          Abstract

          Primary signet-ring cell adenocarcinoma of the urinary bladder is a rare tumor. We report in this study the case of a 53 year old man consulting for gross hematuria. Computed tomography imaging demonstrated right hydronephrosis and an invasive bladder tumor. The bladder biopsy showed a signet-ring cell carcinoma; the exploration of the gastrointestinal tract did nor reveal any other tumor localization. A total cystoprostatectomy was performed followed by adjuvant chemotherapy with cisplatin and gemcitabine. The aim of this study is to determine the anatomoclinical, therapeutic and evolutionary characteristics of this rare tumor.

          El adenocarcinoma de células en anillo de sello primario de vejiga es un tumor raro. Describimos un caso de un varón de 53 años que consultó por hematuria macroscópica. La imagen de la tomografía axial computerizada mostró un hidronefrosis derecha y un tumor invasivo de la vejiga urinaria. La biopsia mostró un carcinoma de células en anillo de sello. No se detectó localización primaria digestiva en la exploración del tracto grastrointestinal. El paciente se trató con cistoprostatectomía total y quimioterapia adyuvante con cisplatino y gemcitabina. El objetivo de este caso es presentar las características anatomoclínicas, tratamiento y evolución de este infrecuente tumor.

            Primary adenocarcinoma of the urinary bladder: a report of 185 cases.

            To evaluate the outcome after the treatment of primary non-urachal vesical adenocarcinoma and to determine the significant prognostic factors. The records of 185 patients with vesical adenocarcinoma were reviewed. The pathological evaluation included the determination of pathological stage, tumour grade, presence or absence of mucin and its location, evidence of bilharzial infestation and flow-cytometric DNA analysis. The mean follow-up of the treated patients was 3.1 years. Disease-free survival was estimated and the results correlated with patient and tumour characteristics (univariate analysis). Cox's proportional hazards analysis was used to determine prognostic factors. The overall 5-year disease-free survival was 55%; only three factors had a significant impact on survival, the tumour pathological stage and grade, and lymph node involvement. Radical cystectomy remains the only satisfactory treatment option for primary vesical adenocarcinoma. Tumour stage, grade and lymph node involvement are the only significant prognostic factors.

              Signet-ring cell adenocarcinoma of bladder.

              Three cases of signet-ring cell adenocarcinoma of the bladder are presented: a primary vesical adenocarcinoma, a metastatic one, and a signet-ring cell adenocarcinoma of the urachus. A review of the literature found 69 additional reports of primary signet-ring cell adenocarcinoma, 20 of urachal signet-ring carcinoma, and 4 of the metastatic variety. The pathogenesis, clinical, and histologic findings, prognosis, and management are discussed.
