          Abstract

          ABSTRACT Objective: To assess the knowledge on oral hygiene, infective endocarditis, and antibiotic prophylaxis of parents of children and adolescents with heart diseases assisted at the Hospital Ana Nery (reference hospital), Salvador, Bahia, Brazil. Methods: Cross-sectional study with 43 guardians of children and adolescents aged 2 through 16 years, performed with the application of a questionnaire. Results: In the sample of parents studied, 64% had completed high school and 47% had a monthly family income of up to one minimum wage. The majority (81%) reported knowing that oral infections may lead to heart complications, 42% knew that oralbacteria may migrate to the heart, and 51% knew the term infective endocarditis. When questioned about antibiotic prophylaxis, 93% had no knowledge about it and 88% did not know the reasons for using antibiotics before dental treatment. Conclusion: Parental knowledge was limited and fragmented. Further attention is required when transmitting information to parents, focusing on prevention strategies in order to guarantee the integral health of these children and adolescents.

          RESUMO Objetivo: Avaliar o conhecimento sobre higiene bucal, endocardite infecciosa, e profilaxia antibiótica dos pais de crianças e adolescentes com cardiopatia congênita assistidas no Hospital Ana Nery, Salvador, Bahia. Métodos: Estudo transversal envolvendo 43 responsáveis de crianças e adolescentes de 2 a 16 anos, realizado através da aplicação de questionário. Resultados: Na amostra de pais estudada, 64% possuíam ensino médio completo e 47% tinham como renda familiar até um salário mínimo. A maioria (81%) afirmou ter conhecimento que infecções bucais podem levar a complicações cardíacas, 42% sabiam que bactérias da boca podem ir para o coração e 51 % conheciam o termo endocardite infecciosa. Quando questionados o que é profilaxia antibiótica, 93% não sabiam o que é e 88% desconheciam os motivos que levavam a necessidade do uso de antibióticos anteriormente a procedimentos odontológicos. Conclusão: O conhecimento dos pais mostrou-se limitado e fragmentado. É necessária maior atenção na transmissão da informação aos mesmos, focando em estratégias de prevenção a fim de garantir a saúde integral destas crianças e adolescentes.

          Epidemiology of congenital heart disease in Brazil

          Introduction Congenital heart disease is an abnormality in the structure or cardiocirculatory function, occurring from birth, even if diagnosed later. It can result in intrauterine death in childhood or in adulthood. Accounted for 6% of infant deaths in Brazil in 2007. Objective To estimate underreporting in the prevalence of congenital heart disease in Brazil and its subtypes. Methods The calculations of prevalence were performed by applying coefficients, giving them function rates for calculations of health problems. The study makes an approach between the literature and the governmental registries. It was adopted an estimate of 9: 1000 births and prevalence rates for subtypes applied to births of 2010. Estimates of births with congenital heart disease were compared with the reports to the Ministry of Health and were studied by descriptive methods with the use of rates and coefficients represented in tables. Results The incidence in Brazil is 25,757 new cases/year, distributed in: North 2,758; Northeast 7,570; Southeast 10,112; South 3,329; and Midwest 1,987. In 2010, were reported to System of Live Birth Information of Ministry of Health 1,377 cases of babies with congenital heart disease, representing 5.3% of the estimated for Brazil. In the same period, the most common subtypes were: ventricular septal defect (7,498); atrial septal defect (4,693); persistent ductus arteriosus (2,490); pulmonary stenosis (1,431); tetralogy of Fallot (973); coarctation of the aorta (973); transposition of the great arteries (887); and aortic stenosis 630. The prevalence of congenital heart disease, for the year of 2009, was 675,495 children and adolescents and 552,092 adults. Conclusion In Brazil, there is underreporting in the prevalence of congenital heart disease, signaling the need for adjustments in the methodology of registration.

            Knowledge and Oral Health Attitudes among Parents of Children with Congenital Heart Disease

            ABSTRACT Congenital heart disease (CHD) is a devastating complex of diseases resulting from defects of development. It affects more than 1 of every 100 live births. Early preventive dental care should be adjusted to the special needs of these children in their first years of life. Knowledge of parental attitudes and experiences of dental care are therefore important. Aim : This study was done to assess the knowledge and attitudes among parents of children with congenital heart disease towards oral health and dental care. Materials and methods : Parents (n = 105) of children with congenital heart disease of an age ranging from 0 to 16 years were included in the study. A questionnaire was used to assess the knowledge and oral health attitudes. Results : The parents’ knowledge was fair but the oral health attitudes were not very satisfactory. The parents in this study also recognized the importance of oral health for the well-being of rest of the body. Conclusion : The results of this study indicate that parents’ and children’s attitudes toward oral health and dental care need to be improved.

              The impact of oral health on the quality of life of young patients with congenital cardiac disease.

              The purposes of our study were, first, to determine the prevalence of dental caries in young patients with congenital cardiac disease, second, to determine the effect of oral health in their quality of life, third, to examine parental knowledge about associations between oral health and cardiac health, and fourth, to examine parental perceptions regarding access to dental care. A standardized questionnaire was given to a convenience sample of parents of 43 children with cardiac disease and 43 healthy controls from 12 to 71 months of age. In every patient, we performed a dental examination. Descriptive statistics, the two-tailed t-test, and Fisher's exact test were used for statistical analysis. Of the children with cardiac disease, 17% had caries, compared to 13% of the control group. Almost half of those with cardiac disease had never seen a dentist, compared to 35% of the control subjects. Of the parents of those with cardiac disease, one-fifth did not know whether oral health was important for the heart, compared to one-tenth of controls. Insurance and access to care were not barriers to obtain dental treatment. Statistically significant differences were, first, parents of children with congenitally malformed hearts felt more guilt about their child's oral health than control parents (p = 0.026), second, they were more upset about the dental problems and/or treatments of their children (p = 0.012), and third, they thought that dental problems and/or dental treatment made their children more irritable (p = 0.012). Our findings indicate that it is of paramount importance that cardiologists and their associated staff educate patients and families about oral health and other issues associated with congenital cardiac disease.
