          Abstract

          Resumen El artículo reflexiona sobre la historia de la Sociedad Chilena de Radiología después de 80 años de existencia y plantea diversas consideraciones para el futuro de la especialidad. Se destaca el crecimiento de la tecnología de imagenología en Chile, mejorando el acceso a los exámenes diagnósticos, a pesar de lo cual, el acceso terapéutico sigue siendo limitado en algunas regiones. Se plantea la importancia de la educación en radiología, no solo para formar a nuevos radiólogos, sino también para capacitar a los clínicos en el uso adecuado de los exámenes de imagen. Se sugiere fortalecer los esfuerzos de la radiología para involucrarse en la Salud Pública y la defensa de los consumidores. Se menciona el impacto de la Inteligencia Artificial (IA) en la radiología y su potencial. Se mencionan propuestas provocativas para el futuro de la radiología tales como repensar la forma en que se realizan los informes radiológicos. También se destaca la importancia de cuidar el bienestar de los radiólogos y fomentar la integridad y el trabajo en equipo. En general, el artículo reflexiona sobre los desafíos y oportunidades que enfrenta la radiología en Chile, y plantea ideas para mejorar la práctica y prepararse para el futuro.

          Translated abstract

          Abstract The article reflects on the history of the Chilean Society of Radiology after 80 years of existence and raises several considerations for the future of the specialty. It highlights the growth of imaging technology in Chile, improving access to diagnostic tests, although therapeutic access is still limited in some regions. The importance of education in radiology is raised, not only to train new radiologists, but also to train clinicians in the proper use of imaging tests. It is suggested to strengthen the efforts of radiology to get involved in Public Health and consumer advocacy. The impact of Artificial Intelligence (AI) on radiology and its potential is mentioned. Provocative proposals for the future of radiology such as rethinking the way radiology reports are made are mentioned. The importance of caring for the well-being of radiologists and fostering integrity and teamwork is also highlighted. Overall, the article reflects on the challenges and opportunities facing radiology in Chile, and puts forward ideas for improving practice and preparing for the future.

          High-performance medicine: the convergence of human and artificial intelligence

          The use of artificial intelligence, and the deep-learning subtype in particular, has been enabled by the use of labeled big data, along with markedly enhanced computing power and cloud storage, across all sectors. In medicine, this is beginning to have an impact at three levels: for clinicians, predominantly via rapid, accurate image interpretation; for health systems, by improving workflow and the potential for reducing medical errors; and for patients, by enabling them to process their own data to promote health. The current limitations, including bias, privacy and security, and lack of transparency, along with the future directions of these applications will be discussed in this article. Over time, marked improvements in accuracy, productivity, and workflow will likely be actualized, but whether that will be used to improve the patient-doctor relationship or facilitate its erosion remains to be seen.

            Evidence for overuse of medical services around the world.

            Overuse, which is defined as the provision of medical services that are more likely to cause harm than good, is a pervasive problem. Direct measurement of overuse through documentation of delivery of inappropriate services is challenging given the difficulty of defining appropriate care for patients with individual preferences and needs; overuse can also be measured indirectly through examination of unwarranted geographical variations in prevalence of procedures and care intensity. Despite the challenges, the high prevalence of overuse is well documented in high-income countries across a wide range of services and is increasingly recognised in low-income countries. Overuse of unneeded services can harm patients physically and psychologically, and can harm health systems by wasting resources and deflecting investments in both public health and social spending, which is known to contribute to health. Although harms from overuse have not been well quantified and trends have not been well described, overuse is likely to be increasing worldwide.
