          Abstract

          Resumo O objetivo do estudo foi analisar a associação entre o sobrepeso e a obesidade com a pressão arterial elevada (PAE), de acordo com a prática esportiva de jovens. Participaram do estudo 636 jovens de 10 a 17 anos da cidade de Londrina-PR. Foram analisadas as variáveis sexo, idade, escolaridade paterna, comportamento sedentário, prática esportiva, estado nutricional e pressão arterial. A regressão de Poisson foi utilizada para estimar a razão de prevalência (RP) e os intervalos de confiança de 95%. Jovens com sobrepeso e obesidade apresentaram maiores prevalências de PAE, comparados aos eutróficos, quando analisadas a amostra total (25,7 e 29,5 vs 15,2%) e os não praticantes de esportes (29,7 e 33,3 vs 15,1%), o mesmo não ocorrendo com os praticantes de esportes (17,1 e 18,2 vs 15,5%). Foram encontradas associações positivas entre o sobrepeso e a obesidade com a PAE na amostra total (RP = 1,60, 1,02-2,52 e 1,93, 1,15-3,25) e nos jovens não praticantes de esportes (RP = 1,80, 1,05-3,14 e 2,15, 1,10-4,16). Para os jovens praticantes de esportes não foram encontradas associações entre sobrepeso e obesidade com a PAE (RP = 1,01, 0,36-2,82 e 1,09, 0,48-2,48). O excesso de peso não se associou com a PAE em jovens praticantes de esportes, sugerindo proteção cardiovascular nos jovens com sobrepeso e obesidade.

          Translated abstract

          Abstract The aim of this study was to analyze the association between overweight and obesity with high blood pressure (HBP) according to sports practice in young people. Took part in this study 636 young people aged 10 to 17 years of Londrina, Parana State, Brazil. Sex, age, parental education, sedentary behavior, sports practice, nutritional status and blood pressure were analysed. Poisson regression was used to estimate the prevalence ratio (PR) and 95% confidence intervals. Overweight and obesity young people showed higher prevalence of HBP compared to eutrophics when analysed the total sample (25,7 e 29,5 vs 15,2%) and non sport practitioners (29,7 e 33,3 vs 15,1%), which did not occur with sports practitioners (17,1 e 18,2 vs 15,5%). Positive associations were found between overweight and obesity with HBP in total sample (PR = 1,60, 1,02-2,52 and 1,93, 1,15-3,25) and on non sport practitioners (RP = 1,80, 1,05-3,14 and 2,15, 1,10-4,16). For young people sports practitioners were not found associations between overweight and obesity with HBP (PR = 1,01, 0,36-2,82 and 1,09, 0,48-2,48). Weight excess was not associated with HBP in young people sports practitioners, suggesting cardiovascular protection in young people with overweight and obesity.

          Trends in cardiovascular health metrics and associations with all-cause and CVD mortality among US adults.

          Recent recommendations from the American Heart Association aim to improve cardiovascular health by encouraging the general population to meet 7 cardiovascular health metrics: not smoking; being physically active; having normal blood pressure, blood glucose and total cholesterol levels, and weight; and eating a healthy diet. To examine time trends in cardiovascular health metrics and to estimate joint associations and population-attributable fractions of these metrics in relation to all-cause and cardiovascular disease (CVD) mortality risk. Study of a nationally representative sample of 44,959 US adults (≥20 years), using data from the National Health and Nutrition Examination Survey (NHANES) 1988-1994, 1999-2004, and 2005-2010 and the NHANES III Linked Mortality File (through 2006). All-cause, CVD, and ischemic heart disease (IHD) mortality. Few participants met all 7 cardiovascular health metrics (2.0% [95% CI, 1.5%-2.5%] in 1988-1994, 1.2% [95% CI, 0.8%-1.9%] in 2005-2010). Among NHANES III participants, 2673 all-cause, 1085 CVD, and 576 IHD deaths occurred (median follow-up, 14.5 years). Among participants who met 1 or fewer cardiovascular health metrics, age- and sex-standardized absolute risks were 14.8 (95% CI, 13.2-16.5) deaths per 1000 person-years for all-cause mortality, 6.5 (95% CI, 5.5-7.6) for CVD mortality, and 3.7 (95% CI, 2.8-4.5) for IHD mortality. Among those who met 6 or more metrics, corresponding risks were 5.4 (95% CI, 3.6-7.3) for all-cause mortality, 1.5 (95% CI, 0.5-2.5) for CVD mortality, and 1.1 (95% CI, 0.7-2.0) for IHD mortality. Adjusted hazard ratios were 0.49 (95% CI, 0.33-0.74) for all-cause mortality, 0.24 (95% CI, 0.13-0.47) for CVD mortality, and 0.30 (95% CI, 0.13-0.68) for IHD mortality, comparing participants who met 6 or more vs 1 or fewer cardiovascular health metrics. Adjusted population-attributable fractions were 59% (95% CI, 33%-76%) for all-cause mortality, 64% (95% CI, 28%-84%) for CVD mortality, and 63% (95% CI, 5%-89%) for IHD mortality. Meeting a greater number of cardiovascular health metrics was associated with a lower risk of total and CVD mortality, but the prevalence of meeting all 7 cardiovascular health metrics was low in the study population.

