      The epidemiology and medical care costs of Echinococcus granulosusis in Jahrom, southern Iran from 2007 to 2017

          ABSTRACT

          Background: Echinococcus granulosus is a rare parasitic infection causing Cystic Echinococcosis, which can be dangerous due to involving the body. This parasitic infection is a significant health problem in Iran. However, little is known about this disease, specifically in Jahrom city; thus, we aimed to investigate the epidemiology and the economic impact of the illness.

          Methods: In this descriptive cross-sectional study, the files of 137 patients who were under the care, and treatment of the final diagnosis of Cystic Echinococcosis were evaluated by reviewing the information such as age, gender, occupation, place of residence was collected, and analyzed.

          Results: Human cystic echinococcosis cases were more common in females, 57.2% (12 patients) and 42.8% (9 patients) were male. In terms of age, most patients (23.8%) were in the age range from 21 to 30 years. The chief complaint at diagnosis, in all cases, was abdominal pain. Besides, 71.42% of the cases had the liver involvement alone, 9.52% had the lung involvement alone, 9.52% had a co-infection of liver and lung, and 4.74% had the kidney involvement alone.

          Conclusions: The results of the present study are beneficial in determining the disease status and the epidemiology of hydatid cyst in this area.

            Epidemiology and Clinical Features of Hydatid Cyst in Northern Iran from 2005 to 2015

            Background: Human hydatid disease imposes significant impacts on public health by producing substantial morbidity and mortality in involved communities. This study aimed to evaluate the epidemiology and clinical features of hydatid cyst in northern Iran as a breeding center for the infection. Methods: In this cross-sectional study, the hospital records of all hydatidosis-affected patients admitted in three teaching hospitals of Mazandaran Province between Mar 2005–2015 were reviewed. Hydatidosis-relevant demographic characteristics, clinical findings, and laboratory data were collected. The descriptive statistical analysis was performed by SPSS software. Results: Totally, 79 patients with the mean age of 42.00±23.82 yr were admitted with cystic echinococcosis (CE) diagnosis. Moreover, the highest and the lowest prevalence of CE cases were in age ranges of 50–59 (19.0%) and more than 80 (5.0%) yr, respectively. Male/female ratio was 0.88 (47.0% vs. 53.0%). Majority of the cases were urban residents (54.0%) and had no close contact with animals (58.0%). Nearly, two third of the patients (n=54), the affected organ was liver. The diameter of the cysts was variable from 2 to 15 cm. Most of the patients had a single hydatid cyst. Four patients were diagnosed as secondary hydatid cyst. Medical treatment with antiparasitic agents was done for 47 individuals and in 7 cases; it was the only treatment approach. Percutaneous puncture-aspiration-injection reinjection (PAIR) technique was applied for 6 cases. Sixty-six patients underwent radical surgery. No data was available on eosinophil count or serological tests. Conclusion: CE is approximately prevalent in Iranian population. Development of new diagnostic methods and therapeutic procedures is worthy. Moreover, it is necessary to design and develop a registry and surveillance system by a multidisciplinary team.

              Epidemiological and clinical aspects of patients with hydatid cyst in Iran

              Hydatid cyst is one of the most common Zoonotic diseases occurs due to infection with larval stage of Echinococcus granulosus parasite. Based on epidemiological reasons, World Health Organization has introduced Iran as a hyperendemic region. This study aims to investigate the clinico-epidemiological characteristics of patients with hydatid cyst in Iran. We used information registered in the surveillance system of the Iranian Ministry of Health and Medical Education attributed to Mazandaran province. Data were entered into SPSS software V.20 after refinement and analyzed using percent frequency, mean, standard deviation and statistical tests such as Chi-square and Exact Fisher tests. Mean (SD), minimum and maximum ages of the participants were 42.2 (17.2), 6 and 72 years respectively. Most of them were female (63.4 %) and mean (SD) number of cysts was 1.6 ± 0.8. Of them, 51.2 % living in rural areas and 58.5 % of them were housewives. Liver was the most common organ involved. The main diagnostic procedures were radiography and CT scanning. The majority of participants (73.2 %) consumed unsterilized vegetables. This study showed women more susceptible compared to men probably because of more exposure to parasites through vegetables. We also found the liver as the most affected organ in hydatid cyst patients.
