      The Prevalence of Intestinal Parasitic Infections Among Kosovar and Serbian School-children in Kosovo

          Summary

          The aim of this study was to estimate the prevalence and distribution of species of intestinal parasites in the Balkans on the example of Kosovar and Serbian residents from the region of Gnjilane ineastern Kosovo,where the epidemiology of intestinal parasitic infections remains unknown. Parasitological examination of stools pecimens was performed in 2017 and 2018. Stool samples were collected from asymptomatic school-children aged 6–17 years: 530 Kosovars from the municipality of Kaçanik and310 Serbs from the municipalities of Kamenica and Strpce. Each patient provided two stool samples collected every second day,fixed in SAF preservative and 70 % spiritus vini, transported to the Department of Epidemiology and Tropical Medicine at the Military Institute of Medicine in Poland,and tested by light microscopy using three diagnostic methods: directs mearin Lugol’s solution,decantation in distilled water, and Fülleborn’s flotation. A total of 101 Kosovar children (19.1 % of the study group)were found to be infected with intestinal parasites: nematodes (n=20), cestodes (n=2), trematodes (n=2), and protozoa (n=79). Only 13 Serbian children (4.2 %) were found to beinfected with nematodes (n=4),cestodes (n=3),and protozoa (n=6). Giardia intestinalis was themost prevalent intestinal parasite in both groups (14,9 % vs. 1.9 % children). The prevalence ofasymptomatic parasitic infections was significantly higher in Kosovars in comparison to the Serbsliving in the same region of eastern Kosovo. This fact shows that there may be significant differencesin the quality of health care and sanitation as well as feed hygiene between these two communities.

            The drugs we have and the drugs we need against major helminth infections.

            Parasitic worms (helminths) have accompanied humans for thousands of years and, still today, they are pervasive where poverty persists, including large parts of Southeast Asia and the Western Pacific Region. The global strategy for the control of helminth infections is morbidity control and elimination as a public health problem. Regular administration of anthelminthic drugs to at-risk populations (e.g. school-aged children) serves as the backbone of interventions in areas where helminth infections are highly endemic. In this review, we focus on soil-transmitted helminthiasis (ascariasis, hookworm disease, strongyloidiasis and trichuriasis) and food-borne trematodiasis (clonorchiasis, fascioliasis, intestinal fluke infections, opisthorchiasis and paragonimiasis) and discuss the few drugs that are currently available for their treatment and control. Emphasis is placed on efficacy with new light shed on multiple dosing and combination therapy. We summarise recent advances made with anthelminthic drugs that might become the future armentarium for the control of major helminthiasis (e.g. artemisinins, cyclooctadepsipeptides, mefloquine, monepantel, nitazoxandide, synthetic peroxides and tribendimidine). Issuing from our review are current research gaps and the need for concerted efforts to discover, develop and deploy the next generation of anthelminthic drugs. Copyright 2010 Elsevier Ltd.               Common intestinal parasites.

              Intestinal parasites cause significant morbidity and mortality. Diseases caused by Enterobius vermicularis, Giardia lamblia, Ancylostoma duodenale, Necator americanus, and Entamoeba histolytica occur in the United States. E. vermicularis, or pinworm, causes irritation and sleep disturbances. Diagnosis can be made using the "cellophane tape test." Treatment includes mebendazole and household sanitation. Giardia causes nausea, vomiting, malabsorption, diarrhea, and weight loss. Stool ova and parasite studies are diagnostic. Treatment includes metronidazole. Sewage treatment, proper handwashing, and consumption of bottled water can be preventive. A. duodenale and N. americanus are hookworms that cause blood loss, anemia, pica, and wasting. Finding eggs in the feces is diagnostic. Treatments include albendazole, mebendazole, pyrantel pamoate, iron supplementation, and blood transfusion. Preventive measures include wearing shoes and treating sewage. E. histolytica can cause intestinal ulcerations, bloody diarrhea, weight loss, fever, gastrointestinal obstruction, and peritonitis. Amebas can cause abscesses in the liver that may rupture into the pleural space, peritoneum, or pericardium. Stool and serologic assays, biopsy, barium studies, and liver imaging have diagnostic merit. Therapy includes luminal and tissue amebicides to attack both life-cycle stages. Metronidazole, chloroquine, and aspiration are treatments for liver abscess. Careful sanitation and use of peeled foods and bottled water are preventive.
