          Abstract

          Resumen Introducción: El estudio y análisis del uso prolongado de Benzodiacepinas (BZD) sigue siendo un objetivo estratégico para todos los niveles asistenciales y en especial para la Atención Farmacéutica. Los cuestionarios como herramienta de investigación científica proporcionan una escala de medición que permite evaluar y comparar individuos con parámetros de fiabilidad y validez. Objetivo: Diseñar un cuestionario de recogida de datos (CDR) que permita la personalización de la dispensación de BZD según las características de los usuarios. Método: Revisión bibliográfica de las evidencias científicas existentes con expertos en Atención Farmacéutica, diseño de la encuesta e implementación piloto (pre-testing) del CRD con usuarios de BZD, y depuración de los ítems y formato de éstos a incluir en el CDR final. Resultados: El CDR diseñado y distribuido en 8 dimensiones, abarca parámetros sociodemográficos, de conocimiento de la medicación, el test de adherencia Morisky-Green, la mediación de calidad de vida por Euroqol 5D3L y escala visual EVA, el test de Pfeiffer para deterioro cognitivo, el test de dependencia a BZD, la opción de registro de la intervención farmacéutica realizada y aceptación de la misma por parte del usuario y el grado de satisfacción del paciente sobre la intervención. Conclusiones: El CDR es un innovador instrumento práctico transferible a la práctica asistencial que permite caracterizar al paciente usuario de BZD en el momento de la dispensación en la farmacia comunitaria permitiendo individualizar y sistematizar la intervención farmacéutica al tiempo que fomentar el uso seguro y eficaz de la BZD por parte del paciente.

          Abstract Introduction: The study and analysis of the prolonged use of Benzodiazepines (BZD) remains a strategic objective for all levels of care and especially for Pharmaceutical Care. Questionnaires as a scientific research tool provide a measurement scale that allows the evaluation and comparison of individuals with parameters of reliability and validity. Objective: To design a data collection questionnaire (CDR) that allows the personalization of BZD dispensing according to the characteristics of the users Methods: Bibliographic review of existing scientific evidence with experts in Pharmaceutical Care, design of the survey and pilot implementation (pre-testing) of the CRD with BZD users, and refinement of the items and their format to be included in the final CDR. Results: The CDR designed and distributed in 8 dimensions, covers socio-demographic parameters, knowledge of medication, the Morisky-Green adherence test, quality of life mediation by Euroqol 5D3L and EVA visual scale, the Pfeiffer test for cognitive impairment, the BZD dependence test, the option of recording the pharmaceutical intervention carried out and its acceptance by the user and the degree of patient satisfaction with the intervention Conclusions: The CDR is an innovative practical tool that can be transferred to healthcare practice, allowing the patient to characterize the BZD user at the time of dispensing in the community pharmacy, enabling the individualization and systematization of the pharmaceutical intervention while promoting the safe and effective use of the BZD by the patient.

          Research Methods in Healthcare Epidemiology: Survey and Qualitative Research

          Surveys are one of the most frequently employed study designs in healthcare epidemiology research. Generally easier to undertake and less costly than many other study designs, surveys can be invaluable to gain insights into opinions and practices in large samples and may be descriptive and/or be used to test associations. In this context, qualitative research methods may complement this study design either at the survey development phase and/or at the interpretation/extension of results stage. This methods article focuses on key considerations for designing and deploying surveys in healthcare epidemiology and antibiotic stewardship, including identification of whether or not de novo survey development is necessary, ways to optimally lay out and display a survey, denominator measurement, discussion of biases to keep in mind particularly in research using surveys, and the role of qualitative research methods to complement surveys. We review examples of surveys in healthcare epidemiology and antimicrobial stewardship and review the pros and cons of methods used. A checklist is provided to help aid design and deployment of surveys in healthcare epidemiology and antimicrobial stewardship. Infect Control Hosp Epidemiol 2016;1–6

              Barriers and facilitators of effective self-management in asthma: systematic review and thematic synthesis of patient and healthcare professional views

              Self-management is an established, effective approach to controlling asthma, recommended in guidelines. However, promotion, uptake and use among patients and health-care professionals remain low. Many barriers and facilitators to effective self-management have been reported, and views and beliefs of patients and health care professionals have been explored in qualitative studies. We conducted a systematic review and thematic synthesis of qualitative research into self-management in patients, carers and health care professionals regarding self-management of asthma, to identify perceived barriers and facilitators associated with reduced effectiveness of asthma self-management interventions. Electronic databases and guidelines were searched systematically for qualitative literature that explored factors relevant to facilitators and barriers to uptake, adherence, or outcomes of self-management in patients with asthma. Thematic synthesis of the 56 included studies identified 11 themes: (1) partnership between patient and health care professional; (2) issues around medication; (3) education about asthma and its management; (4) health beliefs; (5) self-management interventions; (6) co-morbidities (7) mood disorders and anxiety; (8) social support; (9) non-pharmacological methods; (10) access to healthcare; (11) professional factors. From this, perceived barriers and facilitators were identified at the level of individuals with asthma (and carers), and health-care professionals. Future work addressing the concerns and beliefs of adults, adolescents and children (and carers) with asthma, effective communication and partnership, tailored support and education (including for ethnic minorities and at risk groups), and telehealthcare may improve how self-management is recommended by professionals and used by patients. Ultimately, this may achieve better outcomes for people with asthma.
