      Practices of Healthcare Workers regarding Infection Prevention in Bale Zone Hospitals, Southeast Ethiopia

          Abstract

          Introduction. In Ethiopia, infection prevention to protect patients, healthcare workers, and visitors from healthcare-acquired infections is one of a number of nationwide transformational initiatives to ensure the provision of quality healthcare services. The aim of this research was to assess the practice of healthcare workers regarding infection prevention and its associated factors in Bale zone Hospitals. Methods. A cross-sectional study targeted 402 healthcare workers using simple random sampling to learn about their practices related to infection prevention. Data were collected in interviews using pretested, structured questionnaires. Returned questionnaires were checked for completeness and then data were entered into a database and analyzed using SPSS Version 20. Adjusted odd ratio (AOR) with a 95% confidence interval was calculated to determine the strength of association, and variables with a p value <0.05 in the final model were considered as statistically significant. Results. Three hundred ninety-four healthcare workers participated in the study. Of these; 145 (36.8%, 95% CI 32, 42%) of them were found to have self-reported good infection prevention practice. Good knowledge towards infection prevention (AOR = 1.84, 95% CI 1.02, 3.31), availability of personal protective equipment (AOR = 1.96, 95% CI 1.16, 3.32), and water (AOR = 4.42, 95% 2.66, 7.34) at workplace were found to have a statistically significant association with healthcare workers self-reported good infection prevention practices. Conclusions. In this study, slightly more than one-third of the healthcare workers reported to have good infection prevention practice. Good knowledge towards infection prevention, working in departments, availability of personal protective equipment, and water at work place were found to have statistically significant association with self-reported good infection prevention practices.

          Estimation of the global burden of disease attributable to contaminated sharps injuries among health-care workers.

          The global burden of hepatitis B (HBV), hepatitis C (HCV), and human immunodeficiency virus (HIV) infection due to percutaneous injuries among health care workers (HCWs) is estimated. The incidence of infections attributable to percutaneous injuries in 14 geographical regions on the basis of the probability of injury, the prevalence of infection, the susceptibility of the worker, and the percutaneous transmission potential are modeled. The model also provides the attributable fractions of infection in HCWs. Overall, 16,000 HCV, 66,000 HBV, and 1,000 HIV infections may have occurred in the year 2000 worldwide among HCWs due to their occupational exposure to percutaneous injuries. The fraction of infections with HCV, HBV, and HIV in HCWs attributable to occupational exposure to percutaneous injuries fraction reaches 39%, 37%, and 4.4% respectively. Occupational exposures to percutaneous injuries are substantial source of infections with bloodborne pathogens among health-care workers (HCWs). These infections are highly preventable and should be eliminated. 2005 Wiley-Liss, Inc.

            Standard Precautions: Occupational Exposure and Behavior of Health Care Workers in Ethiopia

            Background Occupational exposure to blood and body fluids is a serious concern for health care workers, and presents a major risk for the transmission of infections such as HIV and hepatitis viruses. The objective of this study was to investigate occupational exposures and behavior of health care workers (HCWs) in eastern Ethiopia. Methods We surveyed 475 HCWs working in 10 hospitals and 20 health centers in eastern Ethiopia using a structured questionnaire with a response rate of 84.4%. Descriptive statistics and multivariate analysis using logistic regression were performed. Results Life time risks of needle stick (30.5%; 95% CI 26.4–34.6%) and sharps injuries (25.7%; 95% CI 21.8–29.6%) were high. The one year prevalence of needle stick and sharps injury were 17.5% (95% CI 14.1–20.9%) and 13.5% (95% CI 10.4–16.6%) respectively. There was a high prevalence of life time (28.8%; 95% CI = 24.7–32.9%) and one year (20.2%; 95% CI = 16.6–23.8%) exposures to blood and body fluids. Two hundred thirteen (44.8%) HCWs reported that they were dissatisfied by the supply of infection prevention materials. HCWs had sub-optimal practices and unfavorable attitudes related to standard precautions such as needle recapping (46.9%) and discriminatory attitudes (30.5%) toward HIV/AIDS patients. Conclusion There was a high level of exposure to blood and body fluids among HCWs. We detected suboptimal practices and behavior that put both patients and HCWs at significant risk of acquiring occupational infections. Health authorities in the study area need to improve the training of HCWs and provision of infection prevention equipment. In addition, regular reporting and assessment of occupational exposures need to be implemented.

              Infection Prevention Practices and Associated Factors among Healthcare Workers in Governmental Healthcare Facilities in Addis Ababa

              Background Failure to follow proper infection prevention practices puts healthcare workers, patients and communities at risk. Despite the increases of highly contagious infections, infection prevention practices among healthcare workers is unknown in many developing countries. The need to understand infection prevention practices is important for prevention and control of nosocomial infections. Therefore, the objective of this study was to assess infection prevention practices and associated factors among healthcare workers in governmental healthcare facilities in Addis Ababa, Ethiopia. Methods A facility based cross-sectional study design was conducted from February to March 2016, in Addis Ababa among 629 healthcare workers who were selected by multistage sampling technique from 30 governmental healthcare facilities. Data were collected using pre-tested interviewer administered structured questionnaire. Data were entered into Epi -data 3.1 and exported to SPSS version 20 for analysis. Multivariable logistic regression model was used to identify factors associated with infection prevention practices.. Findings were presented using odds ratios with their 95% confidence intervals., A p-value less than 0.05 were used to declare statistical significance. Results Six hundred five (96.2%) healthcare workers participated in the study. Four hundred (66.1%, (95%CI: 62.1%–70.1%)) health care workers had good infection prevention practices. Having good knowledge on infection prevention meausures (AOR =1.53, 95%CI: 1.05–2.22), having positive attitude towards infection prevention practices (AOR=2.03, 95%CI: 1.26–3.26), having awareness on availability of standard operating procedures (AOR=1.97, 95%CI: 1.34–2.93) and presence of continuous water supply (AOR=1.68, 95%CI: 1.11–2.56) were predictors of good infection prevention practices. Conclusion Two-third of the healthcare workers had good infection prevention practices. Having good knowledge on infection prevention measures, having positive attitude towards infection prevention practices, having awareness on availability of standard operating procedures and presence of continuous water supply were predictors of good infection prevention practices. To sustain good practices, adequate pre-service and in-service training should be in place to equip and update health care workers about infection prevention precautions. The need for continuous supervision should be implemented to strengthen adherence for infection prevention practices among workers along with sustainable and reliable water supply is crucial.
