      Evaluation of Medical Confidentiality Breaches on Twitter Among Anesthesiology and Intensive Care Health Care Workers

          Abstract

          BACKGROUND:

          With the generalization of social network use by health care workers, we observe the emergence of breaches in medical confidentiality. Our objective was to determine, among anesthesiology and intensive care health care workers, the rate of medical confidentiality breaches among professional tweets.

          METHODS:

          We performed a retrospective analysis of public Twitter data available through the official Twitter application program interface. The profiles of anesthesiology and intensive care professionals were identified thanks to keywords in their biography. All the tweets with a photograph and all the text-only tweets containing at least one specific keyword related to anesthesiology or intensive care were extracted. We selected only the tweets with a health care–related character. Then, we analyzed 10% of the tweets with a photograph and 10% of the text-only tweets extracted and noted those presenting a breach of medical confidentiality.

          RESULTS:

          After a first screening of 12,705 accounts, we manually analyzed 431 tweets with photograph(s) and 9000 text-only tweets from 1831 accounts. We found 44 (10.2%) breaches of medical confidentiality among the photographs and 76 (0.8%) among text-only tweets. These 120 problematic tweets came from 96 profiles (96/1831; 5.2%); 3.7% of North American profiles breached medical confidentiality versus 6.3% of profiles from other areas; P = .03. When comparing the distribution of the number of followers and tweets, accounts with breach of medical confidentiality tweets had more tweets and followers than profiles without (both P < .0001).

          CONCLUSIONS:

          We found a significant proportion of tweets with breach of medical confidentiality among anesthesiology and intensive care professionals accounts.

          Characterizing Social Media Metrics of Scholarly Papers: The Effect of Document Properties and Collaboration Patterns

          A number of new metrics based on social media platforms—grouped under the term “altmetrics”—have recently been introduced as potential indicators of research impact. Despite their current popularity, there is a lack of information regarding the determinants of these metrics. Using publication and citation data from 1.3 million papers published in 2012 and covered in Thomson Reuters’ Web of Science as well as social media counts from Altmetric.com, this paper analyses the main patterns of five social media metrics as a function of document characteristics (i.e., discipline, document type, title length, number of pages and references) and collaborative practices and compares them to patterns known for citations. Results show that the presence of papers on social media is low, with 21.5% of papers receiving at least one tweet, 4.7% being shared on Facebook, 1.9% mentioned on blogs, 0.8% found on Google+ and 0.7% discussed in mainstream media. By contrast, 66.8% of papers have received at least one citation. Our findings show that both citations and social media metrics increase with the extent of collaboration and the length of the references list. On the other hand, while editorials and news items are seldom cited, it is these types of document that are the most popular on Twitter. Similarly, while longer papers typically attract more citations, an opposite trend is seen on social media platforms. Finally, contrary to what is observed for citations, it is papers in the Social Sciences and humanities that are the most often found on social media platforms. On the whole, these findings suggest that factors driving social media and citations are different. Therefore, social media metrics cannot actually be seen as alternatives to citations; at most, they may function as complements to other type of indicators.

            Online posting of unprofessional content by medical students.

            Web 2.0 applications, such as social networking sites, are creating new challenges for medical professionalism. The scope of this problem in undergraduate medical education is not well-defined. To assess the experience of US medical schools with online posting of unprofessional content by students and existing medical school policies to address online posting. An anonymous electronic survey was sent to deans of student affairs, their representatives, or counterparts from each institution in the Association of American Medical Colleges. Data were collected in March and April 2009. Percentage of schools reporting incidents of students posting unprofessional content online, type of professionalism infraction, disciplinary actions taken, existence of institution policies, and plans for policy development. Sixty percent of US medical schools responded (78/130). Of these schools, 60% (47/78) reported incidents of students posting unprofessional online content. Violations of patient confidentiality were reported by 13% (6/46). Student use of profanity (52%; 22/42), frankly discriminatory language (48%; 19/40), depiction of intoxication (39%; 17/44), and sexually suggestive material (38%; 16/42) were commonly reported. Of 45 schools that reported an incident and responded to the question about disciplinary actions, 30 gave informal warning (67%) and 3 reported student dismissal (7%). Policies that cover student-posted online content were reported by 38% (28/73) of deans. Of schools without such policies, 11% (5/46) were actively developing new policies to cover online content. Deans reporting incidents were significantly more likely to report having such a policy (51% vs 18%; P = .006), believing these issues could be effectively addressed (91% vs 63%; P = .003), and having higher levels of concern (P = .02). Many responding schools had incidents of unprofessional student online postings, but they may not have adequate policy in place.

              The intersection of online social networking with medical professionalism.

              To measure the frequency and content of online social networking among medical students and residents. Using the online network Facebook, we evaluated online profiles of all medical students (n = 501) and residents (n = 312) at the University of Florida, Gainesville. Objective measures included the existence of a profile, whether it was made private, and any personally identifiable information. Subjective outcomes included photographic content, affiliated social groups, and personal information not generally disclosed in a doctor-patient encounter. Social networking with Facebook is common among medical trainees, with 44.5% having an account. Medical students used it frequently (64.3%) and residents less frequently (12.8%, p < .0001). The majority of accounts (83.3%) listed at least 1 form of personally identifiable information, only a third (37.5%) were made private, and some accounts displayed potentially unprofessional material. There was a significant decline in utilization of Facebook as trainees approached medical or residency graduation (first year as referent, years 3 and 4, p < .05). While social networking in medical trainees is common in the current culture of emerging professionals, a majority of users allow anyone to view their profile. With a significant proportion having subjectively inappropriate content, ACGME competencies in professionalism must include instruction on the intersection of personal and professional identities.
