          Abstract

          Se presentaron 3 casos de pacientes con heridas penetrantes por arpones de pesca con retención intracraneal localizadas en la mitad anterior del cráneo con introducción más allá de la línea media del cráneo. Estos casos fueron atendidos en diferentes instituciones hospitalarias de Cuba y trasladados hacia éstas con el objeto in situ. En ausencia de sospecha de daño vascular resultaron suficientes para el diagnóstico y proceder, los estudios radiográficos simples de rigor. Aparece descrito el proceder quirúrgico aplicado a cada paciente: la combinación técnica de craneotomía-craniectomía con la intención de exponer el objeto en toda su extensión, en 2 de ellos y la combinación craniectomía-craniectomía en el tercero. Se demostró que el proceder quirúrgico a utilizar para retirar el objeto puede hacerse en dirección a su penetración o hacia atrás. El cirujano precisará cuál será el que menor daño vascular o del parénquima cerebral produce.

          Translated abstract

          3 cases of patients with penetrating wounds caused by spearsuns with intracranial retention located in the anterior half of the cranium with introduction beyond the medias line of the cranium were presented. These patients received attention at different medical institutions in Cuba and were transferred to them with the object in situ. As there was no suspcion of vascular damage, the routine radiographic studies were sufficient for the diagnosis and procedure. The surgical procedure performed is described. The technical combination of cranietomy-craniectomy with the purpose of exposing the object in all its extent was used in 2 of them, and the craniectomy-cranioctomy in the third. It was shown that the surgical procedure to be used to remove the object may be performed in the direction of the penetration or backwards. The surgeon will determine which one produces less vascular or cerebral, parenchyma damage.

          Analysis of 2187 consecutive penetrating wounds of the brain from Vietnam.

          A series of 2187 cases of penetrating wounds of the brain, treated in a U.S. Army Hospital in Vietnam, is analyzed according to operability, wounding agents, sites of cranial penetration, associated organ system injuries, operative and postoperative complications, and mortalities. A detailed description of the operative technique of thorough intracranial debridement and dural repair is presented and stressed. The previously established principles of combat neurosurgery are confirmed and their continued use recommended.

            Evolution of craniotomy as a debridement technique for penetrating craniocerebral injuries.

            A craniotomy debridement technique was recommended for penetrating craniocerebral injuries as early as 1940, in World War II. However, with due consideration for the bacterial contamination of penetrating injuries, the safety of this technique was questionable. The technique has been recommended in each succeeding war, but no data substantiating the safety or eventual sequelae have been available. Analysis of the data from the large Head Injury Registry of Vietnam casualties indicates that, in properly selected cases, debridement by craniotomy technique can be safe and efficacious.
