      Paediatric burns secondary to nail adhesives: a case series

          Abstract

          Background

          Nail adhesives are widely available beauty products that are used in the cosmetic industry and households to attach false nails. Nail adhesive burns are uncommon, and it is crucial that healthcare providers and the public are aware of its potential to cause chemical burn.

          Case Presentation

          Case series of accidental burns secondary to cyanoacrylate nail glue treated at the Welsh Centre for Burns and Plastic Surgery (WCBPS) in Swansea, United Kingdom.

          Conclusion

          All of the burns were observed in children and occurred due to accidental spillage. Therefore, it is important for the public to be aware that nail glue is a chemical agent which should be stored safely away from the reaches of young children. The case series highlights the importance of first aid in reducing the severity of chemical burns secondary to the nail adhesives, and its early recognition and treatment are emphasised.

          Contact dermatitis to ethyl-cyanoacrylate-containing glue.

          3 patients with contact dermatitis to an ethyl cyanoacrylate glue are presented. Although reactions to cyanoacrylate glues are considered rare, more widespread use of these products by nail salons is likely to be associated with an increased incidence of positive reactions. All 3 of our patients came into contact with the glue during "nail wrapping". In this process, ethyl cyanoacrylate or another "instant glue" is used to adhere glue-impregnated silk or linen to the nail plate which is then filed to shape the nail. This procedure creates fine acrylic-containing dust which may facilitate an allergic response. Fine particulate matter may be transferred to other distant cutaneous sites, such as the eyelids, resulting in more widespread cutaneous eruptions. Dermatologists in areas where nail wrapping is becoming more fashionable are advised to be alert to potential cyanoacrylate glue allergies which present as periungual eczema which may be associated with eyelid dermatitis and features of nummular dermatitis particularly over the dorsal hand.

            Cyanoacrylate glue burn in a child--lessons to be learned.

            Cyanoacrylate is the generic name for cyanoacrylate based 'fast-acting' adhesives such as methyl-2-cyanoacrylate and ethyl-2-cyanoacrylate(commonly sold under trade names like Super Glue and Krazy Glue). Other forms have been developed as medical grade glues to be non-toxic and less irritating to skin tissue. Cyanoacrylate has a multitude of uses, ranging from simple domestic applications, to those for industrial purposes. It is an acrylic resin which rapidly polymerises in the presence of water (specifically hydroxide ions), forming long, strong chains, joining the bonded surfaces together. Cyanoacrylate sets quickly, often in less than a minute and a normal bond reaches full strength in two hours and is waterproof. This case report outlines an interesting case of a full thickness burn resulting from skin in contact with cyanoacrylate glue, including highlighting several areas where the management was not appropriate. This was likely due to the novel presentation of this patient, and lack of education of staff. Although there have been cases of burns resulting from this type of glue documented in the literature, there are none of this exact nature and depth. [See figure in text].
