Zusammenfassung. Akute gastrointestinale Infektionen mit ihrer Hauptmanifestation des Brechdurchfalls sind ein in der ambulanten Medizin häufig anzutreffendes Krankheitsbild. Der meist selbstlimitierende Verlauf bedingt, dass auf eine ätiologische Abklärung häufig verzichtet werden kann. Virale Infektionen, hauptsächlich ausgelöst durch Noro- und Rotaviren stellen die grösste Gruppe dar, während bakterielle Infektionen aufgrund der guten Hygieneverhältnisse in entwickelten Ländern erst in zweiter Linie anzutreffen sind. Hauptvertreter hierbei sind der Campylobacter sowie Salmonellen. Im Spitalsetting und inbesondere nach vorgängiger antibiotischer Therapie muss an eine Infektion durch Clostridium difficile gedacht und der Erreger aktiv gesucht werden. Ein Spezialfall stellt die Diarrhö auf Reisen dar, wobei diese meist durch enterovirulente E. coli ausgelöst wird. In der vorliegenden Zusammenstellung werden die wichtigsten Erreger im Einzelnen beschrieben.
Abstract. Acute gastrointestinal infections and diarrhea with vomiting as its main presentation are a frequently encountered entity in an outpatient setting. Due to a mostly self-limiting disease course a diagnostic work-up is often futile. Viral infections caused by Noro- or Rotavirus are most frequent, while bacterial infections are second line due to high hygienic standards in developed countries. In an inpatient setting and after a precedent antibiotic treatment one should think of clostridium difficile. Traveler’s diarrhea represents a special case, with most of the cases caused by enterovirulent E. coli. In this mini review we describe the most important pathogens in detail.
Résumé. Les gastroentérites aiguës, dont la manifestation principale est la gastro-entérite, sont très fréquentes en médicine ambulatoire. En raison de son évolution la plupart du temps auto-limitée, on peut fréquemment renoncer à un diagnostic étiologique. Les infections virales (principalement provoquées par les norovirus, rotavirus) constituent le plus grand groupe alors que les infections bactérielles en raison du bon niveau d’hygiène dans le monde développé ne constituent que le deuxième groupe (les bactéries principales étant le campylobacter et la salmonelle). Dans un milieu hospitalier, notamment après un traitement antibiotique, il faut penser au clostridium difficile qui doit être recherché activement. La diarrhée du voyageur est un cas à part qui est souvent provoquée par E. coli entérovirulente. Dans cette récapitulation les pathogènes plus importants sont décrits individuellement.
