          Abstract

          Background

The clinical importance of preoperative tumor markers remain elusive in gastric cancer. The aim of this study was to evaluate the prognostic value of AFP, CEA, CA19-9, and CA50 in T4a stage gastric cancer.

          Methods

Two hundred and seventy-three T4a gastric cancer patients who underwent curative D2 gastrectomy between 1996 and 2005 were evaluated. The correlation between tumor markers and clinicopathologic characteristics and prognostic value of preoperative tumor markers were investigated.

          Results

          Correlation analysis showed that AFP was associated with Borrmann type ( P = 0.010); CEA with sex ( P = 0.029), tumors site ( P = 0.014), and N stage ( P = 0.001); CA19-9 with age ( P = 0.047), tumor site ( P = 0.011), lymphovascular invasion ( P = 0.004), and N stage ( P = 0.000); CA50 with age ( P = 0.017), tumor site ( P = 0.004), tumor size ( P = 0.014), and N stage ( P = 0.000). Multivariate analysis showed that the positivity of preoperative CEA, CA19-9, and CA50 were major independent poor prognostic factors of patients with T4a stage gastric cancer.

          Conclusions

Preoperative serum tumor marker might be a candidate for the staging system in addition to conventional factors.

Japanese Classification of Gastric Carcinoma - 2nd English Edition -

PREFACE: The first edition of the General Rules for Gastric Cancer Study was published by the Japanese Research Society for Gastric Cancer (JRSGC) in 1963. The first English edition [1] was based on the 12th Japanese edition and was published in 1995. In 1997, the JRSGC was transformed into the Japanese Gastric Cancer Association and this new association has maintained its commitment to the concept of the Japanese Classification. This second English edition was based on the 13th Japanese edition [2].The aim of this classification is to provide a common language for the clinical and pathological description of gastric cancer and thereby contribute to continued research and improvements in treatment and diagnosis.

Circulating nucleic acids (CNAs) and cancer--a survey.

It has been known for decades that it is possible to detect small amounts of extracellular nucleic acids in plasma and serum of healthy and diseased human beings. The unequivocal proof that part of these circulating nucleic acids (CNAs) is of tumor origin, initiated a surge of studies which confirmed and extended the original observations. In the past few years many experiments showed that tumor-associated alterations can be detected at the DNA and RNA level. At the DNA level the detection of point mutations, microsatellite alterations, chromosomal alterations, i.e. inversion and deletion, and hypermethylation of promoter sequences were demonstrated. At the RNA level the overexpression of tumor-associated genes was shown. These observations laid the foundation for the development of assays for an early detection of cancer as well as for other clinical means.
