      Asociación entre índice de masa corporal y agresividad del carcinoma papilar de tiroides

          Abstract

          El objetivo del estudio fue evaluar la asociación del índice de masa corporal (IMC) con las características clínico-patológicas y la recurrencia del carcinoma papilar de tiroides. La cohorte consistió en 208 pacientes con carcinoma papilar de tiroides diagnosticado en 2003-2014, en Buenos Aires, Argentina. El seguimiento fue semestral los primeros 2 años y luego anual. Los pacientes fueron agrupados según el IMC de la siguiente manera: IMC < 18.5 kg/m² (bajo peso); IMC ≥ 18.5 y < 25 kg/m² (peso normal); IMC ≥ 25 y < 30 kg/m² (sobrepeso); IMC ≥ 30 kg/m² (obesidad). Dos patólogos experimentados revisaron todas las muestras para determinar las características del tumor y la extensión de la enfermedad. La recurrencia tumoral fue evaluada mediante diagnóstico por imágenes y confirmación histológica. Se utilizó un análisis de regresión para identificar la asociación del IMC con las características clínico-patológicas del tumor y con la recurrencia. El aumento de 5 puntos en el IMC se asoció significativamente con mayor tamaño tumoral (OR 1.21; IC 95% 1.1-1.5; p = 0.01) y con mayor extensión extranodal de las metástasis cervicales (OR 1.11; IC95% 1.06-1.21; p = 0.03). No se observó asociación entre el aumento del IMC y el riesgo de recurrencia (HR 1.11; IC95% 0.91-1.22). En conclusión, se demostró una asociación directa del IMC con el tamaño tumoral y la extensión extranodal pero no con la recurrencia del tumor.

          Translated abstract

          The aim of the study was to evaluate the association of the body mass index (BMI) with the clinical-pathological characteristics and the recurrence of papillary thyroid carcinoma. The cohort consisted of 208 patients with papillary thyroid carcinoma diagnosed in 2003-2014, in Buenos Aires, Argentina. The patients were grouped according to the BMI as follows: BMI <18.5 kg/m² (low weight); BMI ≥ 18.5 and < 25 kg/m² (normal weight); BMI ≥ 25 and < 30 kg/m² (overweight); BMI ≥ 30 kg/m² (obesity). Two experienced pathologists reviewed and cross-checked all pathology specimens to confirm diagnosis, tumor characteristics and extent of the disease. All patients were followed every 6 months for 2 years, and annually thereafter. Recurrences were searched by using diagnostic imaging and histological confirmation when necessary. Regression analysis was applied to define associations of BMI with clinical, pathological, and prognosis features of the disease. A 5-point increase in BMI was significantly associated with tumor size (OR 1.21; 95% CI 1.1-1.5; p = 0.01) and greater extranodal extension in cervical metastases (OR 1.11; 95% CI 1.06-1.21; p = 0.03). The analysis of prognostic variables showed no association between increase in BMI and risk of recurrence (HR 1.11; 95% CI 0.91-1.22). In conclusion, we found that BMI relates directly with tumor size and extranodal extension, but not with recurrence.

          International patterns and trends in thyroid cancer incidence, 1973-2002.

          During the past several decades, an increasing incidence of thyroid cancer has been reported in many parts of the world. To date, no study has compared the trends in thyroid cancer incidence across continents. We examined incidence data from cancer incidence in five continents (CI5) over the 30-year period 1973-2002 from 19 populations in the Americas, Asia, Europe, and Oceania. Thyroid cancer rates have increased from 1973-1977 to 1998-2002 for most of the populations except Sweden, in which the incidence rates decreased about 18% for both males and females. The average increase was 48.0% among males and 66.7% among females. More recently, the age-adjusted international thyroid cancer incidence rates from 1998 to 2002 varied 5-fold for males and nearly 10-fold for females by geographic region. Considerable variation in thyroid cancer incidence was present for every continent but Africa, in which the incidence rates were generally low. Our analysis of published CI5 data suggests that thyroid cancer rates increased between 1973 and 2002 in most populations worldwide, and that the increase does not appear to be restricted to a particular region of the world or by the underlying rates of thyroid cancer.

            Prospective study of hyperglycemia and cancer risk.

            To investigate whether hyperglycemia is associated with increased cancer risk. In the Västerbotten Intervention Project of northern Sweden, fasting and postload plasma glucose concentrations were available for 33,293 women and 31,304 men and 2,478 incident cases of cancer were identified. Relative risk (RR) of cancer for levels of fasting and postload glucose was calculated with the use of Poisson models, with adjustment for age, year of recruitment, fasting time, and smoking status. Repeated measurements 10 years after baseline in almost 10,000 subjects were used to correct RRs for random error in glucose measurements. Total cancer risk in women increased with rising plasma levels of fasting and postload glucose, up to an RR for the top versus bottom quartile of 1.26 (95% CI 1.09-1.47) (P(trend) <0.001) and 1.31 (1.12-1.52) (P(trend) = 0.001), respectively. Correction for random error in glucose measurements increased these risks up to 1.75 (1.32-2.36) and 1.63 (1.26-2.18), respectively. For men, corresponding uncorrected RR was 1.08 (0.92-1.27) (P(trend) = 0.25) and 0.98 (0.83-1.16) (P(trend) = 0.99), respectively. Risk of cancer of the pancreas, endometrium, urinary tract, and of malignant melanoma was statistically significantly associated with high fasting glucose with RRs of 2.49 (1.23-5.45) (P(trend) = 0.006), 1.86 (1.09-3.31) (P(trend) = 0.02), 1.69 (0.95-3.16) (P(trend) = 0.049), and 2.16 (1.14-4.35) (P(trend) = 0.01), respectively. Adjustment for BMI had no material effect on risk estimates. The association of hyperglycemia with total cancer risk in women and in women and men combined for several cancer sites, independently of obesity, provides further evidence for an association between abnormal glucose metabolism and cancer.

              Screening for Thyroid Cancer: US Preventive Services Task Force Recommendation Statement.

              The incidence of thyroid cancer detection has increased by 4.5% per year over the last 10 years, faster than for any other cancer, but without a corresponding change in the mortality rate. In 2013, the incidence rate of thyroid cancer in the United States was 15.3 cases per 100 000 persons. Most cases of thyroid cancer have a good prognosis; the 5-year survival rate for thyroid cancer overall is 98.1%.
