      Efficacy of olanzapine in symptom relief and quality of life in gastric cancer patients receiving chemotherapy

          Abstract

          Background:

          Considering the incidence and prevalence rates of gastric cancer in Mazandaran Province of Iran, this research was performed to evaluate the efficacy and safety of olanzapine in symptom relief and quality of life (QOL) improvement of gastric patients receiving chemotherapy.

          Materials and Methods:

          This clinical trial was conducted on thirty new cases of gastric cancer patients whose treatment protocol was planned on chemotherapy and were allocated into two groups by simple random sampling. Intervention group (15 patients) received olanzapine tablets (2.5–10 mg/day) a day before the beginning of chemotherapy; in the 1 st day of chemotherapy to 8 weeks after chemotherapy, besides the routine treatment regimens. The control group received only the routine treatment regimens. The patients were followed for 8 weeks after intervention. All of the patients were assessed with Hospital Anxiety and Depression Scale (HADS) and WHO-QOL-BREF questionnaires; further, Rhodes index was used to evaluate nausea and vomiting (N/V) status.

          Results:

          All the recruited patients continued the allocated interventions (no lost to follow-up). N/V decreased in the case group, but the difference was not statistically significant ( P = 0.438). The patients' appetite and body mass index increased ( P = 0.006). Anxiety and depression subscales of HADS had significant differences between the two groups ( P < 0.001) in the 4 th and 8 th week after treatment. Among the different subdomains of QOL, only physical health improved significantly after intervention ( P < 0.05), but no significant difference was observed in other subdomains and also total QOL score ( P > 0.05). No significant increase was observed in fasting and 2-h postprandial blood glucose and lipid profile ( P > 0.05).

          Conclusion:

          Olanzapine can be considered as an effective drug to increase appetite and decrease anxiety and depression in patients with gastric cancer.

          The Hospital Anxiety and Depression Scale (HADS): translation and validation study of the Iranian version

          Background The Hospital Anxiety and Depression Scale (HADS) is a widely used instrument to measure psychological morbidity in cancer patients. This study aimed to translate and test the reliability and validity of the Iranian version of the HADS. Methods The English language version of the HADS was translated into Persian (Iranian language) and was used in this study. The questionnaire was administered to a consecutive sample of 167 breast cancer patients and statistical analysis was performed to test the reliability and validity of the HADS. Results In general the Iranian version of the HADS was found to be acceptable to almost all patients (99%). Cronbach's alpha coefficient (to test reliability) has been found to be 0.78 for the HADS anxiety sub-scale and 0.86 for the HADS depression sub-scale. Validity as performed using known groups comparison analysis showed satisfactory results. Both anxiety and depression sub-scales discriminated well between sub-groups of patients differing in clinical status as defined by their disease stage. Conclusion This preliminary validation study of the Iranian version of the HADS proved that it is an acceptable, a reliable and valid measure of psychological distress among cancer patients.

            Sample size estimation in epidemiologic studies.

            This review basically provided a conceptual framework for sample size calculation in epidemiologic studies with various designs and outcomes. The formula requirement of sample size was drawn based on statistical principles for both descriptive and comparative studies. The required sample size was estimated and presented graphically with different effect sizes and power of statistical test at 95% confidence level. This would help the clinicians to decide and ascertain a suitable sample size in research protocol in order to detect an effect of interest.

              Gastric cancer in Iran: epidemiology and risk factors.

              Although the global incidence of gastric cancer has been decreased dramatically in recent decades, it is the most common cancer in north and northwest Iran. The wide variation in incidence across different geographical areas and higher proportion of cardia cancer are two main characteristics of gastric cancer in Iran. Current investigations indicate that a high prevalence of H.pylori infection, high dietary intake of salt and smoking are the main environmental factors of gastric cancer in Iran. Gastroesophageal reflux disease is another contributing factor in populations with a higher incidence of gastric cardia cancer. While interventions on modifiable environmental risk factors should be considered as the main modality to reduce gastric cancer development, surveillance programmes for early detection of cancer in highly selected groups may increase overall survival rates in potential patients in this country.
