      Development of an accreditation model for health education and promotion programs in the Iranian primary healthcare system: a Delphi study

          Abstract

          Background: Considering the lack of accreditation models for health education and promotion(HEP) activities in the Iranian primary health care (PHC) system we conducted the present study to develop a national accreditation model for HEP actions in the Iranian PHC system.

          Methods: After a comprehensive review on the accreditation models in PHC field, especially those concentrated on the HEP programs, an initial HEP accreditation model was developed.Then, applying the Delphi technique, 18 experts in the Iranian PHC system with field experience in HEP programs were invited to assess the initial model. In the two-round Delphi study,aggregation was provided on the opinions and the standards and indicators were finalized.Conventional content analysis was applied to make sense of the data collected in the study.

          Results: The developed HEP accreditation model encompassed 62 indicators and five standards.The standards were as follow: "resources for HEP programs", "educational needs assessment of the target groups", "methods of providing a community with education", "management of health volunteers’ actions" and "evaluation of HEP programs".

          Conclusion: The standards and indicators found in the present study may serve as an educational rationale for health educators while designing high-quality health education/promotion programs. This model may be helpful for health policy-makers and stakeholders while planning to assess the continuous quality improvement of HEP services delivered in the PHC systems.

          Primary health care networks in the Islamic Republic of Iran.

The establishment of the networks is outlined and the vital elements within the system described, such as the community health workers (behvarz) and the health information system.             Weaknesses and challenges of primary healthcare system in Iran: a review.

            While the primary healthcare (PHC) services in Iran were appropriate to the needs of the population in the late 1970s and 1980s, the changing burden of disease and shifting demand patterns have rendered the existing PHC system no longer suitable for meeting current and emergent needs. This has serious implications for the PHC system in Iran, which has clearly succeeded in addressing high levels of communicable diseases, maternal deaths and infant mortality, but appears less well prepared to address the emerging challenges of noncommunicable diseases (NCD). We conducted a systematic review of the available literature in the past 10 years related to the PHC system in Iran to assess its weaknesses and challenges. This paper categorizes PHC system weaknesses from the studied articles into two groups: (i) those related to the key functions of PHC, and (ii) others, which refer to health system weaknesses existing with the current PHC model. Iran can draw on international experience and evidence regarding interventions, which can be used to develop an effective and responsive PHC system designed to address current and emerging needs, in particular the NCD burden.

              Evaluation of accreditation program in non-governmental organizations' health units in Egypt: short-term outcomes.

              To determine the effect of accreditation of non-governmental organizations' health units on patient satisfaction and provider satisfaction and the output of accreditation on compliance to some accreditation standards. Sixty non-governmental health units were selected as follows: 30 units already submitted for accreditation in three governorates and 30 pair-matched units not programmed for accreditation. Matching was done according to the socioeconomic standard and administration type, and from the same governorate. Satisfaction was measured by an interview questionnaire using the Likert scale. Assessment of compliance to some accreditation standards was done using a checklist. Mean patient satisfaction scores were significantly higher among the accredited non-governmental health units regarding: cleanliness, waiting area, waiting time, unit staff and overall satisfaction. No significant differences were noticed in provider satisfaction except for the overall satisfaction score. Most of the checked standards had compliance above 90% in the accredited units and were significantly higher than in the non-accredited units. Accreditation of the non-governmental health units has a positive effect regarding patient satisfaction and the continuation of performance according to the accreditation standards compared with non-accredited health units. This short-term effect was shown within the first year from accreditation. Future research is needed to assess long duration effects of applying accreditation in non-governmental health units.
