitness intervention programs to manage and prevent obesity in preschoolers.

            Prevention of non-communicable disease in a population in nutrition transition: Tehran Lipid and Glucose Study phase II

            Background The Tehran Lipid and Glucose Study (TLGS) is a long term integrated community-based program for prevention of non-communicable disorders (NCD) by development of a healthy lifestyle and reduction of NCD risk factors. The study begun in 1999, is ongoing, to be continued for at least 20 years. A primary survey was done to collect baseline data in 15005 individuals, over 3 years of age, selected from cohorts of three medical heath centers. A questionnaire for past medical history and data was completed during interviews; blood pressure, pulse rate, and anthropometrical measurements and a limited physical examination were performed and lipid profiles, fasting blood sugar and 2-hours-postload-glucose challenge were measured. A DNA bank was also collected. For those subjects aged over 30 years, Rose questionnaire was completed and an electrocardiogram was taken. Data collected were directly stored in computers as database software- computer assisted system. The aim of this study is to evaluate the feasibility and effectiveness of lifestyle modification in preventing or postponing the development of NCD risk factors and outcomes in the TLGS population. Design and methods In phase II of the TLGS, lifestyle interventions were implemented in 5630 people and 9375 individuals served as controls. Primary, secondary and tertiary interventions were designed based on specific target groups including schoolchildren, housewives, and high-risk persons. Officials of various sectors such as health, education, municipality, police, media, traders and community leaders were actively engaged as decision makers and collaborators. Interventional strategies were based on lifestyle modifications in diet, smoking and physical activity through face-to-face education, leaflets & brochures, school program alterations, training volunteers as health team and treating patients with NCD risk factors. Collection of demographic, clinical and laboratory data will be repeated every 3 years to assess the effects of different interventions in the intervention group as compared to control group. Conclusion This controlled community intervention will test the possibility of preventing or delaying the onset of non-communicable risk factors and disorders in a population in nutrition transition.               2000 CDC Growth Charts for the United States: methods and development.

              This report provides detailed information on how the 2000 Centers for Disease Control and Prevention (CDC) growth charts for the United States were developed, expanding upon the report that accompanied the initial release of the charts in 2000. The growth charts were developed with data from five national health examination surveys and limited supplemental data. Smoothed percentile curves were developed in two stages. In the first stage, selected empirical percentiles were smoothed with a variety of parametric and nonparametric procedures. In the second stage, parameters were created to obtain the final curves, additional percentiles and z-scores. The revised charts were evaluated using statistical and graphical measures. The 1977 National Center for Health Statistics (NCHS) growth charts were revised for infants (birth to 36 months) and older children (2 to 20 years). New body mass index-for-age (BMI-for-age) charts were created. Use of national data improved the transition from the infant charts to those for older children. The evaluation of the charts found no large or systematic differences between the smoothed percentiles and the empirical data. The 2000 CDC growth charts were developed with improved data and statistical procedures. Health care providers now have an instrument for growth screening that better represents the racial-ethnic diversity and combination of breast- and formula-feeding in the United States. It is recommended that these charts replace the 1977 NCHS charts when assessing the size and growth patterns of infants, children, and adolescents.
