          Abstract

          Se realizó un estudio prospectivo de los pacientes que fueron atendidos en el Servicio de Urgencias del Hospital Docente Clinicoquirúrgico "Vladimir Ilich Lenin" con lesiones traumáticas de la muñeca, entre enero de 1993 y diciembre de 1995. Cuarenta y ocho pacientes presentaron como complicación una inestabilidad del carpo. Ésta fue más frecuente en mujeres y posterior a una fractura de Colles. El 35,41 % requirió tratamiento quirúrgico. Se discute el diagnóstico y el tratamiento.

          Une étude prospective des patients traités dans le Service d'Urgence de l'Hôpital Clinico-Chirurgical Universitaire "Vladimir Ilich Lenin", du fait des lésions traumatiques du poignet, a été réalisée entre janvier 1993 et décembre 1995. Quarante-huit pacients ont présenté en tant que complication une instabilité du carpe. La instabilité a été plus fréquente chez les femmes et a survenu après une fracture de Colles. Le 35,41 % a demandé un traitement chirurgical. Le diagnostic et le traitement sont discutés.

          Translated abstract

          A prospective study of the patients that were attended at the Urgency Department of the "Vladimir Ilich Lenin" Clinical and Surgical Teaching Hospital with wrist traumatic injuries from January 1993, to December 1995, was conducted; 48 patients had carpal instability as a complication. This instability was more frequent in women and after a Colles' fracture; 35.41 % needed surgical treatment. The diagnosis and treatment are discussed.

            Extrinsic wrist ligaments in the pathomechanics of ulnar translation instability.

            A description of the various combinations of ligament disruptions necessary for ulnar translation instability is presented based on a series of cadaver dissections and load studies. Dissections and load studies demonstrated that major ligament disruption, even to the extent that only the dorsal ulnolunate, the palmar ulnolunate or the radioscaphocapitate, and the long radiolunate ligaments were left intact, failed to result in meaningful ulnar translation of the carpus. The radioscaphocapitate ligament alone could not prevent significant ulnar translation. Palmar translation instability was evident with less ligament disruption than that needed for ulnar translation and was always evident if there was ulnar translation. These studies imply that ulnar translation represents a much more global ligament disruption and instability than previously suggested in the literature.

              Extra-articular fractures of the distal radius.

              Extra-articular distal radius fractures frequently are undertreated. Results correlate directly with restoration of anatomy. Immobilization in plaster is an inferior method of preserving radial length and palmar tilt. Failure to maintain reduction with plaster can be predicted by the initial injury, if reduction is lost, closed reduction of the redisplaced fracture should be accompanied by external fixation.
