          Abstract

          Introducción. La caries es una enfermedad multifactorial considerada un problema de salud pública -ya que, a escala mundial, 95% de los niños la padecen- que está íntimamente asociada a la calidad de la higiene oral. El objetivo de este trabajo fue determinar la experiencia promedio de caries dental en un grupo de escolares del municipio de Yautepec, Morelos, y su relación con la higiene oral. Métodos. Se llevó a cabo un estudio epidemiológico de carácter descriptivo, transversal y prospectivo en 771 escolares con dentición mixta del municipio de Yautepec, estado de Morelos. Para medir la experiencia promedio de caries dental y la calidad de la higiene oral se llevó a cabo el registro de los índices CPOD, ceod e IHOS de acuerdo con los criterios establecidos por la Organización Mundial de la Salud (Cr =0.03, Ca =0.89, k =0.942). Resultados. La experiencia promedio de caries dental tuvo una tendencia de aproximadamente dos dientes primarios con alguna experiencia de caries, y en dentición secundaria el promedio fue de casi cero. En ambos casos, las diferencias resultaron estadísticamente significativas ( p <0.0001). Asimismo, se observó que la enfermedad no tuvo relación con el sexo ( p >0.05), pero sí con la calidad de la higiene oral ( p =0.0001). Conclusiones. La experiencia promedio de caries dental en ambas denticiones fue baja. La condición más frecuentemente observada en cuanto a la calidad de la higiene oral fue buena. La experiencia promedio de caries mostró estar asociada con la edad y con la calidad de la higiene oral.

          Translated abstract

          Background. Caries is a multifactorial disease and is considered a public health problem because 95% of children worldwide suffer from it. It is intimately associated with oral hygiene quality. The objective of this work was to determine the relationship between the experience of dental caries and oral hygiene in school-age children living in Yautepec, Morelos, Mexico. Methods. A cross-sectional study of 771 school-age children with mixed dentition in Yautepec, Morelos, Mexico was undertaken. An oral exam was used to collect data on oral hygiene and dental caries in primary and permanent teeth. This evaluation was developed by two dentists using standardized DMFT/dmft index to determine dental caries according to the criteria established by the WHO (Cr =0.03, Ca =0.89, k =0.942). Results. The average experience of dental caries was a trend of approximately two primary teeth with some experience of caries and in secondary dentition the average was almost zero. In both cases these differences were found to be statistically significant ( p <0.0001). It was also noted that the disease had no relationship with gender ( p >0.05) but was related with the quality of oral hygiene ( p =0.0001). Conclusions. The average experience of dental caries in both dentitions was low. The condition most frequently observed in terms of quality of oral hygiene was good. The average experience of dental caries was shown to be associated with age and quality of oral hygiene.

          The caries decline: a review of reviews.

          The aim of this paper is to review publications discussing the declining prevalence of dental caries in the industrialized countries during the past decades, focusing on some main conferences addressing this issue. Has there been a real decline in the prevalence of dental caries? Several excellent papers and reviews have been published, and there is a general agreement that a marked reduction in caries prevalence has occurred among children in most of the developed countries in recent decades. This fact has stimulated much debate and attempts to identify the most likely explanations for this change. The conclusion made from this review is that the various authors believe that the use of fluoride in various forms has contributed most significantly to the decline in dental caries prevalence. A number of other factors must, however, also be taken into consideration, and such factors have been extensively discussed in the various publications.

            Factors associated with prevalence and severity of caries experience in preschool children.

            The aim of the present study was to examine the prevalence and severity of caries experience in the primary dentition of preschool children and to assess the association of disease distribution with oral hygiene levels, reported oral health behaviours and socio-demographic factors. Study samples comprised 1250 3-year-old and 1283 5-year-old pre-school children from four distinct geographical areas in Flanders. Information on oral hygiene and dietary habits, oral health behaviours and socio-demographic variables was collected using questionnaires completed by the parents. Clinical examinations were performed using standardized criteria. Caries experience was recorded at the level of cavitation (d(3) level). Simple as well as multivariable logistic regression analyses were performed in order to identify factors associated with prevalence and severity of caries experience. Visible plaque was present in 31% of 3-year-olds and 37% of 5-year-olds. In 3-year-olds, 7% presented with caries experience while this was the case in 31% of 5-year-olds. Multivariable logistic regression revealed significant associations, in 3-year-olds, of caries experience with presence of dental plaque (OR = 7.93; 95% CI: 2.56-24.55) and reported consumption of sugared drinks at night (OR = 7.96; 95% CI: 1.57-40.51). In 5-year-olds, significant associations were seen with age (OR = 7.79; 95% CI: 2.38-25.43), gender (OR = 0.37 with 95% CI: 0.19-0.71 for girls), presence of visible dental plaque (OR = 3.36; 95% CI: 1.64-6.89) and reported habit of having sugar-containing drinks in between meals (OR = 2.60 with 95% CI: 1.16-5.84 and OR = 3.18 with 95% CI: 1.39-7.28, respectively for 1x/day and > 1x/day versus not every day). In 5-year-olds with caries experience (30.8% of total sample), the severity of disease was further analysed (d(3)mft between 1 and 4 versus d(3)mft 5 or higher). Multivariable analyses showed a significant association with gender [girls more likely to have higher disease levels; OR = 4.67 (95% CI: 1.65-13.21)] and with presence of plaque (OR = 3.91 with 95% CI: 1.23-12.42). Presence of visible plaque accumulation and reported consumption of sugared drinks were associated with prevalence of caries experience in Flemish preschool children. Severity of disease was associated with gender and with presence of plaque. Results underline the importance of plaque control and diet management from very young age on.

              Dental caries and associated factors in Mexican schoolchildren aged 6-13 years.

              The objectives of the present study were to establish dental caries prevalence (percentage with caries) and experience in the primary and permanent dentition (dmft and DMFT) of 6 to 13-year-old schoolchildren in Campeche, Mexico, and to estimate the contributing roles of the likely risk indicators. A cross-sectional study was carried out in 1,644 children aged 6-13 years. Self-administered questionnaires obtained information on social, economic, behavioral, and demographic variables. The primary dentition of 1,309 children and the permanent dentition of 1,640 children were evaluated in the oral examinations. The main outcome measures were DMFT, dmft, and SiC indices. Data were modeled using logistic regression analysis. The overall caries prevalence was 77.4%, 73.6% in the primary dentition (61.6% in 6-year-olds), and 49.4% in the permanent dentition. The dmft and DMFT indices were 2.85+/-2.73 and 1.44+/-2.05, respectively (DMFT = 3.11+/-2.62 in 12-year-olds). The SiC index was 6.05 at 12 years of age. Associated variables to dental caries in both dentitions were presence of enamel defects, presence of dental plaque, low socio-economic status, female sex, and older age. Mother's schooling was negatively associated (OR = 0.95) with caries in primary dentition. Caries experience in the primary dentition (OR = 6.02) was positively associated with caries in the permanent dentition. Dental caries status in these Mexican children was closer to the goals proposed by the WHO/FDI for 2000 than previous studies. This study has identified clinical, socio-economic, and behavioral determinants for dental caries in primary and permanent dentition on Mexican schoolchildren.
