          Abstract

          Fundamento: la gonartrosis del compartimento medial de la rodilla asociada a desviación en varo requiere de la combinación de procedimientos quirúrgicos para su tratamiento. Una de las variantes más utilizadas es la artroscopia y osteotomías en un mismo tiempo quirúrgico. Objetivo: mostrar la combinación de la artroscopia con la osteotomía tibial alta abierta en el tratamiento de la gonartrosis. Caso clínico: paciente de 56 años de edad, blanco, masculino, ingresado en el servicio de Ortopedia y Traumatología del Hospital Universitario Manuel Ascunce Domenech de la ciudad de Camagüey, por presentar gonartrosis izquierda con predominio del compartimento tibiofemoral medial. En el mismo tiempo quirúrgico se realizó artroscopia mediante la cual se llevó a cabo meniscectomía parcial y microfracturas del cóndilo medial, luego se realizó osteotomía tibial alta abierta. Conclusiones: la combinación de artroscopia y osteotomía permite actuar simultáneamente en pacientes con gonartrosis, al tratar las lesiones articulares y corregir las deformidades angulares que presentan. Las dos técnicas pueden ser realizadas en el mismo tiempo quirúrgico sin prolongar su duración.

          Translated abstract

          Background: Knee osteoarthritis of the medial compartment is usually associated to varus deformity, and the patients will need a combination of different surgical procedures for treatment. One of the most common combinations in the same surgical time is: arthroscopy and osteotomy. Objective: to show the combination of arthroscopy and high open wedge medial osteotomy of the tibia in patients with the diagnosis of osteoarthritis of the knee. Case report: A 56 year old male, white patient was admitted to the department of Orthopedics and Traumatology at Manuel Ascunce provincial teaching hospital in Camaguey city, because of osteoarthritis of the left knee affecting predominantly the medial tibiofemoral compartment. At the same surgical time, an arthroscopy was carried out to perform a partial meniscectomy and microfracture of the medial condyle, later a high open wedge medial osteotomy of the knee was performed. Conclusions: the combination of arthroscopy and osteotomy, let to treat lesion inside the joint and correct angular deformities of the limb at the same time. These techniques can be done together without a long surgical time.

            Outcome after high tibial open-wedge osteotomy: a retrospective evaluation of 533 patients.

            Open-wedge valgus high tibial osteotomy is a well-established procedure in the management of medial osteoarthritis of the knee. In recent years, improved osteotomy and fixation methods have led to an increased use of this technique. The aim of this study was to identify predictive parameters for the clinical outcome after valgus high tibial osteotomy.

              Open-wedge osteotomy using an internal plate fixator in patients with medial-compartment gonarthritis and varus malalignment: 3-year results with regard to preoperative arthroscopic and radiographic findings.

              Our purpose was to evaluate the 3-year clinical results of patients with medial-compartment osteoarthritis of the knee and varus malalignment who underwent open-wedge high tibial osteotomy (HTO) with an internal plate fixator (TomoFix; Synthes, Solothurn, Switzerland). Clinical results are correlated with arthroscopic and radiographic findings at the time of surgery. This study included 69 patients with a minimum follow-up of 36 months who underwent open-wedge HTO for medial-compartment osteoarthritis of the knee. Knee function was assessed before surgery and at 6, 12, 24, and 36 months after HTO by use of subjective International Knee Documentation Committee and Lysholm scores. Arthroscopic findings before HTO and radiographic assessment of the metaphyseal deformity of the proximal tibia (tibial bone varus angle) were correlated with clinical outcome. A significant continuous increase in International Knee Documentation Committee score from 47.25 ± 18.71 points before surgery to 72.72 ± 17.15 points at 36 months after HTO was found (P .05 at all time points). The tibial bone varus angle was correlated significantly with greater improvement and better clinical outcome after HTO (P < .01). The overall complication rate of 8.6% was mostly related to surgical causes; nevertheless, a high proportion of patients reported discomfort related to the implant at some point during the follow-up period (40.6%). Open-wedge osteotomy by use of the TomoFix system leads to reliable 3-year results. Results do not depend on the severity of medial cartilage defects, whereas partial-thickness defects of the lateral compartment seem to be well tolerated. The prognostic relevance of patellofemoral cartilage defects remains unclear. Local irritation of the implant was observed in a significant number of patients. Level IV, therapeutic case series.
