      QUISTE BRONCOGÉNICO: REPORTE DE DOS CASOS Y REVISIÓN DE LA LITERATURA

          Abstract

          El quiste broncogénico es una malformación de la porción ventral del intestino primitivo que ocasiona alteración en el desarrollo del árbol traqueo bronquial. Reportamos dos casos de quistes broncogénicos en niños, ambos con síntomas respiratorios crónicos, uno de localización mediastinal a nivel paratraqueal derecho y, el otro, intraperenquimatoso en lóbulo superior derecho. La radiográfia y tomografía de tórax permitieron evaluar el tipo de lesión, localización y tamaño, así como la compresión de la vía áerea. Ambos pacientes fueron intervenidos quirúrgicamente no se presentaron complicaciones y los síntomas respiratorios desaparecieron. El estudio histopatológico confirmó el diagnóstico.

          Translated abstract

          Bronchogenic cyst arises from abnormal budding of the diverticulum of the foregut that leads to abnormality of the tracheobronquial tree. We report two cases of these cysts in children with respiratory chronic symptom. One was localized in paratraqueal mediastinum and other, intrapulmonary in lower upper right side. The chest radiography and computed tomography allowed evaluation as to the type, site and size of lesion as well as airway compression. Both patients underwent surgery; there were no complications and the respiratory symptoms disappeared. Histologic features confirm the diagnosis.

            Congenital lung disease: a plea for clear thinking and clear nomenclature.

            Antenatal ultrasound has allowed the discovery of abnormalities which in the past often escaped detection. It may be unclear what advice to offer. Many large lung malformations seen at the routine 20-weeks gestation scan have largely disappeared at term. There is now even more confusion in terms of how these malformations should be described. The nomenclature of congenital lung disease was never very clear, with terms such as sequestrated segment, cystic adenomatoid malformation, hypoplastic lung, and malinosculation being used to describe often overlapping abnormalities. However, today these terms are used inconsistently in the ante- and postnatal periods. For example, congenital cystic adenomatoid malformation (CCAM) is used prenatally to describe a lesion which may well disappear before birth, but is used postnatally to describe an abnormality which may require lobectomy. CCAM may have a pulmonary arterial supply, or be supplied like a sequestration from the aorta, and histological features of both lesions may coexist. Thus, a complete reappraisal of the nomenclature of congenital lung disease is timely; this review does not discuss treatment options.

              Congenital masses of the lung: prenatal and postnatal imaging evaluation.

              Congenital masses of the lung are a spectrum of interrelated abnormalities that includes congenital lobar overinflation, bronchogenic cyst, congenital cystic adenomatoid malformation (CCAM) and sequestration. The prenatal and postnatal imaging features of these lesions are reviewed, emphasizing the importance of serial prenatal sonograms and postnatal imaging studies, including radiography and computed tomography. Masses that become inconspicuous, or disappear on serial prenatal sonograms are discussed, as well as the importance of postnatal imaging studies in the evaluation of these lesions. Finally, the management of congenital masses of the lung is reviewed, emphasizing the importance of imaging studies in the preoperative evaluation.
