      Therapieziele und ihr Zusammenhang mit frühem Therapiefortschritt im ambulanten kognitiv-verhaltenstherapeutischen Setting

          Abstract

          Zusammenfassung. Hintergrund. Der Zusammenhang zwischen Inhalt und Ausrichtungsfokus von Therapiezielen und erfolgreichem Therapieverlauf ist wenig erforscht, wobei insbesondere eine mögliche Assoziation von Inhalt und Fokus vernachlässigt wurde. Fragestellung. Untersucht wurde die Assoziation von Therapiezielinhalt und -fokus und deren Zusammenhang mit frühem Therapiefortschritt. Methode. 716 Therapieziele von 163 Patientinnen und Patienten in ambulanter KVT wurden nach Inhalt und Fokus (Annäherung vs. Vermeidung) kategorisiert und der Zusammenhang von Zielart und Zielerreichung sowie Symptomreduktion nach 12 Sitzungen analysiert. Ergebnisse. Zielinhalt und –fokus waren assoziiert. Die fremdbeurteilte Zielerreichung war bei problembezogenen Annäherungszielen am höchsten. Die Symptomreduktion war dagegen am höchsten, wenn problembezogene Vermeidungsziele vorlagen. Schlussfolgerung. Therapiefortschritt steht im Zusammenhang mit den gesetzten Therapiezielen. Welche Therapiezielart, insbesondere welcher Fokus Therapiefortschritt und -erfolg begünstigt, bleibt weiterer zu erforschen. Dabei sollten zukünftig Zielinhalt und –fokus gemeinsam untersucht werden.

          Treatment Goals and Early Treatment Progress in Cognitive Behavioral Therapy

          Abstract. Background: The relationship between the therapy progress and the themes and focus (approach vs. avoidance) of treatment goals has been studied only rarely. In particular, the possible association of the themes and the focus has been neglected. Objective: This article investigates the association of themes and focus of treatment goals with early therapy progress. Methods: 716 treatment goals of 163 patients in outpatient CBT were categorized according to their content and focus (approach vs. avoidance). The association of the goal type with goal achievement as well as with symptom reduction after 12 sessions was analyzed. Results: Goal themes and focus were associated with problem-related goals having an avoidance focus more often than nonproblem-related goals. Goal achievement was highest for problem-related approach goals. In contrast, symptom reduction was highest in the presence of problem-related goals with an avoidance focus. Conclusions: Therapy progress is related to the set goals. Further research considering both the themes and the focus of therapy goals is needed to identify which type of treatment goals, particularly which focus, relates to therapy progress and success.

              Psychometric analysis of the brief symptom inventory 18 (BSI-18) in a representative German sample

              Background The BSI-18 contains the three six-item scales somatization, depression, and anxiety as well as the Global Severity Index (GSI), including all 18 items. The BSI-18 is the latest and shortest of the multidimensional versions of the Symptom-Checklist 90-R, but its psychometric properties have not been sufficiently clarified yet. Methods Based on a representative sample of N = 2516 participants (aged 14–94 years), detailed psychometric analyses were carried out. Results The internal consistency was good: Somatization α = .82, Depression α = .87, Anxiety α = .84 and GSI α = .93. Confirmatory factor analysis supported the three scales as second-order and GSI as first-order factors. The model fit based on RMSEA is good but that model fit based on CFI and TLI are too low. Conclusions Therefore, it is a very short, reliable instrument for the assessment of psychological distress. The BSI-18 can be used to reliably assess psychological distress in the general population. However, further studies need to evaluate the usefulness of standardization in clinical samples.