            Physical activity and obesity in children.

            Globally, obesity is affecting an increasing proportion of children. Physical activity plays an important role in the prevention of becoming overweight and obese in childhood and adolescence, and reducing the risk of obesity in adulthood. Puberty and the following adolescent period are acknowledged as particularly vulnerable times for the development of obesity due to sexual maturation and, in many individuals, a concomitant reduction in physical activity. In many Western settings, a large proportion of children and adolescents do not meet recommended physical activity guidelines and, typically, those who are more physically active have lower levels of body fat than those who are less active. Active behaviours have been displaced by more sedentary pursuits which have contributed to reductions in physical activity energy expenditure. Without appropriate activity engagement there is an increased likelihood that children will live less healthy lives than their parents. Owing to the high risk of overweight adolescents becoming obese adults, the engagement of children and adolescents in physical activity and sport is a fundamental goal of obesity prevention.

              Light-Intensity Physical Activity and Cardiometabolic Biomarkers in US Adolescents

              Background The minimal physical activity intensity that would confer health benefits among adolescents is unknown. The purpose of this study was to examine the associations of accelerometer-derived light-intensity (split into low and high) physical activity, and moderate- to vigorous-intensity physical activity with cardiometabolic biomarkers in a large population-based sample. Methods The study is based on 1,731 adolescents, aged 12–19 years from the 2003/04 and 2005/06 National Health and Nutrition Examination Survey. Low light-intensity activity (100–799 counts/min), high light-intensity activity (800 counts/min to <4 METs) and moderate- to vigorous-intensity activity (≥4 METs, Freedson age-specific equation) were accelerometer-derived. Cardiometabolic biomarkers, including waist circumference, systolic blood pressure, diastolic blood pressure, HDL-cholesterol, and C-reactive protein were measured. Triglycerides, LDL- cholesterol, insulin, glucose, and homeostatic model assessments of β-cell function (HOMA-%B) and insulin sensitivity (HOMA-%S) were also measured in a fasting sub-sample (n = 807). Results Adjusted for confounders, each additional hour/day of low light-intensity activity was associated with 0.59 (95% CI: 1.18–0.01) mmHG lower diastolic blood pressure. Each additional hour/day of high light-intensity activity was associated with 1.67 (2.94–0.39) mmHG lower diastolic blood pressure and 0.04 (0.001–0.07) mmol/L higher HDL-cholesterol. Each additional hour/day of moderate- to vigorous-intensity activity was associated with 3.54 (5.73–1.35) mmHG lower systolic blood pressure, 5.49 (1.11–9.77)% lower waist circumference, 25.87 (6.08–49.34)% lower insulin, and 16.18 (4.92–28.53)% higher HOMA-%S. Conclusions Time spent in low light-intensity physical activity and high light-intensity physical activity had some favorable associations with biomarkers. Consistent with current physical activity recommendations for adolescents, moderate- to vigorous-intensity activity had favorable associations with many cardiometabolic biomarkers. While increasing MVPA should still be a public health priority, further studies are needed to identify dose-response relationships for light-intensity activity thresholds to inform future recommendations and interventions for adolescents.
