          Abstract

          Resumo Introdução Péssimas condições de saúde bucal vêm sendo apontadas como prejudiciais às pessoas acometidas pela hanseníase, pois contribuem para a piora do quadro clínico desses indivíduos. Objetivo Este trabalho objetiva avaliar as condições de saúde bucal dos indivíduos atendidos por um serviço especializado no município de Sobral, no Ceará, e comparar dois grupos (com e sem reação hansênica) quanto às condições orais. Método Trata-se de um estudo transversal realizado no Ambulatório de Hanseníase do município de Sobral, no Ceará, com 56 pacientes em tratamento. Foram realizados um levantamento epidemiológico em saúde bucal e uma entrevista que coletaram informações sobre condições de saúde bucal, dados socioeconômicos, utilização dos serviços odontológicos e percepção entre doença e saúde bucal. Para a análise estatística entre os grupos, foi realizada uma análise bivariada, seguida por uma análise multivariada. Resultados Houve predomínio de sexo masculino, baixa escolaridade e diagnóstico multibacilar. Apesar da relação estatisticamente significante entre reação hansênica e cárie dentária, necessidade de exodontia e/ou endodontia, sangramento gengival, cálculo dentário e bolsa periodontal, apenas esta última apresentou associação significativa (p = 0,019) na análise multivariada. Conclusão Os participantes deste estudo apresentaram precárias condições de saúde bucal, sendo piores naqueles com reações hansênicas.

          Abstract Background Poor oral health conditions are being considered as harmful for people affected by Hansen’s disease, since they contribute to worsening the clinical condition of these individuals. Objective This study aims to evaluate the oral health conditions of individuals attended by a specialized service in the city of Sobral, Ceará, and to compare two groups (with and without Hansen’s disease reaction) regarding oral conditions. Method A cross-sectional study was carried out at the Ambulatório de Hanseníase in the city of Sobral, Ceará, Brazil, with 56 patients being treated. An epidemiological survey was conducted on oral health and an interview that collected information on oral health conditions, socioeconomic data, use of dental services and perception between the disease and oral health. For the statistical analysis between the groups, a bivariate analysis was performed, followed by a multivariate analysis. Results Predominance was the male gender, low level of schooling and multibacillary diagnosis. Despite the statistically significant relationship between presence of Hansen’s disease reaction and the presence of dental caries, need for exodontia and / or endodontics, gingival bleeding, dental calculus and periodontal pocket, only the latter had a significant association (p = 0.019) in the multivariate analysis. Conclusion The participants of this study presented poor oral health conditions, being worse in those who presented Hansen’s disease reactions.

          Dental and oral condition in leprosy patients from Serra, Brazil.

          To describe dental and periodontal diseases and oral lesions in newly diagnosed leprosy patients. Cohort study with 99 leprosy patients carried out at the Leprosy Control Programme Outpatient Clinic, Serra-ES, Brazil. A questionnaire about demographic and clinical data was used. Clinical oral examination was performed through the decayed, missing and filled teeth index (DMFT index), the use and need of prosthesis, periodontal disease and the presence of mucous membrane oral lesions. Skin and oral mucous biopsies were also undertaken. Decayed teeth were present in 73% of the patients, at least one lost tooth was present in 71.4%, the mean of the number of lost teeth among the patients in this survey was 88; and 603% of the patients did not have their teeth filled. Periodontal disease was present in 80.8%, and gingival bleeding in 92% of the patients. DMFT index average was 14.4. Nine out of the 63 patients presented with oral clinical lesions, however, most of them presented with unspecific chronic inflammation and typical epithelial hyperplasia. These newly diagnosed leprosy patients were similar in respect of oral health to the normal Brazilian population. Serious dental loss and edentulism were observed, as were a high DMFT index and frequency of periodontal diseases. These data highlight a lack of oral health prevention and treatment and poor access even when available.

            Oro-facial manifestations of 100 leprosy patients

            Objectives: To verify the frequency of oral and facial involvement in diagnosed leprosy patients. Study design: This study was performed on 100 leprosy patients (62 male, 38 female, mean ages 51.86±6.1). After explaining the study design, we studied descriptive information including: patient’s sex, age, job, place of birth, familial history of leprosy, types of disease (lepromatous, borderline and tuberculoid leprosy), ocular and oral lesions, facial involvement and neuropathy. The statistical signification was measured by chi-square test. Results: A total of 46 (23 lepromatous, 15 borderline, and 8 tuberculoid leproy) out of 100 patients with leprosy had oral lesions. Statistical analysis did not show any significant difference in frequency of oral lesions between different types of disease. Facial lesions were presented in 57 (39 lepromatous, 10 borderline, and 8 tuberculoid leprosy) patients. There was a statistical significant difference in frequency of facial manifestations between different types of leprosy. It has to be mentioned that, atrophy of nasal spine, facial nerve involvement, ocular lesions and facial deformity were seen in 15%, 17%, 22% and 44% of leprosy patients, respectively. Conclusion: Examination of leprosy patients should be extended to the oral mucosa because oral mucosa may be a secondary source of M.Leprae transmission and infection.               Clinical variables associated with leprosy reactions and persistence of physical impairment

              Introduction Leprosy is a chronic disease that affects skin and peripheral nerves. Disease complications include reactional episodes and physical impairment. One World Health Organization (WHO) goal of leprosy programs is to decrease the number of grade 2 impairment diagnoses by 2015. This study aims to evaluate clinical factors associated with the occurrence of leprosy reactions and physical impairment in leprosy patients. Methods We conducted a retrospective study of data from medical records of patients followed in two important centers for the treatment of leprosy in Aracaju, Sergipe, Brazil, from 2005 to 2011. We used the chi-square test to analyze associations between the following categorical variables: gender, age, operational classification, clinical forms, leprosy reactions, corticosteroid treatment, and physical impairment at the diagnosis and after cure. Clinical variables associated with multibacillary leprosy and/or reactional episodes and the presence of any grade of physical impairment after cure were evaluated using the logistic regression model. Results We found that men were more affected by multibacillary forms, reactional episodes, and grade 2 physical impairment at diagnosis. Leprosy reactions were detected in a total of 40% of patients and all were treated with corticosteroids. However, physical impairment was observed in 29.8% of the patients analyzed at the end of the treatment and our multivariate analysis associated a low dose and short period of corticosteroid treatment with persistence of physical impairments. Conclusions Physical impairment should receive an increased attention before and after treatment, and adequate treatment should be emphasized.
