          Abstract

          Resumen La epidemia de opioides declarada en Estados unidos ha suscitado una profunda preocupación y polémica en torno al uso de estos fármacos. Aunque este problema no se ha extendido a Europa hasta el momento, se considera importante estudiar cuáles han podido ser los errores que se han cometido en Estados Unidos en el abordaje de esta epidemia. En este trabajo se identificaron cuatro errores respecto a las premisas adoptadas por los Estados Unidos para establecer la estrategia de lucha contra la epidemia: falta de adecuada identificación de las causas y el origen de la epidemia, error en la identificación de culpables, la mezcla de datos correspondientes a opioides de prescripción con los de opioides ilegales y una incorrecta cuantificación del problema. Como consecuencia de estos errores las soluciones y medidas aplicadas fueron ineficaces, tardías y estuvieron dirigidas a unos objetivos incorrectos.

          Translated abstract

          Abstract The opioids epidemic declared in the United States of America has generated a deep preoccupation and controversy in relation to the use of these drugs. Although this problem has not been extended to Europe until today, it seems relevant to analyze the errors that have been made by United States. In this article we have identified four errors related to the premises adopted by this country in the design of the strategy to fight the epidemic: inadequate identification of causes and origin of the epidemic, error in the identification of guilty, mixing of data related to prescription opioids and illegal opioids and an incorrect quantification of the problem. As consequence of these errors, the applied solutions and measures were inefficacious, late and were directed towards incorrect targets.

          A Brief History of the Opioid Epidemic and Strategies for Pain Medicine

          The opioid epidemic has resulted from myriad causes and will not be solved by any simple solution. Consequent to a staggering increase in opioid-related deaths in the USA, various governmental inputs and stakeholder strategies have been proposed and implemented with varying success. This article summarizes the history of opioid use and explores the causes for the present day epidemic. Recent trends in opioid-related data demonstrate an almost fourfold increase in overdose deaths from 1999 to 2008. Tragically, opioids claimed over 64,000 lives just last year. Some solutions have undergone legislation, including the limitation of numbers of opioids postsurgery, as well as growing national prevalence of enhanced recovery after surgery protocols which focus on reduced postoperative opioid consumption and shortened hospital stays. Stricter prescribing practices and prescription monitoring programs have been instituted in the recent past. Improvement in abuse deterrent strategies which is a major focus of the Food and Drug Administration (FDA) for all opioid preparations will likely play an important role by increasing the safety of these medications. Future potential strategies such as additional legislative policies, public awareness, and physician education are also detailed in this review.

              Pain as the fifth vital sign: exposing the vital need for pain education.

              The push to evaluate pain in patients as exemplified by the fifth vital sign has exposed serious deficits in practitioner education and training in pain assessment and management because patient report of pain level has become commonplace in clinical practice. The rapid increase in prescription opioid medications suggests that practitioners are trying to address their patients' pain by prescribing opioids. However, the increase in prescription opioids has also been associated with an increase in prescription opioid-related unintended deaths. In clinical practice, the fifth vital sign has proven to be more complex to assess, evaluate, and manage than originally anticipated. Expanding pain education and training is critical to remedying some of the issues the routine report of pain by patients has uncovered.
