          Abstract

          ABSTRACT Objectives To develop an application program for prevention and treatment of complications of intestinal peristomal skin. Method The framework of the application software development were: Phase 1 - “Design, identification of the needs for the application development”: In this phase, the authors identified during their clinical practice that some professionals and caregivers find it difficult to provide care for at-risk ostomized patients or those with some kind of peristomal skin complication; Phase 2 - “Application prototype development”: this phase included the integrative literature review in the main databases; Phase 3 - “Creating the application”: this phase was intended to generate a decision tree algorithms, structure the database, and develop the software; Phase 4 - “Transition”: performing the application functionality testing. Results The application “Dermatite Periestoma App” has 36 screens and 21 figures describing the procedures for prevention and treatment of complications of intestinal peristomal skin. It is freely available from the Play Store and has been registered with the Instituto Nacional da Propriedade Industrial Ministério do Desenvolvimento, indústria e Comércio Exterior (Brazilian Ministry of Development, Industry and Foreign Trade, National Institute of Industrial Property). Conclusion This study made it possible to describe the stages of planning and development of the mobile application “Dermatite Periestoma App”. The steps taken indicate that the “Dermatite Periestoma App” has great potential for clinical practice in the evaluation of patients with stomal complications or at risk for developing peristomal skin complications, preventive measures, therapeutic approaches, and for nursing education through the use of technology.

          RESUMO Objetivos Desenvolver um aplicativo para prevenção e tratamento de complicações da pele periestoma intestinal. Método As fases de desenvolvimento da estrutura do aplicativo foram: Fase 1 ‒ “Concepção, identificação das necessidades do desenvolvimento do aplicativo”: nessa fase os autores identificaram, durante sua prática clínica, que alguns profissionais e cuidadores têm dificuldade em prestar assistência aos ostomizados com risco ou que adquirirem algum tipo de complicação da pele periestoma; Fase 2 ‒ “Elaboração do protótipo do aplicativo”: essa fase contemplou a revisão integrativa da literatura nas principais bases de dados; Fase 3 ‒ “Construção do aplicativo”: essa fase consistiu na elaboração da árvore de decisão dos algoritmos, estruturação do banco de dados e desenvolvimento do software; Fase 4 ‒ “Transição”: foram realizados os testes de funcionalidade do aplicativo. Resultados O aplicativo “Dermatite Periestoma App” tem 36 telas e 21 figuras descrevendo os procedimentos para prevenção e tratamento das complicações da pele periestoma intestinal. O mesmo está disponível gratuitamente naPlay Store e foi registrado no Instituto Nacional da Propriedade Industrial Ministério do Desenvolvimento, indústria e Comércio Exterior. Conclusão Este estudo possibilitou descrever as etapas do planejamento e desenvolvimento do aplicativo móvel “Dermatite Periestoma App”. As etapas percorridas indicam que o “Dermatite Periestoma App” tem grande utilidade potencial para a prática clínica na avaliação do ostomizado com complicação ou que apresenta risco para desenvolver complicações da pele periestoma, medidas preventivas, condutas terapêuticas, e para o ensino de Enfermagem por meio do uso de tecnologia.

          Association of sociodemographic and clinical factors with the self-image and self-esteem of individuals with intestinal stoma

          Objective: To evaluate the clinical and sociodemographic factors and correlate them to the self-image and self-esteem. Methods: A study conducted at Ostomy Pole of PousoAlegre. The data were collected between December 2012 and May 2013. This was a non-probabilistic, by convenience, sample. For data collection, the Rosenberg Self-Esteem Scale/UNIFESP-EPM and Body Investment Scale were used. For statistical analysis, chi-squared test, Kruskal-Wallis test and Spearman correlation were used. Levels of significance of 5% (p ≤ 0.05) were considered. Results: Participants had a mean score of 10.81 in the Rosenberg Self-Esteem Scale/UNIFESP. Regarding Body Investment Scale results, the mean total score was 38.79; the mean in the domain of body image was 7.74, and for personal touch, 21.31. When comparing data related to the stoma and sociodemographic profiles with the Rosenberg Self-Esteem Scale/UNIFESP and Body Investment Scale, we realize that all patients demonstrated a decrease in self-esteem and self-image. Individuals were over 60 years old, male, retired, married and were not participants in support group/association. With regard to the characteristics of the stoma, those permanent colostomized had as causes of stoma implementation inflammatory disease and neoplasia; stomata measured between 20 and 40 mm; these people used two-piece devices. People who have not been notified that would be subjected to the stoma and in whom no demarcation was done showed worsening in self-esteem and self-image in relation to other features related to injury and sociodemographic data. Conclusion: Patients who participated in this study had low self-image and self-esteem in all characteristics of the stoma and in sociodemographic data, meaning that these individuals had negative feelings about their own bodies.

            Development and validation of an algorithm for laser application in wound treatment 1

            ABSTRACT Objective: To develop and validate an algorithm for laser wound therapy. Method: Methodological study and literature review. For the development of the algorithm, a review was performed in the Health Sciences databases of the past ten years. The algorithm evaluation was performed by 24 participants, nurses, physiotherapists, and physicians. For data analysis, the Cronbach’s alpha coefficient and the chi-square test for independence was used. The level of significance of the statistical test was established at 5% (p<0.05). Results: The professionals’ responses regarding the facility to read the algorithm indicated: 41.70%, great; 41.70%, good; 16.70%, regular. With regard the algorithm being sufficient for supporting decisions related to wound evaluation and wound cleaning, 87.5% said yes to both questions. Regarding the participants’ opinion that the algorithm contained enough information to support their decision regarding the choice of laser parameters, 91.7% said yes. The questionnaire presented reliability using the Cronbach’s alpha coefficient test (α = 0.962). Conclusion: The developed and validated algorithm showed reliability for evaluation, wound cleaning, and use of laser therapy in wounds.

              Ostomia, uma difícil adaptação

              Este estudo objetiva identificar alterações causadas pela confecção de uma ostomia, em uma visão geral sobre o processo de viver do paciente ostomizado. Após a cirurgia, há um período de adaptação, mudanças de sua imagem corporal e de alterações complexas e limitadoras que podem levar o paciente a necessitar de adaptação para o enfrentamento e superação. Estas mudanças podem proporcionar o aparecimento de sentimentos negativos como a mutilação, invalidez, incapacidade, raiva, depressão, entre outros, que podem gerar uma série de obstáculos na reintegração e ajustamento à vida familiar, social e laborativa. Cada paciente, na sua singularidade, possui crenças e valores específicos, que determinam diferentes tomadas de enfrentamento às doenças e suas seqüelas, sendo assim, a adaptação requer um tempo individual. A necessidade do estoma reflete-se no aspecto físico, psicológico, e social, em uma ótica multidisciplinar e na busca de qualidade de vida, necessitando de suporte social principalmente através da família, que o ajudará na reabilitação de seu convívio social e na recuperação da capacidade produtiva do paciente ostomizado.
