      Overweight and Undernutrition in the Cases of School-Going Adolescents in Wolaita Sodo Town, Southern Ethiopia: Cross-Sectional Study

          Abstract

          Background

          This study aimed to assess the prevalence of malnutrition and associated factors among school adolescents in Wolaita Sodo town, Southern Ethiopia.

          Methods

          A school-based cross-sectional study was conducted from May 18–June 10, 2015. A multistage sampling was used to select a random sample of 690 adolescents from selected schools. Data on sociodemographic information were collected by using an interviewer-administered questionnaire, and anthropometric measurements were made by using a digital Seca scale and height measuring board by trained data collectors. Data were entered into Epi-Data version 3.1 software and exported to SPSS version 20. World Health Organization (WHO) Anthro-plus software was used to analyze anthropometric data. Both binary and multinomial logistic regression analyses were done to identify factors associated with the malnutrition of adolescents.

          Result

          The overall prevalence of thinness, stunting, and overweight/obesity among school adolescents was 4.7% (95% CI: 3%–6.4%); 5.2% (95% CI: 3.4%–7%); and 5.0% (95% CI: 3.4%–6.7%), respectively. Being male (AOR = 4.07; 95% CI: 2.35–7.02), learning at a government school (AOR = 0.37; 95% CI: 0.20–0.65), mothers with no formal education (AOR = 4.03; 95% CI: 1.82–8.92), owning no cattle (AOR = 4.92; 95% CI: 2.08–11.64), skipping meals (AOR = 1.70; 95% CI: 1.05–2.74), and illness in 2 weeks prior to survey (AOR = 2.67; 95% CI: 1.49–4.78) were significantly associated with thinness. However, males, students who had their house, and no cattle were more likely to develop overweight/obesity. Maternal education of secondary school (AOR = 0.214; 95% CI: 0.054–0.846) was significantly associated with the stunting.

          Conclusion

          The study showed the coexistence of undernutrition and overnutrition among school adolescents in the study area. There needs to implement evidence-based school nutrition education and health policies and programs to improve nutritional status of adolescents and timely taking action to limit obesity-related health problems.

          Boys are more stunted than girls in Sub-Saharan Africa: a meta-analysis of 16 demographic and health surveys

          Background Many studies in sub-Saharan Africa have occasionally reported a higher prevalence of stunting in male children compared to female children. This study examined whether there are systematic sex differences in stunting rates in children under-five years of age, and how the sex differences in stunting rates vary with household socio-economic status. Methods Data from the most recent 16 demographic and health surveys (DHS) in 10 sub-Saharan countries were analysed. Two separate variables for household socio-economic status (SES) were created for each country based on asset ownership and mothers' education. Quintiles of SES were constructed using principal component analysis. Sex differentials with stunting were assessed using Student's t-test, chi square test and binary logistic regressions. Results The prevalence and the mean z-scores of stunting were consistently lower amongst females than amongst males in all studies, with differences statistically significant in 11 and 12, respectively, out of the 16 studies. The pooled estimates for mean z-scores were -1.59 for boys and -1.46 for girls with the difference statistically significant (p < 0.001). The stunting prevalence was also higher in boys (40%) than in girls (36%) in pooled data analysis; crude odds ratio 1.16 (95% CI 1.12–1.20); child age and individual survey adjusted odds ratio 1.18 (95% CI 1.14–1.22). Male children in households of the poorest 40% were more likely to be stunted compared to females in the same group, but the pattern was not consistent in all studies, and evaluation of the SES/sex interaction term in relation to stunting was not significant for the surveys. Conclusion In sub-Saharan Africa, male children under five years of age are more likely to become stunted than females, which might suggest that boys are more vulnerable to health inequalities than their female counterparts in the same age groups. In several of the surveys, sex differences in stunting were more pronounced in the lowest SES groups.

            Eating patterns, dietary quality and obesity.

            Obesity among children has reached epidemic proportions. Today, an estimated one in four children in the United States is overweight. while 11% arc obese. Children who are overweight tend to remain so up to 20 years of age; in general, they have a 1.5- to twofold higher risk for becoming overweight as adults. The prevalence of overweight has increased approximately twofold in the 20-year period from 1974 to 1994, with the largest increases observed among 19- to 24-year-olds. The annual increases in weight and obesity that occurred from 1983 to 1994 were 50% higher than those from 1973 to 1982. Overweight youth are 2.4 times as likely to have a high serum total cholesterol level, and 43.5 times as likely to have three cardiovascular risk factors. Although the total energy intake of children has remained the same, and the macronutrient density of the diet has changed, the percentage of energy from fat has decreased, while that from carbohydrates and protein has increased. Children have been consuming lower amounts of fats/oils, vegetables/soups, breads/grains, mixed meats, desserts, candy, and eggs. and increasing amounts of fruits/fruit juices, beverages. poultry, snacks, condiments, and cheese. Changes in specific eating patterns may explain the increase in adiposity among children; e.g., increases have occurred in the number of meals eaten at restaurants, food availability, portion sizes, snacking and meal-skipping. Successful prevention and treatment of obesity in childhood could reduce the adult incidence of cardiovascular disease. Because substantial weight loss is difficult to maintain, the prevention of obesity by promoting healthier lifestyles should be one of our highest priorities in the new millennium.

              Maternal education and child nutritional status in Bolivia: finding the links.

              This study models various pathways linking maternal education and child nutritional status in Bolivia, using a national sample of children. Pathways examined include socioeconomic status, health knowledge, modern attitudes towards health care, female autonomy, and reproductive behavior. The data come from the 1998 Bolivia Demographic and Health Survey. Logistic regression results suggest that socioeconomic factors are the most important pathways linking maternal education and child nutritional status, and that modern attitudes about health care also explain the impact of education. Health care knowledge accounts for less of the effect of maternal education on child nutritional status, with autonomy being the weakest pathway. Other pathways, such as reproductive behaviors, appear to influence nutritional status independent of maternal education. Overall, the pathways examined accounted for 60 percent of the effect of maternal education on child nutritional status.
