          Abstract

          Resumen: Introducción: La malformación congénita menos frecuente de la vía biliar es la agenesia de la vesícula biliar; afecta a entre 0.007% y 0.09% de los pacientes sometidos a colecistectomía. Objetiv o: Presentar un caso clínico diagnosticado en el transoperatorio, previo diagnóstico de colecistolitiasis por ultrasonograma. Caso clínico: Femenino de 37 años, acudió a consulta de cirugía general presentando dolor abdominal de cuatro meses de evolución, tipo cólico, en el hipocondrio derecho, irradiado a la escápula derecha, postprandial. El ultrasonido de hígado y vías biliares reportó litiasis vesicular tipo 1. Se programó para colecistectomía abierta; no se logró identificar la vesícula biliar. Se finalizó el procedimiento, evitando lesionar la vía biliar. Se solicitó una colangiorresonancia en el postoperatorio, que confirmó el diagnóstico de agenesia vesicular. Discusión: El diagnóstico preoperatorio de agenesia vesicular nunca ha sido reportado; por lo tanto, resulta un reto para el radiólogo y el cirujano al realizar la colecistectomía. Conclusiones: La agenesia de la vesícula biliar es la malformación menos frecuente de la vía biliar, con muy pocos casos reportados mundialmente; su diagnóstico se realiza durante el transoperatorio, con interpretaciones erróneas en el ultrasonograma. La MRCP (colangiopancreatografia por resonancia magnética) es un estudio complementario cuando existe un ultrasonograma inconcluso; evita cirugías innecesarias y disminuye el riesgo de complicaciones.

          Abstract: Introduction: The less common congenital abnormality of the biliary tract corresponds to gallbladder agenesis, affecting between 0.007% and 0.09% of patients undergoing cholecystectomy. The purpose of this article is to present a clinical case diagnosed during the surgical procedure, after being diagnosed with cholecystolithiasis by ultrasonogram. Clinical case: A 37-year-old female consulted with abdominal pain that irradiated to the right scapula. The gallbladder ultrasound reported type l cholecystolithiasis. An open cholecystectomy was programmed. The gallbladder could not be identified and the procedure was finished in order to avoid harming the biliary duct. A cholangiopancreatography was requested in the postoperative, confirming the diagnosis of gallbladder agenesis. Discussion: A preoperative gallbladder agenesis diagnosis has never been reported, thus this is a great challenge to the radiologist as well as a great concern to the surgeon when performing a cholecystectomy. Conclusions: This is the less common abnormality of the biliary tract, very few cases have been reported worldwide; the diagnosis is made in the intraoperative period due to incorrect diagnostic impressions by the ultrasound. The MRCP is a complementary study that is useful when there is a doubtful ultrasonogram, avoiding unnecessary surgeries and reducing the risk of complications.

          Agenesis of the Gallbladder: Role of Clinical Suspicion and Magnetic Resonance to Avoid Unnecessary Surgery

          Isolated agenesis of the gallbladder is usually a rare asymptomatic anatomical variation, with an estimated incidence of 10–65 per 100,000. Females are more commonly affected (ratio 3: 1), with the disease typically presenting in the second or third decade of their life. Despite an absent gallbladder, half of the patients present with symptoms similar to biliary colic, which is poorly understood. The rarity of this condition combined with its clinical and radiological features often lead to a wrong preoperative diagnosis so that many patients undergo unnecessary operative intervention. Herein, we present the case of a 56-year-old female with a typical biliary colic who was diagnosed to have gallbladder agenesis. Computed tomography and magnetic resonance cholangiopancreatography allowed correct treatment and prevented an unnecessary intervention.
