      Thyroglossal duct cyst carcinoma in child

          Abstract

          Papillary thyroid carcinoma occurring in a thyroglossal cyst is a rare condition especially in children, and there is no consensus regarding management. There are only 10 other documented cases in the English literature for children under the age of 12. We discuss one such case. A 10-year-old female child with an 8-month history of a midline neck cyst underwent Sistrunk's procedure as surgical treatment after clinical and ultrasound scan confirmation. An incidental 9-mm papillary carcinoma was seen on histology within the thyroglossal cyst. Following multidisciplinary team (MDT) discussion, the child underwent total thyroidectomy and radioiodine ablation. There is no consensus regarding the ideal management for thyroglossal duct carcinoma in the paediatric literature. We discuss the treatment options and the importance of MDT involvement.

          Differentiated Thyroid Carcinoma of Children and Adolescents: 27-Year Experience in the Yonsei University Health System

          Thyroid carcinomas are uncommon in childhood and adolescence. The aim of this study was to analyze clinical features and clinical outcomes of thyroid cancer in the pediatric population treated in the Yonsei University Health System. From September 1982 to June 2009, 90 patients (75 females, 15 males; female:male ratio of 5:1) with differentiated thyroid carcinoma were identified in our institute. The mean age at diagnosis was 15.8 yr old (range 4.8-19.9 yr). Cervical masses were most common clinical manifestations at diagnosis in 65 patients (72.2%). Forty-two patients underwent less than total thyroidectomy and 18 patients underwent total thyroidectomy. Thirty patients (33.3%) had lateral neck lymph node metastasis and seven patients (7.8%) had lung metastasis at the time of surgery. Among the 90 patients, recurrence occurred in 14 patients (15.5%). Mean follow-up period for patients with differentiated thyroid carcinoma was 81.6 months (13-324 months). No patients died of differentiated thyroid carcinoma. Patients with differentiated thyroid carcinoma who were < 20-yr-of-age were present with aggressive local disease and a high frequency of lymph node and distant metastasis. It is recommended that pediatric thyroid cancer should be managed mostly using proper surgical approach with thyroidectomy and lymph node dissection when indicated.

            Thyroglossal duct carcinoma in children: case presentation and review of the literature.

            Carcinoma within the thyroglossal duct (TGDCa) is a very rare pediatric tumor. This report presents the case of a 15-year-old girl with TGDCa, reviews the previously published pediatric cases of this tumor, and provides diagnostic and therapeutic considerations in TGDCa in children. Twenty one cases of TGDCa have been reported, 12 of them in females. Mean age at presentation was approximately 13 years for females and approximately 12 years for males. The duration of a midcervical mass prior to the diagnosis of TGDCa varied from 3 weeks to 8 years. The size of the neck mass at presentation averaged 2 cm-4 cm. All masses were asymptomatic, and the diagnosis of TGDCa was incidental following surgery. All pediatric cases of TGDCa reported thus far were of the papillary type, except for 3 patients who presented with a mixed papillary-follicular carcinoma. Capsular invasion was detected in 10 (45%) patients. Local invasive disease was found in 5 (23%) patients, but all removed thyroid glands (12) were free of carcinoma. One patient had lung metastases. Thyroidectomy with subsequent radioiodine ablation was the treatment of choice in the majority of the cases. The reported prognosis for TGDCa in children was favorable, with only one reported death.

              Well-differentiated carcinoma of the thyroid.

              Thyroid cancer will be diagnosed in more than 20,000 individuals in the United States in 2002. Approximately 16,000 of these patients will be women. During the same year, an estimated 1300 deaths from thyroid cancer are expected. The various types of thyroid cancer include papillary carcinoma, follicular carcinoma, Hurthle cell carcinoma, medullary carcinoma, anaplastic carcinoma, and thyroid lymphoma. Papillary, follicular, and Hurthle cell carcinoma are considered well-differentiated thyroid cancers and constitute the focus of this article.
