      Heavy alcohol intake is a risk factor for esophageal squamous cell carcinoma among middle-aged men: A case-control and simulation study

          Abstract

          Despite the advances in surgical techniques and treatments, the prognosis of esophageal cancer remains poor, since the disease is usually diagnosed at an advanced stage. Therefore, prevention plays an important role in reducing mortality. Smoking and alcohol intake are modifiable habits and are important risk factors for esophageal cancer. However, the number of large-scale studies that have investigated the association of the amount and duration of smoking and alcohol intake with esophageal cancer risk, while accounting for the effects of gender and cancer subtypes (squamous cell carcinoma and adenocarcinoma), is limited. Therefore, in this hospital-based matched case-control study we investigated this association while accounting for gender and subtype differences. Chinese male patients <60 years of age with esophageal squamous cell carcinoma (ESCC) from the Fourth Hospital of Hebei Medical University in China and healthy individuals were enrolled between January, 2002 and December, 2006. Each ESCC patient was age-matched to a control subject and a total of 535 pairs were enrolled in this study. The combined variables of amount and duration were created to elucidate their effect and association with ESCC. Multiple conditional logistic regression analysis was used to estimate the odds ratio (OR) and 95% confidence interval (CI) in this model, which included a family history of esophageal cancer, a combined smoking variable and a combined alcohol variable. A simulation study was subsequently performed to confirm the reliability of the results. The results of the present study demonstrated that a family history of esophageal cancer and the combined alcohol variable were significantly associated with ESCC risk. Heavy alcohol consumption and intake for ≤20 years increased the risk compared with no intake (OR=1.91, 95% CI: 1.25–2.92). Heavy alcohol consumption and intake for >20 years exhibited an even higher risk (OR=7.25, 95% CI: 3.12–16.83). These results were similar to those of the simulation. Heavy alcohol intake, even for a short duration, is a critical risk factor and may lead to the development of ESCC in Chinese males.

          Prospective study of risk factors for esophageal and gastric cancers in the Linxian general population trial cohort in China.

          Esophageal cancer incidence and mortality rates in Linxian, China are among the highest in the world. We examined risk factors for esophageal squamous cell carcinoma (ESCC), gastric cardia cancer (GCC), and gastric noncardia cancer (GNCC) in a population-based, prospective study of 29,584 adults who participated in the Linxian General Population Trial. All study participants completed a baseline questionnaire that included questions on demographic characteristics, personal and family history of disease, and lifestyle factors. After 15 years of follow-up, a total of 3,410 incident upper gastrointestinal cancers were identified, including 1,958 ESCC, 1,089 GCC and 363 GNCC. Cox proportional hazard models were used to estimate risks. Increased age and a positive family history of esophageal cancer (including ESCC or GCC) were significantly associated with risk at all 3 cancer sites. Additional risk factors for ESCC included being born in Linxian, increased height, cigarette smoking and pipe smoking; for GCC, male gender, consumption of moldy breads and pipe smoking; and for GNCC, male gender and cigarette smoking. Protective factors for ESCC included formal education, water piped into the home, increased consumption of meat, eggs and fresh fruits and increased BMI; for GCC, formal education, water piped into the home, increased consumption of eggs and fresh fruits and alcohol consumption; and for GNCC, increased weight and BMI. General socioeconomic status (SES) is a common denominator in many of these factors and improving SES is a promising approach for reducing the tremendous burden of upper gastrointestinal cancers in Linxian.

            Tobacco smoking and cancer: a meta-analysis.

            We conducted a systematic meta-analysis of observational studies on cigarette smoking and cancer from 1961 to 2003. The aim was to quantify the risk for 13 cancer sites, recognized to be related to tobacco smoking by the International Agency for Research on Cancer (IARC), and to analyze the risk variation for each site in a systematic manner. We extracted data from 254 reports published between 1961 and 2003 (177 case-control studies, 75 cohorts and 2 nested case-control studies) included in the 2004 IARC Monograph on Tobacco Smoke and Involuntary Smoking. The analyses were carried out on 216 studies with reported estimates for 'current' and/or 'former' smokers. We performed sensitivity analysis, and looked for publication and other types of bias. Lung (RR = 8.96; 95% CI: 6.73-12.11), laryngeal (RR = 6.98; 95% CI: 3.14-15.52) and pharyngeal (RR = 6.76; 95% CI: 2.86-15.98) cancers presented the highest relative risks (RRs) for current smokers, followed by upper digestive tract (RR = 3.57; 95% CI: 2.63-4.84) and oral (RR = 3.43; 95% CI: 2.37-4.94) cancers. As expected, pooled RRs for respiratory cancers were greater than the pooled estimates for other sites. The analysis of heterogeneity showed that study type, gender and adjustment for confounding factors significantly influence the RRs estimates and the reliability of the studies.               Epidemiologic differences in esophageal cancer between Asian and Western populations

              Esophageal cancer is a common cancer worldwide and has a poor prognosis. The incidence of esophageal squamous cell cancer has been decreasing, whereas the incidence of esophageal adenocarcinoma has been increasing rapidly, particularly in Western men. Squamous cell cancer continues to be the major type of esophageal cancer in Asia, and the main risk factors include tobacco smoking, alcohol consumption, hot beverage drinking, and poor nutrition. In contrast, esophageal adenocarcinoma predominately affects the whites, and the risk factors include smoking, obesity, and gastroesophageal reflux disease. In addition, Asians and Caucasians may have different susceptibilities to esophageal cancer due to different heritage backgrounds. However, comparison studies between these two populations are limited and need to be addressed in the near future. Ethnic differences should be taken into account in preventive and clinical practices.
